EE was operating his loader at the Asphalt #6 pile, cage mill when he struck the concrete base of the loadout tunnel with his bucket. The sudden jar must have caused a back injury because on March 2nd, he started to feel pain in his back and pain/numbness in his right arm. This report was given to us on MARCH 7, 2005.

While helping to install drive belts on conveyor 1A, a rock came from the belt and struck employee above the right eye just to the side of the hard hat brim causing a laceration that needed 4 stitches.

EE WAS OPERATING A CAT HAUL TRUCK INT HE AREA WHERE WE WERE DIGGING S UMP WHEN HE STARTED PULLING OUT OF SUMP THE MATERIAL INT HE BED SLID BACK CAUSING THE TRUCK TO STAND UP ON ITS TAILGATE. E E WAS BANGED AGAINST DOOR AND INSIDE OF CAB CAUSING THE LACERATIONS TO HIS HEAD, FACE AND NECK.
REPORTED THAT THE 980C LOADER SHIFTER WAS HARD TO USE AND IT CAUSED HIM PAIN IN HIS LEFT SHOULDER.
LEAD BLASTER IN CHARGE ON TOP OF TRUCK TO MONITOR FILLING OF STICK MAKING BIN. HE DIRECTED OPERATOR TO MOVE BOOM IN HIS DIRECTION. BOOM SWUNG RAPIDLY WITH INITIAL APPLICATION OF HYDRAULIC PRES SURE AND STRUCK BLASTER IN CHEST, KNOCKING HIM OFF THE TRUCK. BLASTER FELL 11 1/2' TO THE GROUND. HE SUFFERED BROKEN RIBS, PUNCTURED LUNG, BROKEN CLAVICLE, CUT TO BACK OF HEAD AND FOREHEAD.
ROCKS STUCK IN C7 CHUTE FELL ON MY BACK, SHOUYLDERS AND HEAD.
