Mostrando los 5The injured employee was sitting between two toolboxes eating lunch. A diesel scoop came up the roadway with a load of belt structure. As the scoop backed up into a crosscut the scoop hit a pothole in the floor and the scoop shifted over and hit the handle of the toolbox. This caused the toolbox to twist around pinning the employee's legs between the two toolboxes.
The employee was riding in on a man-trip and hit a pot hole. EE was bounced up and when EE came back down EE struck EE's middle back on the seat. EE then continued to work until the pain got worse. EE was then transported to the hospital.
Employee was picking up a mantrip with a loader, and the loader caught the mantrip shed door causing it to spring back striking the employees right leg.
Employee struck head against the mine roof which caused stiffness and pain in employee's neck while operating a Lo Trac Machine. Employee did not want to go to the hospital/doctor at the time of the accident but went the next day.
