Mostrando 50 de 113Employee was loading roof straps in the bed of a Ram car in 4 South mains. Using the ram to pull the straps in the bed, the end of the straps struck a piece of metal in the bed which caused the straps to swing hitting the employee in the lower leg.
Employee was riding in #12 Eimco manbus when the bus derailed at the E-7 crossover track switch and he felt pain in his back. The employee continued to work until 10/13/08 when he began missing due to the injury.
employee was taking off track stop at the end of the track. His left hand was on the stop when a supply car moves unexpectedly catching the tip of his left thumb. (fracture tip of thumb)
Employee was operating a supply motor on the 1 east track haulage when the switch at #7 wall track chute had been left thrown for the chute. Unable to stop in time the motor entered the track chute and collided with 2 mantrips and air lock doors. He began missing work due to his injury on 2/6/08.
The individual was walking the 913 refuse belt looking for bad rollers. He observed a build-up of mud on a roller. He tried to remove the debris with a wrench. His arm was caught by a roller and pulled in.
Employee was working on belt when belt tightened up kicking tailpiece sideways pinning employee between structure and tailpiece.
Employee was walking on the opposite end of a Goodman Manbus that was in the process of being jacked back on track at #30 wall spur track switch E-16 Section. He tripped on the rail & fell with his left foot going under the manbus as the bus was slewing over on the other end, causing his left foot to be caught between the bus & rail.
Employee was operating a jeep while firebossing when #144 wall switch, 3 N F-track had been left throw for the curve. Seeing switch, he applied brake & dogged the pole, but jeep kept going around, threw him forward & he struck right side/shoulder against body of jeep. EE cont'd working until 7/25/07 when he had surgery to right shoulder.
Operator was letting scoop bucket down. Injured saw a block underneath and tried to get block out and pinched thumb under bucket.
Injured was walking or standing along side of scoop and his hand got caught between wheel and fender of scoop. Life flighted to Ruby Memorial Hospital.
An A-40 Volvo off-road truck rolled over on a slope while hauling clay for the construction of the soil liner in area 4.
EE was struck by a piece of broken face conveyor flight while observing the Headgate Drive sprocket, E-14 Longwall Section. The face conveyor chain broke, causing a piece of broken flight to be thrown , catching his left forearm between the piece of flight and the coal rib, resulting in a compound fracture to the left forearm.
EMPLOYEE WAS WORKING AT THE TRACK CHUTE, COUPLING MOTOR TO RAIL TRUCKS. EE PLACED HIS RIGHT HAND ON BUMPER CAR & THE END GATE OF THE RAIL CAR DROPPED DOWN ONTO HIS RIGHT HAND.
Employee was on 7I section track attempting to align couplers on battery pack & locomotive. As he was aligning on the locomotive, the battery pack car moved & his right leg was caught between.
8H section, using a scoop to spread calcium on roadways. The scoop drove over a rail, causing the rail to roll onto co-worker's left ankle.
Scoop hit a stack of cribs and pinched leg between cribs and shield.
Employee was tramming a shuttle car over a bridge when it came off the bridge and onto solid bottom. The frame bottomed out, causing her to hit her head on the canopy jarring neck/back/jaw.
EMPLOYEE WAS CLEANING CORDS AND A PIECE OF DOG CHAIN THAT WAS WRAPPED AROUND THE TAILROLLER SHAFT AT THE 4I TAILPIECE. THE BELT WAS TURNED OFF. WHILE ATTEMPTING TO REMOVE THE OBSTRUCTION, THE BELT UNEXPECTEDLY DRIFTED CAUSING HIS HAND TO BE CAUGHT. HIS REACTION WAS TO PULL HIS HAND OUT. THIS CAUSED THE LACERATION.
While operating mantrip the pole came off the wire. Employee had brake held with right hand and tried to pull the pole down with left hand and felt a pain in left shoulder. Diagnosis: sprain in left shoulder.
Employee was operating #11 motor in the E-12 track entry. When he went across the #35 wall track switch the motor whipped to one side dislodging the seat from the motor deck. The seat fell over, causing him pain in his lower back.
7 H LONGWALL SET UP, UNLOADING SHIELD OFF DOLLY. AS EE WAS BACKING SCOOP UP, BACK WHEELS WERE OFF THE GROUND. AS HE TRAMMED SCOOP, WHEELS CAME TO GROUND AND SHIELD BOUNCED THE SCOOP. HE HIT HARD HAT (HEAD) ON CANOPY.
Employee was a passenger, riding out of the mine on a jeep. A low boy trip was parked on the main line (D.C. power had tripped). The jeep hit the parked trip, he put hands up to brace for impact but sustained a fracture to left thumb & wrist.
