EE was getting out of the 933 skid loader and got right index finger stuck in the door handle that is located on the window. The window of the 933 is the door. Employee was opening the door and getting up at the same time. Finger got caught in the latch and twisted finger on handle before letting go. Door is hard to open.

EE WAS DIRECTING THE 939C OPERATOR WHILE HE WAS PULLING A PIECE OF HDPE PIPE. THE CHOKER SLIPPED OFF THE PIPE. WHEN THE PIPE MOVED, IT STRUCK EE'S LEGS AND KNOCKED HIM TO THE GROUND, SCRAPING HIS LEFT CHEEK AND FRACTURING HIS NOSE.
1.INJURED WAS TRAVELING WITH THE BUCKET IN THE RAISED POSITION.2.DENSITY OF THE PIECE THAT WAS IN THE BUCKET. 3. PIECE LANDED ON THE CONCRETE TOP AND LINER PLATE BOLT. 4. SIZE OF THE LINER PLATE BOLT. 5. LINER PLATE BOLT FLEW INTO THE BOBCAT CAB.
INCIDENT NOT REPORTED UNTIL 6/28/04 (AFTER EE HAD BEEN TERMINATED). EE HAD NOT SOUGHT MEDICAL TREATMENT AT THAT TIME. HE INFORMED CO 7/12/04 THAT HE IS GOING TO SEEK TREATMENT FOR SORENESS IN HIS SHOULDER. EE ALLEGES WHILE OPERATING A JLG MANLIFT, HE CAUGHT THE LIFT WHILE RAISING IT. AS IT BROKE FREE THE LIFT JERKED & HE WAS JOSTLED AROUND. LATER HE FELT SORENESS IN SHOULDER.
THE OPERATOR WAS BACKING DOWN A HILL WITH A LOADOF PLASTIC PIPE; HE RAISED THE BOOM OF THE FORK LIFT TO A POSITION THAT COMBINED W/ THE ANGLE OFTHE HILL, CAUSING THE FORKLIFT TO TURN OVER BACKWARD. HE HAD 5 WKS. OPERATION EXPERIENCE FOLLOWING TRAINING AND WAS WEARING HIS SEAT BELT
