A SCD operator was replacing the drive belt on a conveyor when EE's gloved right hand became entangled in the sheave due to not performing lock out tag out. The miner sustained multiple injuries, including a fracture and multiple lacerations, to EE's right hand.

Inertia generated by failed coupling caused trailing railcar to move creating gap between knuckles.EE adjusted knuckle then walked between knuckles verifying hand brake was set. As EE was walking sideways between the knuckles, the rail car returned to the location of the failed coupling, crushing the EE.
A temp EE finished loading a 1000 lb. super sack when the EE noticed the super sack started to slide off the forklift forks. The EE thought they tilted the forks back. The EE then grabbed onto the super sack and felt pain in their lower back. On 2-15-22 temp EE was evaluated and given a prescription and a back belt for a lower lumbar strain.
The Haul Truck Operator driving a 777D unit was traveling with a load to the Quarry Crusher, when the operator veered from roadway, & traveled through an approximate 5'7" berm. The truck rolled down an approximate 15-20 embankment & landed upright at the next roadway area. The operator allegedly nodded off & lost control of the haul truck.
A Quarry Tech was driving an empty haul truck down into the quarry along a haul road. When the front driver's side truck tire hit a ~ 4" deep depression in the road the truck gave the Quarry Tech a jolt which jarred neck causing a strain.
The employee was working at SCD 2nd floor cleaning rocks that had fallen from the 115 belt. A rock fell from the belt and struck the back of the Employee's hand. The Employee continued to work but on Sunday had trouble making a fist. The Employee was sent to be examined by a doctor on 8/28/17. The doctor prescribed PT and sent EE back to work with restrictions.
Temporary worker alleges ee was injured while driving a haul truck through a switchback turn.

