Shift started at 11 PM on 10/21. While employee was attempting to load a barrel of chain onto the forks of the lotrac, the operator exited the lotrac to assist and then the lotrac rolled forward and rolled against employee's right foot/ankle.

Mostrando 50 de 119Employee was finishing up scooping behind the cage. There was a piece of blue Aqua 2" water line about 7' long buried in the gob. When he filled the bucket and raised it up to take it to the car, the pipe got free and slid down the edge of the bucket striking him in the face. He received four sutures in upper lip.
Employee riding bus when bus struck 2" pvc pipe, which pushed the pipe coupling into the employee's left arm, causing a laceration requiring 20 sutures
Motor 57 came off top of hill and slid into 183 man bus. Employee was not able to get off before motor hit. He bumped his knee on edge of bus and twisted his knee. His shift started on 1/25/2015 at 11:00 PM and his injury occurred on 1/26/15 at 6:05 AM.
Supply car came uncoupled from 62 locomotive. In the process the back locomotive was pushing the supply cars and pushed them into the front locomotive no knowing it was uncoupled. This injured the employee's left side rib cage. The result from treatment was a fractured rib.
Manbus #191 stopped for a block light at N-9. He stopped and the #4 section bus came down the track and hit bus #191 that the employee was driving and hurt his back.
Employee was riding in man bus when the bus hit the inside heel flange of the track frog causing the bus to suddenly stop, straining employees back and neck.
Springs broke in the forklift seat causing the employee to be bounced and jarred in his back when moving across the yard. This will be employee's first lost time for this accident. Date of loss started on 1/27/2015.
REPORTABLE ONLY - Employee had sit down in shuttle car and had fingers in the grating (used for a guard between operators compartment and the boom of the car) adjusting seat when he accidentally lowered the boom of the car mashing the end of his finger between a rock hanging on the boom and the grating causing a crush injury with partial amputation of right index finger.
Employee was operating a locomotive when a collision occurred with the inby locomotive causing the operator to hit his left side in the operators compartment causing bruising to his left side. Employee went to the doctor the next day and was taken off work.
Employee was cutting strings off of a belt and the roller jerked the pliers out of his hand and pulled his left index finger into the roller. Employee was sent out for treatment. He received seven stitches.
Employee was pulling the shuttle car with the scoop. The bolt heads that attached the bracket to the shuttle car, that was being pulled, sheared off resulting in one of the bolt heads flying back striking the employee at the top of the eye causing injury to the eye and face. **EE RTW on 10/2/13 but was taken back off work on 10/6/13 for additional treatment.**
Employee was helping put a supply car on the elevator and was standing on the elevator when the operator uncoupled the motor from the car and backed up, the supply car rolled back and the employee's foot was caught between the supply car and the shaft collar resulting in fracture to his right foot.
Employee slipped/fell into structure. Rollers caught her sleeve and pulled her into the rollers causing her fingers to get caught between the rollers. She also bumped her head against the structure causing a black eye. Both fingers were broken and one received stitches.
Rock fell off the side of the shuttle car and struck employee on the top of the left foot. He was treated at DCH and taken off work.

Investigation determined the rails had moisture on them from weather changing. All rails were examined and sand applied. DID NOT START LOOSING TIME UNTIL 1/22/13.
Employee stated that he was moving mantrip when batteries shifted, catching his finger between frame of mantrip and batteries.
Employee was going through flypads, air moving dust around and dust got in employee's eye. Medical treatment and antibiotics were given.

Employee had raised the forks and mask of the Mega Trac to see what was needed to repair or replace the lift jack. The employee positioned himself under the mask; the mask leaked down and pinned the employee between the mine floor and mask.
Employee was standing outside of bus and took the parking brake off the bus. The bus then rolled 2 to 3 inches causing the seat to fall and smash employee's right ring and small finger. Employee had lacerations and needed stitches.
While traveling in #181 man bus on the East main travelway the bus wrecked at bottom of the hill at cross cut 12. Employee had laceration on head and contusion of left hip.
The track that ends at the East B had a slick spot about 10 feet long. The operator yelled out to hold on but it was to late and the bus collided with the inby parked bus. Employee felt pain in back. Employee did not lose time until 2/4/2012.

Employee stated that while talking with another employee, a shuttle car he was standing by was struck and pushed him into the rib. Employee's first lost work day was not until 11/04/2011.
