Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area

Upon leaving, they went forward, accelerated, causing the rear tires to enter the ditch at a higher speed, and the rear of the bus to lift. EE's seated in back of bus lifted from seats, and this EE landed awkwardly. EE was not wearing a seat belt. Seen at ER, released to restricted duty due to back strain.
The crew was pulling the liner down the slope and pulled a little to fast when the liner unraveled and caught speed, they tried to hold it with the forklift, it began to roll down the slope when the slope and the crew on the corners got out of the way, the employee in the middle tried to avoid by jumping and it caught their feet and they slipped down.
An employee was struck by the back end of a Hyster forklift knocking them to the ground, then was struck by the right rear tire pinning the left hip and right foot.
EE reports loading truck w/mud, making 2nd pass, hit return to dig button, lowered bucket halfway. Bucket stopped instantly, jarring them in the cab. They reported the incident 4 days later on 1/19, and sought treatment. Diagnosed with lumbar sprain, released with restrictions site cannot provide modified duty under.
Ee was riding in bus that hit a large pothole. Ee reports being jarred in seat, resulting in back pain. Seen at ER and then at clinic, released to restrictions. Site cannot offer modified duty at this time.

An LHD tire came off the Forks of a Telehandler during transport and rolled down the Range Front decline ramp. The tire contacted a CUSA Minca Paus 18.1 personnel carrier near crosscut 6, with two employees in the Cabin of the Paus. The Paus passenger sustained a laceration on the top of EE's head requiring staples to close.
Ee driving truck when hit a bump/pothole, causing the visor to fall and strike ee's head/neck. Seen at clinic, diagnosed with strain, released to full duty. On follow-up on 4/17, ee was put on restricted duty that site could accommodate, making this reportable for restrictions.
EE was passenger in the shift change bus on the way to the time shack. The bus hit a pothole and it jostled the EE in the seat. Seen at clinic, diagnosed with a contusion to the left knee and on follow-up complained of wrist pain as well. Released to restricted duty.
Ee was operating a guzzler truck. Ee grabbed the lever to open a discharge valve, lifted it, and felt a pop and tingling in right wrist. Seen at clinic, diagnosed with a strain, and on follow-up referred to ortho and put on restrictions.
Ee was lowering a man basket on Sh017 for fueling. Ee had to push hard on the release pin to get the basket to move. The basket dropped down, and the ee reached out to grab the handrail. Ee contacted the man basket safety gate bracket which was sharp, and lacerated right hand.
EE reports being loaded at loader 46, and being "shook hard" when the loader was leveling out material. Reports neck jolted sideways and feeling immediate pain and migraine. Video review does not support verbal report. EE reporting severe pain at follow-up appointment and put off work pending follow-up.

Ee driving tractor underground, reports being unable to downshift and slow tractor. Ee reports ribbing the tractor and jamming right wrist into steering wheel. X-rays negative for fracture. Ee released to light duty site can accommodate.
Two employees were using a telehandler to assist in moving a tractor that had broke down. The tractor could not be shifted into gear. One employee was operating the tractor and the other employee was between the telehandler and the tractor hooking up a chain. The tractor unexpectedly moved backwards striking and pinching the employee between two pieces of equipment.
Ee reports driving truck through the dump, hit a pothole, and the air seat "went out". Ee reports shooting pain in right hip, numbness to outside of right leg to calf, and tingling in right foot. Seen at clinic, diagnosis lumbar disc disease with radiculopathy. Released to modified duty that site can accommodate. Maintenance confirmed seat did not malfunction.
Ee was working from a man basket that was boomed out to remove hose. The basket bounced, and the ee's right ring finger was pinched between the basket and pipe hard line, causing a crush injury and amputation of the tip of the right ring finger. Orthopedic specialist in SLC did a revision amputation to the first knuckle. 60 days permanent disability.
Ee was jostled in the cab of EE's haul truck when shovel operator dropped a load. Seen on 8/31 for initial visit, diagnosed with a lumbar and thoracic strain, released to full duty. On 9/12/22, ee referred for physical therapy, making this reportable for treatment.
Ee was in loader, ee stretched, and ee's hand contacted the fan which did not have a cover on it. Ee was seen at the ER, diagnosed with a laceration that required stitches, and released with the direction to keep wound clean and dry. Returned to work full duty.
EE was getting lime at North Area lime silo. When driving away from the silo, they noticed a burning sensation in eyes and started to cough. EE reported the issue and was seen at health. Diagnosed with chemical conjunctivitis to left eye, scheduled to follow up 4/29. On follow-up EE was released to full duty effective 4/29. Prescription for eyes given.

Ee was operating a 777 haul truck, getting loaded by a loader. Ee reported the loader operator dropped boom of loader on side of truck, more than once, causing the truck to come off the ground, jostling EE in the cab. Reported to supervisor on date of incident. Seen by doctor on 10/29, put off work pending follow-up.
EE had left arm out of door opening between door and door frame of lube truck, holding a wooden block, attempting to use the block and the articulation of the machine to force the door back into position. The lube truck articulated unexpectedly while arm was outside the cab, pinning the arm between the door and frame. Open fracture needed surgery.
Ee reported being jostled in cab of haul truck when rock hit the headache rack. Seen at clinic and diagnosed with thoracic and lumbar strain. On 2/12 investigation complete and claim accepted as occupational making this reportable. Ee released to restricted duty.
Employee was troubleshooting the AC circuit on a 1600 LHD and had removed the cooling fan from the condenser. While testing, the fan started and moved striking the employee on the the thumb causing a cut and a fracture of the bone.
Ee was being loaded by shovel. The first bucket lifted the bed and when it came down the ee was jostled in the cab. EE was seen at the ER and reported a shoulder injury and back injury. Diagnosed with shoulder and back strain and released to modified duty.
Employee was driving down the decline. The employee passed another operator and applied the engine break through the turn and the front left bed corner struck the pillar rib of the muckbay causing the employees face to make contact with the truck radio resulting in a laceration on right cheek and nostril.
After dumping haul truck, ee was watching the bed lower while slowly moving forward. EE hit a bump in the road, and the truck bed seated at the same time, jostling EE in the cab. Diagnosed with spasm and whiplash in the neck and released to modified duty.

Hydraulic line on haul truck broke while ee dumping load. The loaded bed fell quickly, jostling the ee in the cab of the haul truck. EE had immediate pain in the neck and back. Seen at Golden Valley and diagnosed with neck muscle and lumbar strain. Released to restricted duty.

Ee was driving haul truck, it started to slide down a ramp, turned 180 degrees, and hit a dry spot of ground, coming to a sudden stop. Ee's left arm was jerked by the steering wheel. Seen at the clinic and put on modified duty, no use of the injured limb.

Operating a haul truck, employee struck left leg against dash of unit when employee drove over a pothole. Employee was working in tight quarters. Employee reported on 12/12/2019 as for medical on 01/07/2020 as area was irritated. No restrictions.
