While cleaning ground below and adjacent to a troughing belt, a 4 inch diameter stone fell from a height of 11 feet and struck the employee on the right shoulder. Contusion to right shoulder resulted.

Claims he unbuckled seatbelt ,while haul truck was being loaded with shot rock, to get a water bottle. The force of the rock being loaded caused him to be bounced out of the seat and he hit his head on the top of the cab. Strained neck muscles. Also claims he hit his head earlier on a piece of conduit he forgot to duck.
WHILE APPLYING BELT DRESSING EE STATES HE PLACED HIS HAND ON BELT JUST BEFORE IT WAS STARTED UP. BELT STARTED, PULLED HIS FINGERS AND CAUGHT THEM BETWEEN BELT DRIVE AND SHEAVE. RECEIVED RX, FRACTURED TIP OF FINGER.
EE ATTEMPTED TO DISLODGE 2 ROCKS FROM THE FEEDER HOPPER. HE DIDN'T TURN OFF BELT FIRST. HE REACHED IN & THE ROCKS CAUGHT HIS FINGER REMOVING SKIN FROM TIP. EE RECEIVED ONLY FIRST AID AT FIRST. HIS FINGER WASN'T HEALING PROPERLY & HE HAD TO THEN HAVE SKIN GRAPHS ON 9/16/02.
