Employee was operating a fuel truck underground and accidentally hit a rib causing EE's knee to hit against the dash in the cab causing injury.
While conveying a load of rock from the shuttle car into the feeder breaker a rock rolled over the side rail of the conveyor from the shuttle car's throat hitting EE's right hand.
EE could see a cable could be hit by the loader bucket. EE stepped alongside the bucket to quickly move cable. The bucket slid to EE's side and struck EE's ankle. EE received a fractured ankle.
EE went to adjust the sampler brake. EE disengaged the sampler brake and then lifted the cover so EE could pull on the sampler drive belt to get it into position. EE tried to move the sampler forward the sampler did not want to move so, EE moved it in reverse. The sampler must have moved far enough to trigger the position sensor and the system turned on catching EE's right arm.
EE was using a rake to clean up the accumulations that were under the North Mains #1 Belt, the belt was approximately a foot and half off the ground. As EE put the rake under the belt the rake handle came into contact with the belt which then pushed the rake and EE's left hand into a bottom roller causing a laceration on EE's left ring finger
EE was unloading belt supplies from BB-203 at XC-7 in 6 North while unloading the supplies another EE was in BB-219 moving the truck out of the haulage way, while backing up BB-219 the rear fender caught EE's self-rescuer causing EE to get twisted between the two trucks.
Employee was hanging life line and got ankle caught between the rib and a mine vehicle causing injury. The truck backing up in motion was a contributing factor to the accident.
EE was operating controls on material hauler and placed other hand in pinch point on moving part.
While a mechanic was trouble shooting a Utility Vehicle, they accidently pulled forward and the trailer wheel smashed miners calf.
EE reached into engine compartment with motor running on skid steer to tighten nut on fuel line. Left hand was holding wrench and right hand was resting on fan shroud, when they leaned in right hand little finger contacted running fan, causing laceration.
After rock dusting activities, employee was placing the hose into the bucket of the scoop. At this point, the bucket was inadvertently lowered onto the injured's right foot. The injured was taken to the hospital where x-rays revealed no broken bones. EE was prescribed days off work.
Employee was firebossing and adjusting the air on section and a shuttle car bumped into the employee and the employee fell against the rib and felt pain in back and legs.
Loader operator dropped bucket on Employee's boot while Employee was loading a vent tube in bucket.
Loading 8" x 10" x 11" H-beam onto a flat bed trailer with fork lift. The injured attached a chain to the beam to reposition it on the trailer. When the fork lift raised off the bed, the chain slipped causing the beam to roll hitting pinning ee right hand between the beam and trailer resulting in an amputation of the tip of the middle and index fingers.
EE was positioned in between a parked utility vehicle (UV) and a trailer that was attached to the UV. The UV was running and lunged backward when a second employee adjusted the idle of the UV. EE was pinched between the rear bumper of the UV and the tongue of the trailer. Fracture of right leg.
The employee was watering roadways. When she made a left turn from a crosscut to the entry the water car contacted the rib. The employee had her left foot outside of the cab line and her left ankle was fractured.
