EE DROVE LOADED SCRAPER OFF AN APPROX. 20' - HIGH STOCKPILE. EE MISSED THE TURN AND DROVE THE SCRAPER OVER A 3' - HIGH BERM. (SCRAPER DID NOT ROLL; EE WAS WEARING SEAT BELT - NOT EJECTED.)
Dumping material from a 631 scraper on top of berm, can slid to the side causing scraper to turn onto side.
AN EMPLOYEE PARKED A PICKUP/WATER TRUCK NEXT TO DRILL AND WAS FILLING THE DRILL. THE PICKUP ROLLED BACKWARDS AND THE HOSE CAME OFF THE DRILL AND HIT THE EMPLOYEE IN THE HEAD. WHEEL CHOCKS WERE USED BUT SANK IN THE SOFT DIRT.
EE WAS DUMPING A LOAD FROM HIS HAUL TRUCK & THE ROCKING ACTION HURT HIS BACK. HE REPORTED THIS ON 4/27 BUT DID NOT GO TO THE DOCTOR UNTIL 5/4/01WHEN HIS BACK GOT WORSE.
EE PICKED UP HAUL TRUCK WITH LOAD, TRIED TO DUMPIT BUT THE LOAD PLACEMENT DID NOT ALLOW TRUCK TODUMP. EE BACKED INTO A SMALL DITCH & DUMPED LOADAS THE BED WAS RAISING THE FRONT OF THE TRUCK RO SE UP. WHEN LOAD DUMPED, THE TRUCK'S FRONT END CAME BACK DOWN CAUSING HER CHEST TO HIT STEERINGWHEEL.
EE WAS WORKING ON A 12" LINE. THERE WAS A 4" LINE BENEATH THE 12" LINE. A LOADER WAS PULLING OUT FROM UNDERNEATH. AS THE SMALL LINE WAS ABOUT TO PULL CLEAR, IT'S FLANGE CAUGHT ON THE BIG LINE AND PULLED THE BIG LINE ON TO HER ANKLE.
EE WAS OPERATING A HAUL TRUCK. AS SHOVEL OPERATOR SWUNG LOAD TO DUMP IN HER HAUL TRUCK, THE SHOVEL TRACK SANK IN SOFT DIRT. THIS CAUSED BUCKET TO HIT SIDE OF HAUL TRUCK CAUSING EE TO SUFFER PA IN IN BACK.
AS EMPLOYEE WAS RUNNING THE LOADER HE ACTIVATED THE DECLUTCH. WHEN HE DID HE FELT A POP IN HIS KNEE AND BEGAN TO EXPRIENCE PAIN. EMPLOYEE STATED THAT HIS KNEE HAS BEEN HURTING FOR THE LAST T WO WEEKS WHILE OPERATING THE LOADER AND OFF AND ON FOR THE LAST COUPLE OF YEARS.
EE WAS OPERATING A LOADED HAUL TRUCK ON A FLAT SURFACE, STRAIGHT ROADWAY. THE EE ATTEMPTED TO MAKE A LEFT HAND TURN AT A HIGH RATE OF SPEED. WHEN THE HAUL TRUCK STARTED TO TURN IT BEGAN TO SLI DE SIDEWAYS AND ROLLED OVER ON ITS RIGHT SIDE CAUSING THE EE TO SUFFER A BROKEN COLLARBONE.
EE WAS OPERATING HAUL TRUCK. AS THE EE WAS DRIVING WITH A LOAD. SHE FELT SOMETHING IN HER EYE. AS SHE CONTINUED TO DRIVE, SHE RUBBED HER EYE AND THE EYE BECAME IRRITATED.
