EE foot struck by forklift resulting in a fracture
Softrock Haul Truck Operator was operating a CAT 777G Haul Unit too close to berm on left side of road and ran into berm knocking off left wheel of Haul Unit. Truck rolled over onto its left side. Operator jumped out of truck injuring left arm, elbow and left foot.
Employee reported the bed of EE's 777 truck slammed while retracting after dumping the load at the main crusher. EE did not feel any pain until April 28th, 2023. The initial report of injury did not require any restrictions, however, on EE's second visit, after an MRI EE received restrictions on 05/08/2023
Employee claims they had some issues with their back about three weeks ago but reported it today.
On 6/18 employee was involved in a vehicular collision which resulted in minor back pain to lower back. Employee did not want to seek medical attention on that date. On 6/24 employee was still experiencing back pain and was taken to the doctor where EE was placed on restricted duty.
Employee was driving company truck (Ford F150) and veered off a paved road and hit a concrete barrier causing airbags to deploy striking face and chest. Employee sustained a cracked sternum and broken nose.
While operating a loader, the miner hit bumps in the road, potholes, etc. which jarred and injured EE's back.
EE was loading used wire rope into a dumpster. Forklift become unstable and turned over. EE received a laceration to the head that required medical attention, stitches. EE returned to work on the next scheduled shift. MSHA site was down for service Feb. 15 thru Feb 19.
Employee was operating a McCloskey ST110 Portable Stacker via remote control and ran over ee left foot with the left track of the machine, breaking 2nd and 3rd metatarsal bones in foot.
Employee was operating a haul truck and the bed of the truck was lifted by loader when the bucket pushed down material at the end of the bed. Bed lifted and came down shaking the truck.
While driving a forklift the employee ran over a pot hole, the lift jumped and the employee strained back.
EE was operating a forklift at the cement plant finish mill area during a ball mill changeover. While moving a stack of pallets from finish mill area to storage area outside, he accidentally ran over a wooden pallet with rear wheel of forklift. When the pallet broke under the weight of wheel, a piece of wood flew in the air inside the cab and struck EE in mouth.
While driving a manlift employee hit a small ditch bounced lost balance and his right foot slept out of the basket impacting the toe guardsand cutting his calf
A maintenance technician was operating a forklift most of the shift carrying empty oil drums from one part of the plant to another. The road that the forklift traveled was bumpy road with several pot holes. Toward the end of the shift, the technician started feeling pain in his lower back but could not remember a single event to cause the pain.
Employee informed that he felt "a little stiff" in the morning. He did not attribute it to any particular event at this time. We logged the incident in the first aid register at that time and followed up with him later that Friday. However on Tuesday morning 07/22/2014 he complained again of stiffness in his back. He was taken to a clinic. He was diagnosed with a strain back.
Employee complained about back pain on Wednesday 12/11/2013. He claimed that he felt OK through the weekend and Tuesday, but on Wednesday he reported some discomfort. He never reported this but until Wednesday 12/11/2013
