Employee was scraping drive and hit frozen material. Employee was thrown forward and hit steering wheel. Employee had lap belt on during time of accident. Employee did not report injury to IMI till 12/10/2018.

While thawing ice on a bin pulley, a propane thawing device became entangled, causing the employee to become entangled. Investigation continues.

The employee was operating Cat 775D Haul Unit stocking out. The right front tire drove over a pile of stone. The unit bounced the seat bottomed out and the employee felt pain in lower back. Depending on the healing, surgery may be required.

Employee was driving water truck with window down, particles blowing around in cab got in his left eye.

Employee turned off the agricultural lime plant to dislodge a rock in the tail pulley. After locking of the conveyor he dislodged the rock. When he did that, gravity moved the conveyor belt entangling employee's arm in the tail pulley.

Driver was told by IMI to go to level 3 to dump load, While going to level 3 driver lost control of truck on incline & jumped out prior to truck going over embankment Driver suffered injuries to his shoulder, minor cuts to arms & knees Trim off location should have been on level 1. Incline to level 3 is not safe for a loaded dump truck. No sign was posted as to the % of the incline

Employee was driving haul unit, dumping material into hopper. Bed was raised to dump but co-worker radioed saying "don't dump". He pushed lever to lower bed but it slammed, severely jolting truck. He felt pain in his back & right leg, but thought it would get better. It just got worse.

Employee was running the Volvo 330D stone loader, digging in the Louisville Ledge. The terrain was rough and he felt pain in mid-lower back."Feels like there is a knot there and it is still really sore.

We were pulling a belt back up on conveyor with a rope tied off onto a loader. The rope broke and we don't know whether the rope, chain, pulley, clevice or what came up and hit his arm.

Workers were gathering materials for a job & moving from bin #2 to the tower. One was driving a flat bed service truck & 1 was in the passenger seat. Injured was seated on the rear of the flat bed. As the truck moved thru the yard it hit a bump & ee bounce up and back down on the bed of the truck causing pain in his buttocks, hip, tailbone, & broke the skin behind his right knee.

While creating a new haul road and driving the 980E Cat Front End Payloader over ruff terrain, he jostled/jarred his back repeatedly causing damage to his upper and lower back.

LOADER WAS LOADING WASTE MATERIAL FROM AROUND THE PUMP ONTO CAT HAUL UNIT. A LARGE ROCK FELL HARD INTO THE CAT HAUL UNIT, JARRING IT. THE DRIVER BANGED HIS TEETH TOGETHER WHEN THE ROCK HIT, WHICH CHIPPED HIS UPPER FRONT TOOTH.

EE WAS BACKING HAULING UNIT UP TODUMP MATERIAL OVER EDGE OF STOCKPILE. REAR TIRES WENT OVER EDGECAUSING HAULING UNIT TO FALL TO THE RIGHT SIDE AND ROLL DOWN STOCKPILE AND LAND ON ITS WHEELS. MR.BALL RECEIVED LACERATION TO HEAD AND CONCUSSION TO HEAD AND BRUISES TO BODY.

WHILE HAULING GRAVEL TO THE PLANT THE HAUL UNIT MET A READY MIX TRUCK. THE OPERATOR SHIFTED AND KILLED ENGINE AND AVOIDED CLOOISION, BUT THE HAUL TRUCK LOST STEERING AND MOMENTUM O GO OVER RAM P AND TURNED OVER. THE OPERATOR WAS WEARING SEATBELT AND AVOIDED SERIOUS INJURY.

LOADER OPERATOR WAS HAULING FILL STONE TO STOCKPILE AREA. HE SAID HE WAS DRIVING TOO FAST DOWN A SMALL INCLINE WHICH UPSET THE BALANCE OF THE LOADER. HE TENSED UP ON THE STEERING WHEEL AND ACC IDENTALLY SHIFTED THE LOADER INTO REVERSE. HE WAS TROWN AGAINST THE STEERING WHEEL AND THEN BACK IN THE SEAT AT AN AKWARD ANGLE, THUS STRAINING HIS BACK.

EMPLOYEE WAS MOVING SIGN INTO SHOP TO BE PAINTED. SIGN SLIPPED OFF OF FORKS AND WHEN IT HIT THE GROUND IT SPUN AROUND STRIKING HIM IN THE HEAD.
