Employee drove his 775E haul truck over rough ground at the dump location and strained his arm as he held onto the steering wheel.

Employee was attempting to pull a piece of four inch pipe on the stockpile when the pipe got caught between drip and other pieces of four inch. The employee was then thrown off the ATV which then fell over on the top him.

Employee was traveling to the SX on the access road when an oncoming contractor vehicle swerved to miss some deer on the road colliding head on with the employee. Employee was transported to the hospital via ambulance. Employee suffered from whiplash and contusions to her chest area. She was given pain medication and taken off work for two days.

Employee was operating a skid steer loader with a scraper attachment. She was back dragging fine crushed material when the scraper blade caught on some structure and brought her to a stop. The abrupt stop resulted in pain to the neck and shoulder area.

Employee's scraper came to a sudden stop when the slack was released after the hook up. Afterwards, employee began to feel some discomfort in his neck and head. He was transported to the ER where he was checked out. He was taken off work. RTW 2/21/07.

Employee was attempting to adjust the forks on the forklift when his hand became caught between the mast and the fork causing a contusion to his right hand.

Employee was loading pulley onto truck using forklift when pulley rolled off of forks and smashed employee's fingers between bearing housing and torch box.

Employee was driving water truck w/air seat, truck went over bump and employee's hard hat hit roof of cab causing him to bite down hard and fracture tooth

During shift change Employee was moving toward the back of the 140-0 van to be seated. The individual driving the van began to move while employee was still upright causing them to fall. The driver of the van didnt stop to make sure employee was in the seat.

The operator of the 564 HT was backed up in a trench in an attempt to dump his truck. After dumping out the operator pulled out of the trench and in the process the front of his truck came off the ground. When the truck descended it jarred the operator.

The operator of the 520 HT was turning around on the 5700 L Dump. The steering wheel whipped back around catching the operator's left thumb pulling it back.

EE called the 402 and said that she hit a pot hole on the way to the 10 shovel. She was having pain in her neck and back.

While employee was assisting the operator of the forklift, they were in the process of lifting a gear using a lifting sling. Both employees were focusing on the position of the forks and the sling and did not notice the position of the legs and the wheels of the extenda-fork forklift when it came in to contact with one of the employees ankles.

While stopped and waiting for a backhoe to make a right turn the ME-69 pickup was hit in the back by an Empire Suburban.

Employee was sitting in haul truck getting loaded when the loader bucket struck his truck hard causing the truck to rock from side to side and jarring his back.

While the ME-69 pick up was stopped in the road waiting for a back hoe to make a right turn an Empire Suburban ran into the back of the ME-69 causing it to be shoved into the back of the CC-84 pick up that was stopped waiting for the backhoe to turn.

Man lift operator was finshing placing steel. He was retracting the boom and when he started to lower the boom stick the man lift tipped over striking a metal beam approx 19 feet in the air coming to a stop. The man lift was tipped up onto the counter weight.

Operator of the 539 HT was turning right to go to the 62 SH. While in the turn he hit a large pothole that broke his drivers side mirror. The operator then got a 65 SH assignment and headed that way. The operator felt no pain until he was near the 5280 Fuel dock. He pulled over at the 65 SH and called for a supervisor. Operator went to the clinic for observation.

Operator of the 535 HT reported that her bed was bouncing up and down when empty resulting in neck and back pain.

Employee was having trouble dumping the 570 haul truck out on the King 6170 high grade dump. Another operator came and helped the employee dump the haul truck. While dumping the load the employee was jarred.

The operator of the 506 haul truck was loading at the 63 shovel. When he pulled out of the shovel pit he began to experience back pain.

Employee was taking banding outside to the dumpster when he waved the forklift operator down so he could get employee another bundle. As forklift operator was leaving, the banding got caught u nder the tire running over employee's left foot. On Going.

While returning to operations at shift change, the van hit a pot hole and employee hit her head on the roof of the van, causing neck and lower back pain. He was wearing seatbelt. She was taken to the ER, referred to her personal physician, and has been taken off work until completion of physical therapy. ONGOING

WHEN LOWERING THE BED OF TRUCK 6072 769 HAUL TRUCK SLAMMED DOWN AND CAUSED THE DRIVER A NECK SPRAIN.

Employee was traveling to the fill loaded when he hit a low spot in the road. When employee felt pain in back. Employee was taken into the ER and was given two days off work. Employee then went on regular days off and returned on next regular scheduled shift on 8/19/06.

Employee was sitting in the passenger seat of the 543 Haul truck when the truck hit a rut in the road. She felt discomfort in her back.

The 512 Haul Truck was leaving the SRC dump, as he was making the turn down the ramp he started to slide, he then tried to correct the slide and slid into the 521 Haul Truck who was coming up on the loaded side of the ramp.

