Employee report pain in their back when the bed of the haul truck came down after dumping a load of rock to the crusher.
Employee was riding in the back of a tractor. The tractor hit a hole and the employee bounced up and back down striking the bottom of their foot on a hose that was on the floor. Their foot flexed up against the metatarsal boot causing a fracture.
Ee was driving the haul truck and hit a deep pothole. EE struck head on the window of the haul truck. EE was transported to the ER with neck pain. Diagnosed with a cervical strain and put on restricted duty for one day.
Installing Underground Bulkhead Panel. Panel slipped from the left side of the forklift hitting the ground then bouncing and striking the employee in the abdomen area.
Employee was driving an explosive truck and struck left elbow on the ROPS.
Employee was jostled while seated (seatbelt on) in the cab of a haul truck while being loaded. On 3/13/2018 employee went to the Doctor where they were prescribed physical therapy making the event reportable.
Haul truck operator was traveling down ramp and when transitioned onto the working bench ee felt a "pop" in back. The IE continued to feel discomfort and asked to be evaluated by medical provider. As a result physical therapy and work restrictions were issued.
Adjusting width of forks on FL068. Employee was standing on right side of fork, facing the outside of fork. Using both hands, applied pulling pressure toward self with both hands moving fork away from backboard slightly. EE was not aware finger was between fork and backboard. As employee released fork with right hand, it came down pinching tip of left index finger.
LD-237 (Caterpillar 992) loader was going down for mechanical reasons, LD-245 (Caterpillar 994) loader was called to support Sage mill feed. While tramming with a full bucket, the operator drove up on one of the stockpiles which resulted in the loader tipping over and coming to rest on its right hand side. Following Work Care's recommendations operator was released back to work
Haul truck 930 traveling from the 45 stockpile, section 3 area towards the North/South haul road went through the water bar and slowing down for a stop sign, the operator and trainer heard a loud bang which startled the Operator. The Operator reported some pain in lower left back after the event had occurred. Operator was seen by a Nurse Practitioner and placed EE off work.
Haul Truck Driver backed into the Bucket of Loader causing the truck to come to an abrupt stop jarring the drivers back.
As concrete was being pumped to area for placement, concrete was splashed into the right eye causing a corneal abrasion.
Employee was operating scraper. While driving out of the cut, the cab began to bounce significantly & he experienced shaking in the driver's seat. He turned to look back at cushion hinge & recalls seeing piston fully extended. On approximately the 8th bounce of the cab, he felt extreme pain in his back. He immediately turned off the scraper, opened the door and shouted for help.
Employee was driving a haul truck when he reported driving over a rough section of road, after which the employee reported experiencing pain in his back and leg.
While in a manbasket being lowered to the ground with a forklift, the employee pinched his left pinky finger between the basket and the fork frame. This event resulted in three sutures.
Employee was traveling up the decline when his batch truck hit a pothole which resulted in the employee hitting his head on the cab ceiling. Medical evaluation determined the employee experienced a cervical thoracic lumbar strain and was placed on restricted duty.
Loader 52 was loading Haul Truck 105 when the operator of Loader 52 bumped the hoist lever causing the boom of loader 52 to fall slightly. The operator caught the lever and stopped which caused loader 52 to rock. The bucket of loader 52 struck the bed rail of Haul Truck 105. This employee was prescribed physical therapy when he was evaluated at medical facility at a later date.
Employee reported soreness to the back and shoulder after operating a haul truck.
While driving a 789 haul truck the seat belt contacted the seat recline control causing the seat to unexpectedly recline.
While riding on crew bus from time shack to location of equipment in pit, the bus hit a bump.
While trying to shotcrete the Southeast Decline, operator had lost air pressure and had his nozzle plug up. Sprayer operator shut off the air and started disconnecting the air and accelerator valve at the end of the boom when the energy was released it sprayed the operator in the face.
Mechanic & welder were in the JLG man basket removing a leveling cylinder pin. Welder was repositioning the basket the machine was shut off & the boom started to settle resulting in the arm sliding forward. As the boom lowered it pinned the mechanic between the upper bucket & the back railing of the man basket resulting in an injury to the mech left ear. Seen at ER received sutures
While transporting employee's from Pete Bajo back to the dry at the end of day shift, the bus hit a bump and jarred the bus occupants around. One employee reported soreness in his back and requested to seek further medical attention.
