Employee was driving CAT 777 rock truck with an air ride operator's seat. According to the operator, the seat had plenty of air at the beginning of the shift. The air ride seat lost air throughout the day. The employee hit a rough spot in the road and the seat bottomed out causing them to jar lower back and tailbone area.

Employee was operating an empty rock truck back to the pit via the right contour haul road and met a loaded rock truck travelling in the opposite direction. Employee pulled to the side of the road to let the truck by. The rock truck traveled over an irregularity in the road at a high rate of speed, jarring the employee into the right armrest of the operators seat.

An employee received a fracture to the right wrist while driving a 2 man personnel carrier over the mine track system.

TRUCK RAN INTO DITCH AND FLIPPED OVER.

EE was going back to the mine to get loaded. Came upon a soft pushed up spot in the Rd caused by winter thawing. Driver did not slow down and make adjustments for Rd. conditions causing himself to be bounced in the ceiling of the truck he was operating. EE was driving to fast for Rd conditions

While turning a curve around #6 head, the mantrip was pulled toward the rib due to the front tire being caught in a rut. The mantrip struck a timber causing it to become dislodged. It fell and struck employee in the left ear, causing a laceration to the ear and a bruise to the head just behind the left ear.

Pushed clutch down heard a pop in left knee. When he could stop he got out of the truck and could not put weight on left leg.

The injured employee was operating a 4 wheeled battery ride when he started through a check curtain. The curtain caught a bundle of plastic pogo sticks (tied together) and forced the end of the bundle onto the injured employees leg at the same time becoming lodged against the top. The result was a fracture of the lower leg.

Employee stated he was riding on the passenger side of a small personnel ride, on his way outside, and hurt his lower back

Employee was crossing belt when he slipped and fell on the belt. Employee was transported on moving belt 1500' when he stopped the belt. Received a fracture to the L2 lumbar vertebra. Mine was idle from 12/22/11- 01/03/12

IEE was standing outside his truck talking to other drivers. He was waiting on coal. A car driven by another EE came around the small curb said sun was in his eyes and hit my driver causing injuries to his front teeth, lower lip, his right hand and knees.

Employee was operating a scoop in an entry in close proximity of line curtain. Attempting not to tear the curtain, the Employee reached out to push the curtain back. While doing so the employee's right hand was pinched between an off set in the rib and the scoop canopy. Resulted in crushing injuries to the employee's fingers. The middle finger right hand was later amputated.

Employee was operating a small battery-powered transportation vehicle when it ran over a rut or obstruction in the roadway causing the steering wheel to slip from his grip and spin. The steering knob struck him on the hand.

A scoop was being used to pull belt through the tailpiece and belt drive. The wire rope connected to the scoop broke causing the rope to fly back. The rope struck the employee across the forehead and cheek.

Driver fell asleep, steering off to the left of the roadway. Got too close to a sump hole, the edge gave way causing to truck to partially overturn. The truck was going uphill loaded. The driver suffered bruising & stiff neck.

Drove through fly pad & leg got caught & bent back and broke them.

Employee was operating a shuttle car when his hat was pulled off his head as he crossed under a cable hung from the roof. This startled him and caused him to turn too sharply as he attempted to go into the crosscut. His left shoulder came in contact with the rock rib.

EE started down hill at 19 marker towrd Haney Bro. Tk. Lot loaded. EE tried to shift to 3rd gear. EE said truck went into gear and immediatley came back out. EE set park brake, hand brake and pushed foot brake. Truck wouldn't stop, causing it to get out of control, then struck another truck and over turned. EE said brakes wouldn't stop the truck.

Employee was operating a small battery-powered vehicle. The vehicle struck a bag of rock dust, causing the steering wheel to spin and the steering knob to strike the employee's right hand.

Employee in TC Transport truck, states he was on 7-Pines Surface and in dodging a rock truck, hit a pot hole and seat bottomed out.

Employee was placing tools onto a small battery powered vehicle, when it rolled backwards onto his foot. The employee operating the vehicle pulled the vehicle off the employee's foot.

A scoop was being used to pull several loops of continuous mining machine cable. One of the ropes attaching a loop of cable broke and the cable recoiled against the employee's left knee.

Driver was going down tipple hill in 2nd gear, saw a T.C. transport truck on straddle berm and stopped truck, set park brake. Felt truck move a little before park brake set up, pushed park brake button back in releasing brake and truck ran away. He had truck out of gear resulting in truck overturning on side.

Employee got his index finger on left hand caught between canopy of scoop & rib causing the finger to be amputated to second knuckle.

Employee was riding in the mantrip, when it hit a hole, jarring him. He felt pain in right upper body.

Employee was operating a battery powered vehicle when it struck a piece rock in the roadway. This caused the steering wheel to rotate quickly and the steering knob struck the employee on the right hand.

Employee was rock dusting the entry using a scoop with a rock dusting machine in the bucket. The center broke into on the scoop causing the employee to be bounced against the mine roof.

