Mostrando los 28EE was at a fuel station away from pit filling up transfer tank on work truck. Ground conditions were wet from rainstorm previous night. EE turned to pump and at the same time the nozzle started to fall from tank. EE tried to grab nozzle before it fell and in the process slipped on the wet ground resulting in fall, hurting ankle. ER doctor informed EE that it was a sprain.
Employee went to shim support legs on conveyor belt #22 rock fell off of conveyor belt Striking the employee in the back of the head causing a 3in laceration all ppe was worn
Working on log wash and hand got caught in between log washer and the frame.
Employee was standing on a steel plate and leaning over to work on conveyer. The steel plate slid out from underneath EE causing EE to fall onto the conveyer.
Employee was putting drive belts on the crusher when employee's right foot got caught between the rotor and sidewall of the crusher. Employee was wearing composite toe boots when incident occurred. Toe/foot was broke in an open fracture at the bridge of the foot. Dr. pushed bone down and sewed up with 6 stitches.
Sunny day with light winds. 55 degress
Clear day. Employee was attempting to climb off loader to stop feeder for VSI Crusher. Employee slipped on step at rear of loader and fell injuring right knee.
Employee was going to measure a return roller on the conveyor belt to wash screen. Support on frame gave way causing employee to fall.
EE was going to assist another EE get a rock loose from a conveyor belt. EE tripped over an air hose and fell hitting EE's left hand on a PVC pipe.
They were cleaning mud off of the feeder. Co-worker had his back to employee and accidentally hit him on his right hand with a pick ax; causing a cut to his hand.
He missed his step onto the walkway. His knee popped and he collapsed onto the walkway.
Injured employee & another employee were removing gearbox on conveyor. Injured employee had gearbox in hand, turned to set it down when he felt pain in lower back. The other employee said he heard a pop.
EE was loading things into his pickup outside the scale house. He slipped on loose rocks. He fell with his right leg under him. His right foot was under his right hip.
He was hooking a water hose to fill up the water truck tank. He felt a sting or bite on the inside of his left wrist. It started turning red so he was taken to the ER for a spider bite.

He stepped up on the conveyor frame that was about 4 ft high. The conveyor was not working because the generator was down and locked out. When he stepped up he was looking at his co-worker, then fell backwards. He was dazed and confused afterwords. He did not know for sure if he fell or if he passed out.
Employee was checking for an air leak on the front end loader. His left hand got caught between the service brake linkage and the parking brake linkage resulting in a mashed finger.
Operator was climbing down from service area when his foot slipped on bottom rung of ladder causing him to hit his left side on the bumper of the truck.
EE was doing maintenance a shaker screen.While greasing the driveshaft he discovered one grease fittings was not accessible.He rolled the v-belt drive with his left hand to gain access to the fitting,by doing this he pinched his left index & middle finger in between the belt and drive sheave causing an open fracture on both fingers.
He was running the Front End Loader to load trucks with aggregate. His back started hurting and became worse. His back had been bothering him for several weeks and he said it just got worse.
EE was running a skid loader. He stopped it with the bucket up to scratch his leg. He put his foot outside of the cab of the loader. He moved his right foot that was controling the bucket lever and it fell on his left foot.
He was working on the dredge when a hose came loose and hit him on the left side of his upper body.

HE WAS TRYING TO PUT LEGGING ON BELT. GOT FINGERCAUGHT BETWEEN PRY BAR AND FRAME CRUSHER.
EE WAS WALKING ALONG & TRIPPED & FELL. COULD FIND NO OBSTRUCTIONS TO CAUSE THE FALL.
EE WAS GIVING THE CONTROL TO OTHER EE, EE DROPPED IT & IT FELL & HIT HIM IN THE MOUTH
EE WAS PUSHING A LOADED WHEELBARROW ON INCLINE
