While operating a haul truck, the operator hit a rough spot in roadway jarring their back. Did not report incident until 6-2-24. Evaluated by occupational doctor on 6-6-24 and given restrictions that cannot be accommodated. Became lost time injury on Friday, 6-7-24.

Mostrando los 9Employee was moving rigging employee had unhooked from a boom truck headache ball. Employee signaled the operator to hoist up to pull the strap from under the load. When the strap came out, the operator stopped hoisting and the headache ball swung back and struck the employee on the left shoulder, resulting in a contusion to the shoulder.
Operator injured shoulder & upper back some time while operating or climbing onto end dump. On Thursday, July 18th operator was taken to a clinic and diagnosed with a shoulder & upper back strain. Operator was prescribed physical therapy, over the counter pain meds and placed on work restrictions that could be tolerated making this an MSHA recordable.
A privately owned vehicle traveling east bound on public highway (FM 696) lost control and crashed through the fence onto mine property. The victim was ejected from vehicle and died at the scene. The victim was not a miner and did not work for Luminant Mining.
Hauler was being loaded. The loader struck the side of the hauler, causing motion in the hauler resulting in lower back pain to the hauler operator. Initial doctor visit resulted in no restrictions. Injury became recordable when employee was prescribed and received physical therapy on 10-08-2018. 7 sessions prescribed. No restrictions or lost days.
A group of mechanics were working to change out a conveyor gearbox. The mechanics were using a crane to lift the gearbox pedestal slightly, so it could be shifted into position. During this process, one of the lifting eye bolts broke, and the bolt head flew up and struck one of the mechanics in EE's mouth. EE suffered cuts a fractured #7 tooth and cuts to EE's lips and mouth.
An operator was operating a Cat 992G Front End Loader feeding coal into the hopper. The operator ran over a chunk of coal that contained petrified wood in it jarring their lower back. The operator started physical therapy on 4/26/17 and will have 6 sessions making this injury reportable to MSHA.
On 02/03/2017, a dozer operator reported that on 01/24/2017 while ee was operating a front end loader in the pit down #5 ramp ee hurt lower back causing pain as ee hit some bumps. The operator does not know when exactly ee hurt back. The doctor gave the operator restrictions which did not allow ee to operate heavy equipment.
Operating empty coal hauler traveling down haul road. Operator apparently dozed off, the hauler contacted a berm on the l side of the road, traveled the top of the 60' long berm, went off the l side of road, crossed a ditch and came to stop about 407' off the haul road. Operator received a cut to the back of head requiring sutures. EE stated was wearing seat belt.
