Employee was driving down the 3rd 54 supply road when he hit his head on a roof bolt, and this caused him to jam his neck.

Employee was trying to hold curtain out of the way so he could see to scoop area and caught his fingers between the canopy and a damaged roof bolt, injurying his ring finger and amputating his little finger.

Employee was using the feeder to pull a tailpiece back when one of the hooks broke causing the tailpiece to turn sideways and hit the employee knocking him against the coal rib causing a cut on his elbow.

Employee was riding in a diesel mantrip and as it made a turn his foot was caught between the mantrip and the coal rib, fracturing a bone on top of his right foot.

Employee was hanging a piece of wing curtain that had fallen down and the scoop operator was cleaning the entry. He was traveling batteries first and could not see the employee that was hanging the curtain. The rear tire of the scoop ran over the right foot of the curtain hanger causing a fracture of the small bone in the ankle.

Driving tractor down the slope when the engine stalled and the brakes would not engage for approx. 75 feet. The tractor hung into a water hose that was hooked to a fire outlet. The hose stretched pulling the fire outlet loose and throwing it forward hitting employee in the head and hand.

Employee was driving a battery hauler from #3 unit to the battery station and ran over a rough place in the road causing a whiplash effect to his neck.

Employee was reclaiming belt structure and caught his finger in a pinch point between a scoop and the scoop bucket. He is on Restricted duty.

Employee was riding in a diesel mantrip going down the slope. When the mantrip got to the bottom the roadway was slick and as the driver tried to make a left hand turn, the mantrip slid into an I-Beam, causing the mantrip to come to an abrupt stop. This caused the employee to slide forward, striking his arm on the metal seat.

Employee was unhooking a cable from the back of a diesel ride when the ride rolled back and the bumper caught his foot.

Riding in Getman Tractor and as the tractor made a turn he put his hand in a pinch point and caught his hand between the frame of the tractor.

Employee was changing seat direction while driving a shuttle car and twisted his knee. Did not become reportable until 10-22-08.

Riding on mantrip going to the working section and the mantrip hit a bump in the road causing the employee to hit his head on the mine roof. This was not reported to management until 8-4-08.

Ran over rock in road while driving a golf cart causing the steering wheel to turn suddenly, twisting his wrist and side.

Employee was walking to his ride when a suttlecar cable hit him on the knee. He did not miss work but was on restricted duty for 14 days. He returned to full duty 1-8-08.

Employee was riding in a mantrip and when the mantrip made a turn, he caught his foot between the mantrip and the coal rib.

Employee was hanging a miner cable, when the miner backed into his shuttle car, causing the car to hit him on the leg.

Employee was removing a block curtain when a shuttle car turned into the x-cut hitting his left foot with the shuttle car tire causing ligament damage.

EE had just dumped his car at feeder and was turning around in operator's compartment, when another car cable anchored next to him, jumped to the side hitting him on the right side of the head, driving his head into edge of his car canopy. Resulted in lacerations, requiring stitches.

Employee was emptying his car in the feeder when another car came on the dump and hit his car causing him to strain his back.

EE was helping splice belt and usually a piece of diesel equipment was out of service. EE got a golf cart to use for transportation. While traveling down inclined travelway, cable broke on braking system. EE jumped off golf cart and bruised his left hip.

Employee was unloading pump motor from personnel carrier and had positioned himself between a shuttle car and the personnel carrier. Another shuttle car came down the entry and did not see the personnel carrier sticking out of the crosscut about 4 feet, he hit it which in turn caught the employee's leg between the equipment.

Employee was driving 14-man personnel carrier (mantrip bus) and ran over something in roadway causing him to bounce up into mine roof and hurt his neck.

Bolter operator was tramming the bolter with his leg hanging out of the deck when a shuttle car ran into the bolter catching his left leg - causing a compound fracture to the lower leg.

EMPLOYEE WAS OPERATING (DRIVING) A 4-MAN DIESEL MANTRIP. AS HE NEGOTIATED A TURN, THE POWER STEERING FAILED AND CAUSED HIM TO RAISE UP AS HE TRIED TO TURN, RESULTING IN HIS HARD HAT STRIKING THE MINE ROOF.

