Employee noticed hill belt was off and drove EE's personal pickup to where another employee was working on no. 2 head drive. As employee passed, No. 3 head drive EE's pickup slid sideways on slick patch of snow. Striking berm and overturning.

Employee was tramming a scoop when something flew into EE's right eye, dust or dirt.

Employee was operating a man bus that struck a timber lying beside the track. The timber came inside deck on the man bus and struck him in the right thigh.

Employee was cleaning last cross cut with a scoop when he ran over a bucket of plaster. The bucket burst and some of the material hit him in his right eye.

Employee was operating a scoop at the end of the track scooping up a motor to load on a flatcar. He backed the scoop in a break, when he did he backed over a timber 6x6x8 ft long. His head hit the side post of the canopy on the scoop.

Employee was being loaded in the pit area when a rock rolled out of the overburden being loaded and struck the left corner of the truck bed. This caused the employee to be thrown around in the seat. The seat belt pulled him causing pain in lower back and hips.

Employee was scooping up gob with a scoop, when a piece of structure that was in gob hit him on the right shoulder and right side of his neck. bruising his neck.

Employee was mounting Front End Loader when he felt pain in his right hip and knee. Employee didn't report this accident until 9/8/2014 and started losing time on this date..

Operating a loaded tractor trailer coal truck the drive wheels were spinning and a weak transmission mount allowed the shift lever to move excessively smacking the operators hand starting missing on 12/20/2013

Employee was operating shuttle car when he hit a hole in roadway and bounced up and hit his head on the canopy of the shuttle car.

Employee was watching belt run to trim a splice and a piece of the belt line came around and hit him in the face and cut his chin. He received 10 stitches on his chin. Came back to work the next day.

Driving a rock truck hit a hole in the road felt pain in lower back. Started missing work on 6/18/2013 and returned on 6/22/2013.

Driver stated truck stalled on hill started backward down hill lost control of truck trailer went in ditch and over turned the trailer.

Employee was operatting Johnson Ride when the tire hit a cinder block. The steering wheel spun around and hit his left forearm and fractue it.

Employee was operating fork lift placing beams on high wall miner table. When the high wall fell on fork lift he received a laceration to the head and a compound fracture to right arm.

EE had loaded a rock duster he walked back to get water off of scoop, the scoop was moving and rolled over the end of his right foot.

while lowering the bucket on the loader, it stopped half way down jarring him.

Operating scoop pulling tool car had hand out of scoop deck backing up smashing hand between scoop and material (rock and coal)laying on the bottom.

Employee stated that while mounting the fork lift his foot slipped off the third step and he fell to the ground

Another EE was running scoop unloading a new scoop bucket pulled bucket on the ground. EE pulled up to slack the chain, IE stumbled in front of scoop that EE was running mashing IE leg in between the buckets.

Employee was getting in SUV to return to work area when the driver pulled away to quickly causing employee to loose footing causing pain in right knee. Employee started losing time on 10-23-12.

Employee was riding in SUV Man Bus when it hit a dip in road causing pain in back. Employee started losing time on 7/9/12

Employee was loading a bag of rock dust on fork lift when he stepped in front of fork lift and the tire ran over his right foot.

EE was operating a miner when a piece of draw rock fell off of the conveyor and cut his lower left leg. He received 9 stitches came back to work the next day.

Employee was cleaning #5 heading the bottom was busted up scooped middle and right side was backing up out of heading to clean left side. Ran over a piece of rock scoop landed real hard putting head into canopy and sent sharp pain down into lower back and left leg.

Employee was standing next to a scoop when the scoop moved. It struck his left foot.

Repetitive Motion

Employee was operating rock truck going up a hill. The truck spun out and slid backwards straining his back. Employee did not report it to his supervisor until 12/27/11 and went off work on 1/2/2012.

Employee stated that while driving the coal truck he reached over to adjust the radio and drove into the ditch

A non-collision roll over accident occurred. When unit CT 135 was topping out a uphill left hand turn when the truck experienced a non collision roll over. The driver received 6 stitches over his right eye.

Employee stated that while operating the shuttle car, he hit a hole in the roadway and bumped his head against the canopy.

Employee was getting shuttle car loaded when shuttle bounced and his head hit the canopy of the shuttle car.

Employee was working on cable reel on a shuttle car. When he turned the reel on the cable tripped him hurting his left knee.

EE was hauling coal from surface mine in tractor trailer turned the truck over on it's side rounding Gator Bowl Curve.
