Driving tractor down the slope when the engine stalled and the brakes would not engage for approx. 75 feet. The tractor hung into a water hose that was hooked to a fire outlet. The hose stretched pulling the fire outlet loose and throwing it forward hitting employee in the head and hand.
Employee was driving a battery hauler from #3 unit to the battery station and ran over a rough place in the road causing a whiplash effect to his neck.
Employee was reclaiming belt structure and caught his finger in a pinch point between a scoop and the scoop bucket. He is on Restricted duty.
Employee was riding in a diesel mantrip going down the slope. When the mantrip got to the bottom the roadway was slick and as the driver tried to make a left hand turn, the mantrip slid into an I-Beam, causing the mantrip to come to an abrupt stop. This caused the employee to slide forward, striking his arm on the metal seat.

Another shuttle car came down the entry and did not see the personnel carrier sticking out of the crosscut about 4 feet, he hit it which in turn caught the employee's leg between the equipment.

EE was operating a scoop and using a cable to pull a conveyor chain back together in a shuttle car. The cable broke and struck EE in the face.
EE WAS DRIVING SHUTTLE CAR TOWARD CONTINUOUS MINER AND RAN OVER A ROCK. EE WAS THROWN UPWARD AGAINST THE CANOPY, HITTING HIS HEAD AND JAMMING HIS NECK.
EMPLOYEE WAS UNHOOKING TRAILER FROM TRACTOR WHEN THE TRACTOR ROLLED A LITTLE & CAUGHT HIM BETWEEN THE TRACTOR & TRAILER. RT. SHOULDER WAS BRUISED BY IMPACT.
Employee was leaving the dumping point, in a shuttle car, and was looking back at his boom as he lowered it to make sure it cleared some cables hanging across the haul road. His car veered to the right and as he passed a parked golf cart, it struck his right arm which he had outside the operator's compartment.
EE WAS PASSENGER IN MANTRIP WHICH LOST POWER COMING UP THE MAIN SOUTH SLOPE. DRIVER STARTED PUMPING THE FOOT BRAKE BUT IT FAILED TO HOLD THE RIDE AND THEY STARTED ROLLING BACKWARD. DRIVER PUT IT IN PARK, WHICH DID NOT HELP. DRIVER THEN SET OFF THE PARKING BRAKE BUT THEY HAD HIT THE COAL RIB ABOUT THIS TIME, INJURING EE'S RIBS.
EE was working on a Mantrip in the x-cut between the #4 & #5 entry when the unit scoop turned into the x-cut on them. He did not see them working on the mantrip. He struck the injured party cutting his right leg below the right knee.
THE INJURED PARTY WAS DRIVING TO THE #1 UNIT ON THE MAIN EAST WSUPPLY ROAD WHEN HE RAN OVER A ROCK IN THE ROAD THROWING IT UP IN THE AIR STRIKING HIS LEFT LEG.
AS THE INJURED PARTY WALKED DOWN BESIDE THE #1 CAR HE NOTICED THE CAR CABLE UNDER THE CAR. HE REACHED DOWN & JERKED THE CABLE OUT FROM UNDERNEATH THE CAR WHEN THE CABLE REEL KICKED IN JERKING THE CABLE ACROSS HIS FACE INJURING HIS JAW & MOUTH.
WHILE TR4AMMING THE 480 S & S SCOOP THE INJURED PARTY EITHER RAN OVER A ROCK OR A HOLE INT HE ROAD CAUSING THE INJURED PARTY TO JAM HIS NECK INTO THE SCOOP CANOPY. THIS HAPPENED ONT HE 5-6-01. THE INJURED PARTY DID NOT GO TO THE DR TIL 5-13-02.
INJURED PARTY WAS SCOOPING THE RIGHT HAND RIB WITH THE 488 SCOOP AND AS HE WAS TRYING TO SCOOP THE RIB THE VENTILATION CURTAIN WAS GETTING CAUGHT UP ON THE CANOPY. INJURED PARTY REACHED OUT O F THE CANOPY TO MOVE THE CURTAIN AND AS HE DID HE GOT HIS LEFT INDEX FINGER CAUGHT BETWEEN THE RIB AND THE SCOOP CANOPY.
INJURIED PARTY HAD JUST LEFT THE ROOF BOLTER TO GO LOOK FOR SOMETHING WHEN HE WENT THROUGH A BACK UP CURTAIN THE SAME TIME THE SCOOP GAME THROUGH STRIKING HIS RIGHT FOOT.