7H BELT WINDER ROLLING UP BELT, THE ROLLER BROKE ON BOTH SIDES OF THE HANGER. THE ROLLER FELL & STRUCK THE TOP OF RIGHT FOOT CAUSING A FRACTURE TO 2ND & 3RD METATARSUS.
Injured was walking in front of scoop when scoop hit section of track; rail slewed and knocked him off his feet. He fell on his right arm causing a fracture of lower arm.
Employee was operating loead motor, pulling a supply trip. As he approached #4 crossover he saw the track switch was for the curve, his motor struck air lock doors and an omega block fell out and struck back of his neck.
EE'S VEHICLE WAS STOPPED WHEN AN ONCOMING VEHICLE LOST ITS BRAKES, STRIKING THE STOPPED VEHICLE. EE WAS ATTEMPTING TO EXIT HIS VEHICLE WHEN STRUCK, KNOCKING EE TO THE MINE FLOOR. EE HAD A LACERATED LOWER LIP, REQUIRING 6 STITCHES TO CLOSE.
OPERATING MANTRIP WHEN HE PULLED BRAKE HANDLE & ROD CAME OUT OF MASTER CYLINDER & MANTRIP FAILED TO STOP. IT STRUCK AIR LOCK DOOR & TOP OMEGA BLOCK FELL OUT & STRUCK HIS NECK & MIDDLE BACK.
EE STANDING IN THE WALKWAY NEAR THE SHEARER AND A ROCK CAME OUT OF THE CONVEYER AND STRUCK HIM IN THE RIGHT EYE. THE INJURY WAS DIAGNOSED AS A LACERATION REQUIRING 7 SUTURES TO CLOSE.
BUS TRAVELING 3 MPH ON TRACK, BUS FELL OFF TRACK FALLING 6" ON THE FLOOR. EE WORKED REST OF SHIFT NOT COMPLAINING OF INJURY, IN FACT ASSISTED OTHER EE CARRYING ROLLS.
EE STEPPED OUT OF LOADER IN THE #5-#6 ENTRY XCUT IN THE 3-NORTH RIGHT SECTION. WHILE STANDING BETWEEN RAM CAR AND LOADER, THE RAM CAR OPERATOR DROPPED THE TAILGATE TO PREPARE TO BACK DRAG AND CAUGHT INJURED EE'S LEFT FOOT BETWEEN THE TAILGATE AND MINE FLOOR. RESULTING IN FRACTURES TO THE 2ND, 3RD AND 4TH METATARSAL BONES ON THE LEFT FOOT.
INJURED WAS UNLOADING BAG ROCK DUST FROM SCOOP BUCKET WHEN THE BUCKET CAME DOWN ON RT.FOOT CAUSING A FRACTURE TO ONE BONE IN FOOT.
HE HAD SKIDDED A CAR AFTER UNCOUPLING IT FROM THE OTHER CARS THE SKID KICKED OUT. HE WAS WALKING WITH HIS BACK TO THE CARS & THEY STARTED DRIFTING. THE CAR HIT HIM FROM BEHIND & HE SAT DOWN ON CAR, BUT HIS FEET WERE PULLED BACK.
THE EE WAS CARRYING A BELT STAND WITH ANOTHER EEIN PREPARATION FOR CHANGING OUT A BAD BOTTOM ROLLER. THE EE SET HIS END DOWN AND AS THE OTHER END WAS BEING LOWERED THE RUNNING BELT CAUGHT THE EDGE OF THE STAND AND KNOCKED HIM TO THE GROUND AND STRUCK HIM IN THE LEFT ARM AND LEG. INJURY WAS DIAGNOSED AS A SEVERE CONTUSION TO THE BICEPTTENDON.
TAIL MOTOR STOPPED AT TOP OF HILL AS THE LEAD MOTOR PULLED SUPPLY CARS THROUGH THE DIP. CARS UNCOUPLED & ROLLED BACK & STRUCK TAIL MOTOR. OPERATOR GOT DOWN INSIDE KITCHEN.
EE WAS RIDING IN A BUS GOING TO THE SECTION, THE BUS HIT A BROKEN RAIL ON THE WIRE SIDE AND THE BUS STOPPED SUDDENLY. HE IS HAVING PAIN IN HIS NECK & BETWEEN THE SHOULDER BLADES. THE INJURED HAS A HISTORY OF BACK & NECK PROBLEMS. THE ACCIDENT WAS UNDER INVESTIGATION UNTIL 11-7-02.
INJURED WAS SWITCHING OUT SUPPLY CARS. A SKID UNDER CAR WHEEL DID NOT HOLD CAUSING CAR TO MOVE. THE CAR CAUGHT LEFT LEG CAUSING HIS ANKLE TO TWIST RESULTING IN A SPRAIN TO LEFT ANKLE.
