Mostrando los 21Employee was pushing buggy up a hill, the buggy slipped and lost traction pinning the employee between the buggy and the belt structure.
He was shoveling on #2 beltline, as he leaned forward, his face was too close to the moving belt. The edge of the belt at a splice came in contact with his right eye. He had dirt in his eye and a scratch on the side of his eye.
The employee was shoveling the #3 head drive when his shovel became hung in the belt splice and he allegedly twisted his back.
Injured was in the bridge conveyor with a come-along tied to the bridge pulling it over to reload it onto the carrier when the bridge was accidentally started causing injured to fall injuring his shoulder.
INJURED EE STATED THAT HE HAD BEGUN TO TRAM HIS MBC FORWARD WHEN THE DECK SHIFTED, CATCHING HIS FINGER BETWEEN THE DECK OF THE MBC AND THE OIL COOLER ON THE MBC. INJURED EE HAD HIS FINGERS OVER THE SIDE OF THE MBC.
INJURED WAS TRAVELING UNDERGROUND WITH THE REST OF THE CREW IN THE RAIL RUNNER. HE STATED THAT HE WAS LOOKING TO THE SIDE AND AS HE TURNED TO FACE THE DIRECTION OF TRAVEL HIS HEAD STRUCK A ROO F BOLT. THE MINING HEIGHT WAS 40" TO 60" IN THIS AREA. INJURED WAS EXAMINED AT THE LOCAL HOSPITAL AND RELEASED.
INJURED WAS SCOOPING THE ROADWAY WHEN AN OLD PIECE OF BLACK PLASTIC PIPE FLEW UP FROM UNDERNEATHHIS SCOOP AND STRUCK HIM ON THE CHIN, REQUIRING THREE STITCHES. INJURED HAD NO LOST TIME.
INJURED WAS UNHOOKING A BELT TRUST THEY HAD JUST LOADED ON A FLATBED TRUCK TRAILER. HE TRIED TO THROW HOOKS ACROSS THE BELT TRUST. ENDLOADER OPERATOR STARTED BACKING AWAY. ONE OF THE HOOKS HUN G ON TRUST DRAGGING IT OFF THE TRAILER. INJURED MAN WAS ON THE TRUST WHEN IT HIT THE GROUND. HIS LEFT RIB CAGE HIT THE IRON BEAM.
INJURED WAS SCOOPING THE SECTION WHEN A PIECE OF DRAWROCK FELL FROM BETWEEN ROOF BOLTS STRIKING HIM ON THE BACK AND SHOULDERS.
HE WAS ATTEMPTING TO DRAG A ROLL OF CONVEYOR BELT OUT OF THE SCOOP BUCKET & HAD HIS TOES HOOKED ON THE EDGE OF THE BUCKET FOR LEVERAGE. THE SCOOP OPERATOR LET THE BUCKET DOWN & CAUGHT TOES OF HIS RIGHT FOOT UNDER THE EDGE OF THE BUCKET. HE SUFFERED BRUISES TO HIS TOES.
EE WAS TRAMMING THE SCOOP WHEN HE RAN OVER A ROCK IN THE ROADWAY AND JARRED HIS BACK. HE WENT TO THE HOSPITAL WHERE HE WAS CHECKED & RELEASED. NO LOST TIME UNTIL 3/12/02.
INJURED HAD HELPED REMOVE ROCK FROM THE MOBILE BRIDGE CONVEYOR. HE WAS POSITIONED LOOKING TOWARD THE OUTBY DUMPING POINT OF THE MBC WITH HIS HAND LOCATED ON TOP OF THE MBC, JUST ABOVE THE CONVEYOR THROAT. A LG. ROCK CAME DOWN THE CONVEYOR AND STRUCK INJURED ON THE TIP OF SMALLEST FINGER OF THE RT. HAND.
THE INJURED WAS OPERATING A SHUTTLE CAR WHEN IT TORE DOWN A ROOF STRAP WHICH RELEASED A KETTLE BOTTOM THAT FELL AND STRUCK HIM ON THE SHOULDER.
INJURED STATED THAT HE WAS BACKING THE CRAWLER DOWN THE SIDE OF THE LO-LO STRUCTURE AND THE DOLLY HUNG ON THE STRUCTURE. HE ROLLED TO HIS RIGHT SIDE TO LOOK BACK AT THE DOLLY WITH HIS LEFT FOO T PARTIALLY IN THE DECK & PARTIALLY ON A MUCK PILE OUTSIDE THE DECK. HE THEN SAID HE HEARD THE FRONT CRAWLER OPERATOT SAY TO PULL BACK UP & BEFORE HE COULD GET HIS FOOT BACK IN, THE FRONT CRA
INJURED WAS TRAVELING WITH THREE OTHER EE UP THE BELT HAULAGE ENTRY ALONGSIDE THE LO-LO BELT STRUCTURE IN THE PERSONNEL CARRIER. WHEN THEY CAME TO THE POINT WHERE THE HAULAGE SYSTEM DUMPED ON THE LO-LO STRUCTURE, THE OPERATOR OF THE PERSONNEL CARRIER INTENDED TO MAKE A LEFT TURN AND GO THORUGH A CROSSCUT. THE FRONT TIRE OF THE CARRIER HIT SOME MUCK CAUSING OPERATOR TO LOOSE CONTROL
THE INUJURED WAS REACHING INTO SCOOP TO INGAGE WENCH AND ACCIDENTLY HIT TRAM LEVER CAUSING SCOOP TO MOVE FORWARD AND CAUGHT HIS LEG BETWEEN SCOOP AND POWER CENTER.
INJURED WAS HAULING SLATE FROM THE PREP PLANT TO AN AREA TO BED DOWN A ROAD. INJURED ASSUMED IT WOULD BE EASIER TO BACK OUT THE ROAD INSTEAD OF HEADING OUT AS THE OTHERS. AS HE BEGAN TO BACK O UT OF THE ROAD, HE NOTICED HE WAS TOO CLOSE TO THE DITCH. HIS REAR TIRES SLID ABOUT 4 FT INTO THE DITCH OVERTURNING TRUCK. HE WAS EXAMINED AT THE HOSPITAL AND RELEASED.
INJURED WAS TRAMMING A SHUTTLE CAR ACROSS THE SECTION AND RAN OVER ROUGH ROADWAY JARRING HIS BACK.
INJURED WAS RIDING IN THE BED OF A PICK UP TRUCK WHEN THE PICK UP RAN OVER A ROCK AND THE INJURED WAS BOUNCED AROUND AND HIT THE SIDE OF THE BED.
THE INJURED WAS CHECKING THE UNIVERSAL JOINT IN THE SCOOP. THE REAR WHEELS WERE OFF THE GROUND AND ROTATING WHEN HIS FOOT GOT CAUGHT AND WAS PULLED BETWEEN WHEEL AND FRAME.