Employee reported discomfort and knot in groin area at end of shift.
Employee was getting off of a fork lift and he caught the small finger on his left hand between the fork lift body and a door hinge pin. The momentum of his body going forward while his finger was caught resulted in a fractured finger. Employee has not missed work and is working with limited restrictions.
Worker was driving a haul truck when he noticed pain in his shoulder. He had been experiencing this pain for over a year, but it was bothering him enough that he wanted to see a doctor. We have not determined yet if this is work related.
A non-employee entered Specialty Minerals' property and proceeded to the quarry area. The individual committed suicide by driving his 1998 GMC truck over the edge of the quarry. The vechicle fell approximately 300 feet to the floor of the quarry. The Massachusetts State Police investigated the incident and confirmed that the individual committed suicide.
Employee was digging a stripping shot. The bucket hit a large boulder that was covered up smaller stones. This caused the loader to come to a complete stop and the employee's upper body went forward then backward causing a whiplash type injury to his neck. Seatbelt was worn. He continued to work full duty. He was then seen by a doctor on 1/22/07 and placed on restricted duty.
Employee was descending down ladder next to aluminum chop bin platform in furance building. He had his right hand/finger pinched between bin and ladder rung when forklift backing up removing aluminum chop box hit bin. causing bin to move.
EMPLOYEE WAS DRIVING FORKLIFT ON FURNACE FLOOR AND FORKLIFT CAME TO A SUDDEN STOP. FORKS WERE TOO LOW AND HIT FLOOR EXPANSION JOINT CAUSING EMPLOYEE'S HARD HAT TO HIT ROOF AND EMPLOYEE BITE LO WER LIP. EMPLOYEE REQUIRED 5 STITCHES AND THEN WAS PREVIOUSLY SCHEDULED OFF FOR 4 DAYS.
EE REPORTED TO SUPERVISOR AT END OF SHIFT THAT HE HAD INJURED HIS TESTICLES. EE BELIEVES THAT HIS ROUGH-RIDING FORKLIFT HAS CAUSED HIS INJURY. EE SENT TO ER AT 11 PM ON 6/25/03. EE REFERRED TO UROLOGIST AT 9:30 AM ON 6/26/03. UROLOGIST BELIEVES THAT EE'S TESTICLES HAD BECOME TWISTED. TESTICLES WERE DETORSED BY ER PERSONNEL. EE RELEASED TO LIGHT DUTY UNTIL 7/1/03. FOLLOW-UP VISIT TO
TRUCK DRIVER RAISED DUMP TRUCK BODY TO OFFLOAD EXCESS LIMESTONE. WHEN THE DUMP BODY WAS FULLY EXTENDED, THE TRACTOR AND TRAILER TRIPPED OVER ONTO ITS SIDE. DRIVER WAS INSIDE VEHICLE AT THE TIM E OF CAPSIZE, AND BANGED HIS KNEE.
EE WAS OPERATING A HAUL TRUCK & LOADER OPERATOR PLACED A LARGE STONE IN THE BED OF THE TRUCK WHICH ROCKED THE TRUCK & EE HIT HIS LEFT ELBOS ON THE WINDOW HANDLE. HE WAS WEARING HIS SEAT BELT.
EMPLOYEE WAS DRIVING AN ARTICULATED DUMP TRUCK WITH APPROXIMATELY 18 TONS OF MATERIAL. AS HE TOOK THE CORNER AT THE MAINTENANCE SHOP TO GO UP THE HILL THE LOAD SHIFTED TO THE RIGHT AND THE DUM P BODY TURNED OVER ON THE RIGHT SIDE. THE CAB WENT ONTO 2 WHEELS THEN CAME DOWN. EMPLOYEE RECEIVED STITCHES TO FOREHEAD.
EMPLOYEE WAS TRANSPORTING 2000# "SUPER SACKS" TO WAREHOUSE. WHILE PLACING SACK IN WAREHOUSE, EMPLOYEE RIPPED SACK IN BACK OF THE ONE HE WAS STACKING. EE FROM TRUCKING COMPANY WAS PICKING UP AG LIME IN A EXTENDED OPEN DUMP BODY TRUCK. HE WAS STANDING ON A LADDER AT THE FRONT OF THE DUMP BODY. AS HE WAS DIRECTING THE LOADER OPERATOR WHERE TO DUMP THE THIRD OF 4 BUCKETS IN THE MIDDLE OF THETRUCK, THE BUCKET CAME IN CONTACT WITH THE SIDE RAIL AND ROCKED THE DUMP BODY. THE EE ROCKED BACK THEN JUMPED OFF. HE INJURED HIS SHOULDER.
EE WAS ATTEMPTING TO MOVE A FULL PALLET THAT HADCOME DOWN SIDEWAYS ON THE MOVING CONVEYOR. HE STOOD ON THE CONVEYOR WHILE ANOTHER EMPLOYEE WAS AT FLOOR LEVEL WITH A PRY BAR. THE EMPLOYEE ON TH E CONVEYOR FLAGGED THE PHOTO EYE AFTER FREEING UP THE PALLET AND THE CONVEYOR STARTED AUTOMATICALLY PULLING HIS FOOT INTO A 3/4" OPENING BETWEENTHE CONVEYOR AND THE ROLLERS.
BEING LIFTED IN A MAN LIFT BASKET. WHILE HOLDING HAND RAIL ON BASKET, RING FINGER ON RT HAND WAS PINCHED BETWEEN MAN BASKET AND FORK BACK BOARD.

EE WAS DIGGING IN THE MUCK PILE WITH A FRONT ENDLOADER & A ROCK SLID DOWN FROM ABOVE & HIT THE BUCKET & JARRED THE EE'S NECK. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 9/7 AT WHICH TIME HE WAS G IVEN A PRESCRIPTION FOR STEROIDS WHICH MADE THISCASE RECORDABLE.
EMPLOYEE WAS OPERATING THE LOADER DIGGING UP BIG STONES & BOULDERS WHEN A BOULDER SLIPPED OUT OF THE BUCKET. THE LOADER ROCKED UP & DOWN JOLTING THE EMPLOYEE FORWARD & BACKWARDS CAUSING INJURY TO HIS BACK. DX: STRAINED/SPRAINED BACK.