Driver and passenger were driving down the haul road to Pete Bajo Pit from Leeville. When trying to make a left hand corner the truck tipped on the passenger side.

Employee was operating a haul truck when he drove over a rut in the decline and felt a pain in his back. Employee sought medical attention on 8/12/2013 and was given a full duty release. Employee sought medical attention again on 8/16/2013 where he was released with restricted duty.
Haul truck was backing to the shovel to get loaded when contact was made with the bucket of the shovel and the haul truck. causing discomfort to the shoulders and back of the haul truck operator. The Operator did not seek medical attention until 5/9/13
While driving a haul truck in the ramp the employee hit a rough spot in the roadway that jarred him upward and backward in the seat causing pain between his shoulders.
Service truck parked in muck bay. Haul truck pulled in and parked behind service truck in get fuel. When haul truck driver started to exit cab he accidentally released park brake and truck rolled forward pinning maintenance man between trucks.
Employee was jostled in the cab of the equipment, straining his back. On 9-5-12, he was treated and returned full duty. On 9-17-2012, he sought further treatment and was classified as a lost time.
While exiting truck hand was on door frame when door closed fracturing hand.

Haul truck XXR-611 was descending the Reona pad loaded. XXR-611 breached the left side berm on the second 90 degree corner. The truck continued to moved across the flat and came to rest against the berm of another haul road.
While filling hydraulic oil tank, lid fell closed on employee's finger cutting and fracturing it.
Employee got his right thumb caught in the door of a 4-yd loader. His thumb nail was torn off and he received a laceration, which required 2 sutures.
The operator of a service truck was driving up the ramp in Spiral #4 and ran out of fuel and the Getman truck rolled backward and he turned it into the rib and when he did he hit his head on a light mounted on the canopy which caused a laceration on his forehead requiring 4 staples.
Was adjusting forks on forklift when he placed his right hand between the fork and frame. The fork crushed his finger, fracture it.
The miner observed that water/steam was coming out of the top of the Radiator from under the Radiator cap as well as the overflow on LHD Company #61. The Miner then placed his right hand on top of the cap to tighten it. The cap came off scalding the Miner's right/ left hand and right arm. The miner received 1st & 2nd degree burns to include restricted duty as of 01/10/2011.
EMPLOYEE WAS OPERATING SCISSOR LIFT #8, HE NEEDED TO ADJUST HIS SEAT. WHEN HE PULLED THE LEVER TO ADJUST HIS SEAT TO MOVE IT BACKWARDS HIS RING FINGER ON HIS LEFT HAND GOT PINCHED IN BETWEEN THE LEVER AND THE FRAME RAIL OF THE SEAT. IT CUT HIS FINGER BAD ENOUGH TO REQUIRE STITCHES
Pickup employee was riding in ran in to berm.
Employee was riding out of the mine on a lube truck when he felt some discomfort in his abdomen. He did have a pre-existing abdominal hernia, however it was determined by three different physicians he had an abdominal muscle strain which was the issue, not the hernia. The determination of the muscle strain was determined on 11/3/2010.
Employee was exiting his loader, when he went to open the door his hand slipped off of the door allowing it to close on his right knee.
employee had a sore thumb for some time and was originally checked at the health clinic in march (non-reportable at that time). On 6-25-10 the doctor reclassified him as a Reportable Illness and he had an outpatient surgery to treat his thumb, also received some sutures.
While walking across the parking lot outside the main gate to the mine, the employee was struck in the right arm by a light vehicle. Employee required medical attention and was diagnosed with fracture to a bone spur. Employee returned to work full duty the same shift.
While operating ULD052, the bucket of the loader struck the left rib causing the employee to hit his head on the door window resulting in a laceration to the left eyebrow. Several sutures were needed to repair the injury and he was returned to full duty.
EE crested ramp from stockpile in 777 haul truck and saw water truck watering ramp. Began catching up to water truck, tried to slow down, lost control and spun out striking base of stockpile with 777.
A light vehicle rolled on top of an employee's foot while he was getting supplies out of the bed of the truck.
Employee injured his left shoulder, when his shoulder struck the steering wheel, after the brakes locked up on his haul truck.
While tramming L.H.D. down main decline employee felt something in his eye.
Employee was bent over starting a pump when an LHD entered the area. The operator did not see the injured employee and struck him with the mucker bucket. The employee did not suffer any MSHA reportable injuries but was placed on restricted duty.
