Employee was assisting shop welder with hydraulic hammer housing being repaired. Employee used overhead crane to lift one end of hammer housing to enable cleaning and new seal prep. A lifting eye that was tack-welded to the end of the housing, broke free causing the hammer housing to fall to the floor and crush the employee's toes. Employee was wearing safety toed shoes.

Loader was dumping material in bed of haul truck when a large boulder dropped out of loader bucket into haul truck bed and caused haul truck to shift abruptly. Thus, resulting in a strain to haul truck operator's neck and shoulders.
Was working on the SB2 with a loader when some of the SB2 fell and landed on the loader bucket causing the loader to bounce. Employees head hit the dome light and cut the top of his head.
THE TRUCK DRIVER RAN OVER A BUMP IN THE HAUL ROAD CAUSING HIM TO BE JARRED INSIDE THE TRUCK WHICH CAUSED HIM BACK PAIN. DRIVER WAS GIVEN AN IMMEDIATE RETURN TO WORK STATUS WITH "NO BENDING GREATER THAN 6 TIMES PER HOUR" RESTRICTIONS. FOLLOW UP APPOINTMENT SCHEDULED FOR 9-18-2013
EE was being loaded with material when the bucket of the loader came down and hit the bed of the haul truck. EE was jarred when the bucket hit.
Bobcat was doing clean up at the hopper when haul truck backing up to dump didn't see the bobcat & bumped the back of bobcat with tire. Employee's neck was hurting.
Backing haul truck under loader bucket. The hyd. failed and the bucket came down on bed of truck. Ee twisted his neck.
Washing mud out of gyro, when belt was started up a rock fell out of gyro & hit the belt & bounced off hitting ee's little finger on handrail.

EE WAS OPERATING A WATER TRUCK AND HIT POTHOLE IN ROAD AND JARRED HIS BACK.
PIN CAME OUT OF LIFT ARM ON TERMINATOR AND BOOM FELL FORWARD AND JARRED EMPLOYEE IN LOADER.
