Mostrando los 11EE positioned a cap wedge between the flight conveyor chain & the drive sprocket on the vessel & then instructed the plant operator to jog it. The chain broke causing the flight to reverse direction and pinch EE's left arm between the flight and the chute. EE was transported to CAMC for x-rays but they were unable to test due to swelling. EE is currently off work awaiting MRI.

While operating a 4 seat mantrip, injured claims to have run over a rock in the roadway. When he ran over the rock, he claims to have felt pain in his back. He continued working until 1-31-09, at which time he went to the dr and the dr removed him from work for 3 days.
EE was running SC and hooked back fender in #7 entry. When EE turned corner his canopy caught the corner, broke off a piece of rib and it came in the deck and hit his head and left leg, causing injury.
While tramming scoop, employee hit hole jamming his head into canopy of scoop. Accident was reported on 4/3/08.
EE was moving shuttle car in #6 entry, so ribs could be rock dusted. When shuttle car caught a rib board which flew back and struck him in the right ear and check.
