He was running the Front End Loader to load trucks with aggregate. His back started hurting and became worse. His back had been bothering him for several weeks and he said it just got worse.

EE was running a skid loader. He stopped it with the bucket up to scratch his leg. He put his foot outside of the cab of the loader. He moved his right foot that was controling the bucket lever and it fell on his left foot.

TURNED TRUCK OVER ON ITS SIDE.

EMPLOYEE STATED THAT HIS LOWER BACK STARTED HURTING WHEN HE STARTED TO OPERATE A LARGE PIECE OF EQUIPMENT THAT HE HAD NOT PREVOUSLY BEEN OPERATING.

JUNK IRON WAS BEING MOVED BY LOADER. OPERATOR THOUGHT EVERY ONE WAS OUT OF THE WAY AND LOWERED BUCKET AND IT FELL ON INJURED'S FOOT.

THE EMPL WAS CLEANING AROUND THE ROCK CRUSHER WHEN A ROCK FELL FROM THE CRUSHER STRUCK A PIECE OF METAL AND SHATTERED SEVERAL OF THE ROCK FRAGMENTS LODGED INTO I NAME R INDEX FINGER REQUIRING MEDICAL ATTENTION