Employee was operating a 777 Cat rock truck and while backing into the dump the back berm failed causing truck to go through berm. Truck rolled onto driver's side and slid down dump.

Employee was backing up in a 4 wheel battery ride. He jammed his head into the roof at a point where the roofline transitioned lower from the entry to the crosscut.

Employee was riding in a diesel rubber tired mantrip. As the mantrip made a right hand turn the end of his right foot was momentarily caught between the rib and the canopy support post.

Employee received a laceration requiring sutures when he struck a bit bucket. Employee was operating a 2 man ride and bent sideways to go under a shuttle car cable.

Employee was operating a 35 C Fairchild supply scoop and hit a hole in roadway causing him to jar in the seat, and he felt pain in the lower back.

Employee experienced pain in his back while operating his shuttle car. He jarred against the seat and canopy as he traversed over rough bottom (i.e. holes, ledge, etc.). This is a recurring pain for this employee caused by the repeated trauma of operating in adverse roadway conditions. This is being reported as an occupational illness. (8/29/06) represents the 10th working day.

Employee was operating a shuttle car and hit a hole in the roadway causing him to jar his lower back.

Employee received fractured ribs when the shuttle car he was operating struck a hole in the haulage way. Sudden bounce caused him to be jarred in the seat.

Injured and a coworker were riding a battery powered 4 wheeled vehicle in the primary intake entry. The employee was operating the vehicle. As they were traveling, the hood of his jacket snagged on a metal roof support strap and this pulled his head toward the roof. His ear contacted something on the roof resulting in a laceration.

Employee was operating a Johnson 4 man personnel carrier in reverse when the front tires struck the coal rib. The sudden stop of the tires against the rib caused the steering wheel to spin and the knob on the wheel struck the employee on the left hand causing a fracture.

Rail runner went through 1st set of doors, skidding through second set of doors that were closed.

Employee was operating a battery forklift near the end of the track unloading rockdust and contacted the mine roof with his left ear and side of head while backing up. This resulted in a laceration to his left ear that required 12 sutures.

EE, a miner operator, was helping install a buggy anchor. After the anchor was installed, EE bent down to pick up a piece of key steel from the roof bolter drill boom. The center shuttle car took up slack on the cable one break inby, causing the anchor to strike EE in the chest. EE sustained a cracked sternum and is currently Lost Time.

EMPLOYEE WAS TRYING TO GET S/C ON THE OUTBY SIDE WHILE IT WAS LOADING. HE TRIPPED ON TRAILING FALLING DOWN. WHEN EE GOT BACK UP ON HIS FEET THE S/C BEGAN TO TRAM TOWARDS THE FEEDER, STRIKING HIM ON BOTH LEGS BELOW THE KNEE, KNOCKING HIM ON HIS BACK. EE GRABBED ONTO THE BUMPER OF THE S/C DRAGGING HIM APPROX. 5' BEFORE STOPPING.

Employee stated that he struck his head on the scoop canopy as he was transporting a power center in the scoop bucket. The power center became dislodged when the scoop trammed over a hill and caused the scoop to bounce. Employee recieved a laceration when he struck the canopy

(B) UNDERGROUND LOCATION: SECTION BELT TAIL. EE WAS ATTEMPTING TO REMOVE A CRIB TIE FROM UNDER THE FEEDER AS IT WAS BEING TRAMMED BACK ONTO THE TAIL PIECE. WHEN THE EE PLACED HIS HANDS ON THE CRIB TIE THE FEEDER TILTED BACK ONTO THE CRIBS. THE EE'S 3RD & 4TH FINGERS R. HAND WERE CAUGHT BETWEEN CRIB TIE AND MINE FLOOR - TIPS SEVERED. EE DID NOT HEAR THE ALL CLEAR SIGN.

EE WAS RIDING ON A MANTRIP TO THE SECTION AND TURNED TO WARN OTHER WORKERS TO BE AWARE THAT THE MANTRIP WAS CROSSING UNDER AN OVERCAST. AS THE EE TURNED BACK HE STRUCK THE OVERCAST WITH HIS FO REHEAD RESULTING IN A LACERATION. SUTURES WERE APPLIED TO CLOSE THE WOUND.

DRIVER WAS ON THE STOCKPILE, TRUCK FACING DOWN HILL. TRUCK BED WAS FULLY RAISED. DRIVER PULLED FORWARD AND STOPPED. BED STARTED ROCKING FROM SIDE TO SIDE, THEN FELL OVER. CAB STAYED UPRIGHT. B ED WAS ON THE (DRIVERS SIDE) GROUND, ON ITS SIDE. STOCK PILE WAS LEANING TO THE DRIVERS SIDE.

EMPLOYEE COMPLAINED OF PAIN IN THE LOW BACK AFTER STRIKING A HOLE IN THE ROADWAY WITH A SHUTTLE CAR. ON 8/5/2003 EMPLOYER WAS NOTIFIED THAT THE EMPLOYEE SOUGHT MEDICAL TREATMENT FROM A CHIROPA CTOR ON 7/16/2003.
