EE was under the 4 South No. 2 belt, at about 24 X cut, changing bottom rollers. The area was wet and muddy. A roller fell about 24" and struck the little finger of the right hand, resulting in an "open fracture", requiring 5 sutures and a splint.
Employee was removing crib block from in front of #11 Voest Alpine Miner with a co-worker in the 3 North Right Section. Both employees picked up a crib block to toss aside when the crib blocks collided catching the tip of injured's right middle finger between the two crib blocks. Employee sustained a fracture to the right middle finger. Employee was wearing Meta Miner gloves.
An unplanned roof fall occurred in the 12 I longwall tailgate. The fall measured approx. 16ft wide 8 feet high by 25 feet long. No injuries or equipment damage as a result of this incident.
A malfunction of the crabapple elevator occurred due to worn out contactor tips. The elevator was able to be operated by manual means, but could not be operated in automatic. This problem occurred throughout this shift and was completely corrected at the end of Midnight shift by replacing the worn contactor tips and replacing a relay.
crew of 4 men were turning over Dosco beams that were stacked upside down, EE did not let go as others were dropping the beam and the tip of his right middle finger was caught between the beam s as he pulled away. the end of his finger was severed and remained in his glove, no bone fragments were in the glove.

EE was installing a bottom belt roller on wide side of the 3 north #2 belts at #36 walls when the roller slipped and caught the tip of his left index finger between the end of the roller and frame of roller hanger. EE sustained a laceration to the tip of is left index finger requiring 7 sutures. EE was wearing Meta miner gloves.

13-I-The miner operator was sumping the miner out. The right side miner bolter was beside the miner. Some bolts were stored on top of the miner. One bolt fell, struck a stack of augers bits that were also being stored on the miner. The augers then struck the right side bolter operator on the lip. This resulted in a laceration to his lower lip.

The miner was trying to pry a battery cover open with a hammer the lid slipped and struck small finger of left hand causing contusion and abrasion to the finger.