EE was turning around in deck of shuttle car when he caught his left foot under the seat and twisted his left knee.

EMPLOYEE WAS HELPING RECLAIM HIGH VOLTAGE CABLE AND LOADING IT ONTO A TRAILER WHICH WAS BEING PULLED BY A 4-MAN DIESEL PERSONNEL CARRIER. EMPLOYEE WAS NOT AWARE OF HIS POSITION IN RELATION TO THE EQUIPMENT AND GOT HIS HEAD CAUGHT BETWEEN FENDER AND MINE ROOF.

EE was operating a scoop and using a cable to pull a conveyor chain back together in a shuttle car. The cable broke and struck EE in the face.

EE WAS DRIVING SHUTTLE CAR TOWARD CONTINUOUS MINER AND RAN OVER A ROCK. EE WAS THROWN UPWARD AGAINST THE CANOPY, HITTING HIS HEAD AND JAMMING HIS NECK.

EMPLOYEE WAS UNHOOKING TRAILER FROM TRACTOR WHEN THE TRACTOR ROLLED A LITTLE & CAUGHT HIM BETWEEN THE TRACTOR & TRAILER. RT. SHOULDER WAS BRUISED BY IMPACT.

Employee was leaving the dumping point, in a shuttle car, and was looking back at his boom as he lowered it to make sure it cleared some cables hanging across the haul road. His car veered to the right and as he passed a parked golf cart, it struck his right arm which he had outside the operator's compartment.

EE WAS PASSENGER IN MANTRIP WHICH LOST POWER COMING UP THE MAIN SOUTH SLOPE. DRIVER STARTED PUMPING THE FOOT BRAKE BUT IT FAILED TO HOLD THE RIDE AND THEY STARTED ROLLING BACKWARD. DRIVER PUT IT IN PARK, WHICH DID NOT HELP. DRIVER THEN SET OFF THE PARKING BRAKE BUT THEY HAD HIT THE COAL RIB ABOUT THIS TIME, INJURING EE'S RIBS.

EE was working on a Mantrip in the x-cut between the #4 & #5 entry when the unit scoop turned into the x-cut on them. He did not see them working on the mantrip. He struck the injured party cutting his right leg below the right knee.

THE INJURED PARTY WAS DRIVING TO THE #1 UNIT ON THE MAIN EAST WSUPPLY ROAD WHEN HE RAN OVER A ROCK IN THE ROAD THROWING IT UP IN THE AIR STRIKING HIS LEFT LEG.

AS THE INJURED PARTY WALKED DOWN BESIDE THE #1 CAR HE NOTICED THE CAR CABLE UNDER THE CAR. HE REACHED DOWN & JERKED THE CABLE OUT FROM UNDERNEATH THE CAR WHEN THE CABLE REEL KICKED IN JERKING THE CABLE ACROSS HIS FACE INJURING HIS JAW & MOUTH.

WHILE TR4AMMING THE 480 S & S SCOOP THE INJURED PARTY EITHER RAN OVER A ROCK OR A HOLE INT HE ROAD CAUSING THE INJURED PARTY TO JAM HIS NECK INTO THE SCOOP CANOPY. THIS HAPPENED ONT HE 5-6-01. THE INJURED PARTY DID NOT GO TO THE DR TIL 5-13-02.

INJURED PARTY WAS SCOOPING THE RIGHT HAND RIB WITH THE 488 SCOOP AND AS HE WAS TRYING TO SCOOP THE RIB THE VENTILATION CURTAIN WAS GETTING CAUGHT UP ON THE CANOPY. INJURED PARTY REACHED OUT O F THE CANOPY TO MOVE THE CURTAIN AND AS HE DID HE GOT HIS LEFT INDEX FINGER CAUGHT BETWEEN THE RIB AND THE SCOOP CANOPY.

INJURIED PARTY HAD JUST LEFT THE ROOF BOLTER TO GO LOOK FOR SOMETHING WHEN HE WENT THROUGH A BACK UP CURTAIN THE SAME TIME THE SCOOP GAME THROUGH STRIKING HIS RIGHT FOOT.
