Employee was in the process of loosening a hydraulic hose on a low trac on the 004 MMU. While loosening the hose the pallet forks leaked off squeezing EE's head against the tilt arm of the low trac and the low trac frame. This resulted in 6 stitches.

Employee was sitting in the rear seat of the mantrip. During investigation it was determined EE re-adjusted EE's position in the seat and contacted a bolt installed in the mine roof.

Employee alleged they were sitting on mantrip and raised up to adjust their belt. Through investigation, it was determined the employee was positioned on their knees and raised up to get their dinner bucket when they came into contact with the metal beam, resulting in a laceration requiring 4 stitches.

Employee was a passenger (driver's side rear) on a 4-man mantrip traveling outside at the end of shift. The mantrip hit a hole in the road, measuring 36" wide x 36" long and 2-3" in depth, in the travel way and the employee shouted to stop. When the mantrip stopped the employee fell out of the ride. The employee complained of lower back pain, and numbness in left leg.

Torque Contractor was coming off dump hill and hit jake brake. Acted like it went from 3rd stage to 1st stage and jerked EE's neck. First day missed was 12/10/2024.

Employee was adjusting the width on the fork spacing to pick up a pallet of block with fork lift on lo-trac. When sliding fork in, EE caught EE's left ring finger between the fork and the frame of the fork lift on the lo-trac. This resulted in 3 stitches.

Contractor was riding in mantrip. EE got up on knees to adjust SCSR and to put coat on and grabbed side of mantrip. Mantrip was pulling out and smashed EE's hand between the mantrip and rib on the corner.

Employee stuck a finger between a moving roller and a sensor mounted on structure. This resulted in a fracture to the left index finger and a laceration requiring 5 stitches.

Employee pulled 12-man mantrip up to intersection to hook up cable and didn't set park brake or turn the ride off. Employee got in front of ride with back toward the ride. The ride began to roll and contacted lower part of left leg resulting in a fracture.

Employee came in contact with scoop causing a laceration to the back of EE's lower right leg.

Operator was carrying a large rock out of pit. The loader turned down grade and rear came off the ground causing rock to come out of bucket which resulted in loader bouncing and bouncing the operator in the cab causing excessive strain on EE's left shoulder.

EE was operating a mini trac lifting the belt up to prep for a move. The suspended belt dropped approx. 16" when the machine pushed on a rope attached to the belt. The dropping belt made contact with the EE's head in a sitting position on the lo trac and pushed EE forward suddenly which resulted in a hyperextension and displacement and fracture of #4 vertebra.

Driver was dumping a load of coal when the hoist on the trailer failed. The trailer fell quickly and did some damage to the trailer. The driver was inside the tractor and was shaken up.

Contractor stated was riding out of the mine at the end of the shift. When the man trip turned a corner on the travel way it must have hit a hole in the roadway raising the contractor up and the contractor moved forward hitting head of the latch for the man trip stored SCSR cache. This resulted in a laceration to the forehead that required stitches.

Employee states that while traveling the 2A belt on EE's exam EE was looking for draw rock when EE's mantrip bounced EE and EE felt pain in EE's lower back. Management traveled the same area right after the incident and found no deformities that would cause EE to bounce.

Employee alleged that while making the turn into the xcut from the #4 to #5 entry, the car slid into the rib trapping arm between the shuttle car deck and the coal rib. EE continued to tram freeing arm. This resulted in an abrasion to EE's right arm.

Employee was operating a diesel scoop when EE ran over a crib block that fell out of the scoop bucket throwing EE up into the canopy of the scoop and then back down in the seat. This resulted in a compression fracture to the L1 vertebrae.

Employee was coming up 12 belt line on a Mac-3. Employee states employee hit a hole causing the steering wheel to twist catching left hand resulting in a cut to the index finger requiring 3 stitches and a fracture to middle finger at the middle knuckle.

Operator of man trip was following a diesel scoop when the lights in the rearward position on the scoop blinded the operator of the man trip causing EE to steer to the right striking the rib. When the ride moved to the right the employees leg came in contact with the rib while inside the passenger compartment. This resulted in a dislocated hip.

While raising the belt with a mini-trac to make a belt set up, the belt slid off and came down on employee's head. This resulted in pain in employee's neck and lower back. Employee continued to work until 3/28/2018 when employee was taken off work.

Miner was on the blind side of the entry when a shuttle car traveling down the entry contacted the miner, apparently rolling EE, and causing EE to fall down. The injured miner did not lose consciousness and was able to call for help, but was treated for Compartment syndrome of the left lower extremity, and a Left hairline fracture to the fibula.

Employee was knocking stopping with scoop bucket then went to get out of the scoop and an 8" hollow block fell hitting employee on left hand. This resulted in a hairline fracture and 8 stitches to cut on the palm of hand.