HE WAS A PASSENGER IN A JEEP WHEN TROLLEY POLE CAME OFF WIRE & HIT ROOF. A PIECE OF ROCK BROKE & FELL & STRUCK HIS LEFT KNEE. LOST WORKDAYS BEGAN 3-13-02.
INJURED WAS BUILDING CRIBS. THEY HAD LOADED A SCOOP WITH CRIB BLOCKS AND WAS TAKING THEM TO WORK AREA. THE INJURED WAS AT WORK AREA WHEN SCOOP GOT CLOS. HE STARTED TO MOVE. HE TRIPPED AND FELL . SCOOP OPERATOR DID NOT SEE HIM FALL AND WAS LOOKING FOR A SIGNAL TO STOP. SCOOP BUMPED STANDING CRIB AND KNICKED CRIB ON INJURED.
EE WAS A PASSENGER IN MANTRIP TRAVELING SOUTH HAULAGE WHEN MANTRIP JUMPED TRACK AND HE STRUCK TOP OF HEAD ON METAL BOX WHICH WAS SECURRED TO SIDE WALL OF MANTRIP. SUTURES TO CLOSE WOUND.
AFTER WAITING @ SWITCH, 1 JEEP & 2 MANTRIPS STARTED MOVING IN LINE, JEEP BOOM CAME OFF TROLLEY WIRE CAUSING IT TO STOP, THE FOLLOWING BUS STOPPED AND WAS REAR ENDED BY 2ND BUS WHICH FAILED TO STOP IN TIME. ALL INVOLVED EE EXITED MINE UNDER OWN POWER. 3 EE FROM REARENDED BUS SOUGHT MEDICAL TREATMENT FOR BACK PAIN. NO INJURIES REPORTED IN 2ND BUS.

AFTER WAITING C SWITCH, JEEP AND 2 MANTRIP STARTED MOVING IN LINE, JEEP BOOM CAME OFF TROLLEY WIRE CAUSING IT TO STOP, BUX IMMEDIATELY FOLLOWING STOPPED AND WAS STRUCK IN REAR BY 2ND BUS WHICH FAILED TO STOP IN TIME. ALL INVOLVED EE EXITED MINE UNDER OWN POWER 3 EMPLOYEES FROM REAR ENDED BUS SOUGHT MEDICAL TREATMENT FOR BAKC PAIN. NO INJURIED REPORTED IN 2ND BUS.
MOTORMAN WAS TRANSPORTING A ROOF BOLTER FROM BAILEY BOTTOM. THEY HAD 3 LOWBOYS & THE ROOFBOLTER WAS ON THE CENTER CAR. THE DC POWER WAS OFF, HE LIFTED THE TROLLEY GUARDING AWAY TO CLEAR THE BO LTER. AS THE LOWBOY MOVED, HIS LEFT FOOT GOT CAUGHT BETWEEN THE LOWBOY & THE RAIL.
HE WAS TAKING A BOTTOM ROLLER OUT OF THE BELT WHILE IT WAS RUNNING AND THE OPPOSITE SIDE OF THE ROLLER MOVED TOWARDS THE HEAD CATCHING HIS FINGER BETWEEN THE HANDLE ON THE ROLLER AND THE BELT STAND. HE RECEIVED THREE FRACTURES TO THE DISTALPHLANGE OF THE LEFT THUMB AND THREE STITCHES TO CLOSE THE LACERATION.
INJURED WAS RIDING MANTRIP WHEN THE PORTAL BUS CAME UPON A BROKEN RAIL. ONE PART WENT DOWN ONE PART STAYED UP CAUSING BUS TO COME TO A SUDDEN STOP THROWING MEN FORWARD CAUSING A BRUISE TO RI GHT ANKLE.

FAN BELTS WERE INSTALLED ON THE COMPRESSOR FOR THE ON A BROOKVILLE MOTOR. THE FAN BELT HAD A TWIST IN IT AFTER INSTALLATION. THE VICTIM WAS NOT INVOLVED IN THE INSTALLATION OF THE BELTS, BUT W ANTED TO FIX THE TWIST. AT THIS TIME THE MOTORMAN SET THE BREAKER ON THE MOTOR. THE VICTIM REACHED FOR THE BELT AS THE BREAKER WAS SET. COMPRESSOR STARTED HAND WAS CAUGHT BETWEEN BELT & PULLE
INJURED WAS HAULING SUPPLIES WITH A SCOOP WHEN THE SCOOP CAME TOO CLOSE TO A BUNDLE OF CABLE BOLTS CAUSING BAND TO BREAK ON BOLTS. A BOLT CAME INTO THE KITCHEN OF SCOOP STRIKING INJURED ON RI GHT CHEEK CAUSING A PUNCTURE WOUND.