EE was digging in bank with loader. A rock came down and hit loader bucket. This jarred the loader. He complained of discomfort in rib area. He asked to go home, and no other action was taken. ***DID NOT SEEK MEDICAL ATTENTION TILL 7/29/06.***

Employee was driving Mack dumptruck downhill. He lost brake pressure while trying to downshift. The employee tried to manuever the truck around a corner and the truck tipped and fell over on the driver's side.

Employee was operating a 793 Haul Truck. The operator hit a bump and felt a sharp pain in upper back.

EE was sitting in his 769D dump truck waiting to be loaded. The loader operator hit the wrong control and the loader boom came down hard on the dump truck bed twice causing EE's back to pop. Employee did not report incident until 5/27/06 and quit his job on 5/29/06 but was scheduled off from the time of his release on 5/28/06 until 5/31/06.

The 523 was traveling loaded off of Coronado headed down the Northwest ramp. While in the ore lane at approximately the 5150 level the engine compartment caught fire. Employee sprained ankle while dismounting the truck. Fire was immediately extinguished by the truck's fire suppression system.

Operator of the T542 struck a berm at the King leach 5650 after dumping out,in route back to a shovel.

Operator was leaving the North Stacker pad to end night shift. The operator decided to go down along the 13B. Traveling down from the head the operator was unable to make the left hand turn in a downhill descent. While making the left hand turn the operator got the CC-113 pick-up parallel with the berm and rolled the vehicle down the embankment.

A truck was returning to maint. shop from the mill heaping east on south side of maint. shop. EE was operating a forklift hauling west. The truck was on the S. side of the hydrant and did not see the forklift. Both were still moving when they hit. Trying to reach Dr. for claificatio of discrepancy between written report and release he gave EE. D&A performed on 4/6/06 were neg.

Employee was returning to Tyrone from Chino in the S1 and was rear ended at the corner of Hudson and Broadway, causing a cervical strain. Employee was taken to the hospital and given prescriptions for pain and release to work on 4/6/06.

Water truck was traveling down E ramp when lost brakes operator tried to berm it on L side of the road to stop it. When Truck struck L berm ir swerved to the R and came in contact with berm on R side of road and turned over on driver side. EE had injured knee and some abrasions EE went to ER Dr gave him time off.EE was terminated

Employee was cleaning the left side catwalk on the tail of the P5 conveyor. When the air blasters at the head of the P4 conveyor discharged a 3 inch rock was ejected and decsended down towards the tail of the P5 striking the employee in the lower back.

A haul truck was traveling empty from the pit. It struck the berm on the pit edge with its left front tire. Skid marks indicated the truck slid about 4' as a result. The truck then traveled about 200' before stopping half way up the berm. The driver was taken to the hospital with visible head injuries. Initial investigation presumed he fell asleep and was not wearing his seat belt.

While operating a 631, # 12 scraper, EE was loaded and headed to the fill. As he came out of the 8 ditch onto level ground, he lost control of the scraper and ran over a windrow on the left side of the road. After traveling about 20 yards, EE got control and crossed the windrow back onto the road to his right and pulled the scraper off the road to the right side of the road.

Employee was operating the 115 tire service truck and was returning to the tire shop from a callout. As he was traveling down the North West extension haul ramp he lost control of the vehicle and turned it onto its side.

DRIVER OF WATER TRUCK WAS HAULING WATER TO JOB SITE WHEN HE ENCOUNTERED ICE ON ROAD WAY CAUSING ENGINE BREAK TO STALL THE TRUCK ENGINE, RESULTING IN THE DRIVER LOSING CONTROL OF TRUCK. THE TRUCK OVER TURNED. DRIVER COMPLAINED OF PAIN IN LEFT SHOULDER, LEG AND NECK.

While loading under a 994 loader, the first bucket was put into the truck causing it to bounce up and down. Employee felt pain in his back a couple hours later. It was not turned into the supervisor until the end of the shift. Details from medical review were not turned into PDBI until 1/23/06.

While operating a haul truck, employee states he received a rough load and 3 hours later experienced neck/shoulder discomfort.

On 8/17 EE fell to ground while boarding truck. Slight pain in back, dr gave him dose of pain med, released to work. On 8/28 he told his supervisor he'd hit pothole previous day, aggravating back. Went to dr, given full release. Worked reg. schedule till 9/11, called off w/back pain. On 9/12 notified supervisor dr had taken him of work for 30 days.

The shovel dipper made contact with the bed of the haul truck the employee was operating, causing a jarring motion.

Hoist brakes on shovel did not hold dipper as it was loading the truck. The dipper fell and struck the bed of the truck, raising the bed into the air and jarring the truck driver. NOTE: Employee continued to feel pain, therefore he was sent for re-exam. This exam revealed a need for physical therapy elevating this incident from first aid to medical treatment.

Operator was being loaded at a shovel when a rock fell off of the shovel dipper and struck his truck.

Employee was working on a haul truck tire in an elevated platform. A coworker moved the platform, causing it to drop 3-4 feet. Employee was holding an impact wrench (30-40 lbs.) when the platform dropped.

While positioning shovel dipper over haul truck bed, a rock fell into the truck bed.
