Employee was riding on the passenger side of a rubber tired man trip, when they rode over a 2" discharge line causing the end to flip up and strike him in the face. It caused a laceration to his lip.

Employee was moving the S/C down the belt out of the way of the bolter when he hit a bump in the bottom causing him to strike his head on the canopy.

Employee states that while operating a 785b, he was backing under the loader when the 992d bucket struck his cab protector. He states that this occurred several more times during this shift. Employee continued to work until 12/04/2013 when he began to miss. He is complaining of low back pain.

The safety glasses that he was wearing prevented the link from directly striking him in the eye, but the safety glasses caused a laceration to the eye brow area.

Employee was driving, the diesel scoop company #388 motor first outby, when the brakes locked up. This Caused the employee to hit his left shoulder on the steel plate in the cab. A wire had come off the park brake pressure switch which caused the brakes to set.

Employee states that while operating the center car, he hit a hole in the road and it jarred him. He felt a pain in his lower back.

Mechanic was working on the brakes of a Coal Hauler when the brake retainer became lodged in the brake cavity. As he was attempting to un-lodge the retainer it came out striking him on the right had resulting in the middle finger on his right had getting a inch and ahalf laceration on it.

EE states that as he was tramming a rail ride through the air lock doors, one of them started closing on him. The rail ride caught one of the doors causing a cinder block to be displaced and fall. The block struck his hand causing an laceration.

Employee was scooping roadway in wall # 10. When scooping outby corner, the corner broke off and came into scoop deck and smashed/cut employee's left thumb.

Employee was assisting shuttle car operator move his anchor. Employee started the S/C. The anchor and cable took. The end of the anchor turned striking injured in the left hand and left ring finger.

Employee was hooking chain up to tool car and scoop to pull tool car closed to section. He told the scoop operator to lower the scoop bucket. The operator dropped the bucket and injured screamed that the bucket was on his foot.

Employee was tramming his shuttle car to the feeder where he trammed thru bad bottom and a hole in #7 right x cut too fast and it bounced him, hurting his lower back.

Employee was helping recover track rails when one of the track rails slipped in the scoop bucket and caused the other end of the rail to hit him in the right ankle.

Employee was helping to load high voltage cable into a sled. His foot slipped on a rock, causing his foot to slide under the sled. The sled was being pulled by a 35C Fairchild scoop.

EE was coming through the red light toward the prep plant. He turned his windshield wipers on and the lights from the plant blinded him, causing him to strike the rock beside the pond.

EE WAS TRAVELING ACROSS HAUL ROAD ENROUTE TO MINE SITE. EE LOST CONTROL OF HIS VEHICLE IN A CURVE ON SLICK ROAD AND HIT A COAL TRUCK TRAVELING IN OPPOSITE DIRECTION. (PRE-SHIFT ACCIDENT; EE DID NOT SEEK MEDICAL TREATMENT - INJURED HIS LEFT KNEE.)

MINER BACKED UP AND RAISED BUGGY UP, WHEN MINER PULLED UP. BUGGY SLAMMED DOWN CAUSING JASON TO HIT HIS HEAD.

SHUTTLE CAR WAS TRAMMING AWAY FROM MINER TOWARD FEEDER, S.C. OPERATOR WAS MAKING A LEFT TURN, RAN INTO CORNER OF RIB, CAUSING OPERATOR TO LUNGE FORWARD STRIKING RIGHT SHOULDER AGAINST CANOPY P OST.

INJURED WAS OPERATING SHUTTLE CAR WHEN HE HIT HOLE IN ROADWAY CAUSING HEAD TO HIT CANOPY JAMMING NECK AND BACK.

EE WAS HAULING DIRT OUT OF PIT. WHILE PROCEDING UP INCLINE, TRUCK HIT RUT, CAUSING EE TO JAM FOOT AGAINST BRAKE, CAUSING FRACTURE OF FOOT.

WHILE TURNING A RIGHT HAND CROSS CUT, MINER BACKED UP HITTING S.C.PUSHING IT INTO RIB. EE HIT RIB, BRUISING HIS HIP.

EE WAS RIDING IN MOTOR. CANOPY HIT TOP. ROCK CAME OFF TOP OF MOTOR AND HIT HAND.

WHILE HAULING COAL ON A S.C. EMPLOYEE HIT HOLE IN MINE FLOOR, CAUSING PAIN TO LOWER BACK.
