Employee was getting ready to start building mine portal barricades when somehow or another the ground clamps and electrode holder got together while he was holding them.

Injured was building a platform when the electrical hand held grinder he was operating kicked-back striking him on the left side of the face and chipping a tooth.

A rock fall occurred in the #3 Entry (Belt Entry) at crosscut #21. The fall is approx. 34 FT. long, 49 FT. wide, and 10 FT. thick.

Employee was pulling roof bolt straps and tripped over a rock and fell against the mine rib.

Injured had driven a pin out of a conveyor chain when a piece of the pin broke off striking injured in the arm. Injured had no lost time or medical treatment until 6-19-2012 at which time he went to the doctor for infection and was x-rayed.

Employee was operating Roof Bolter, spot bolting on the oo1 MMU and a piece of rock fell from the mine roof approx. 2ft. x 1ft. x 2 inches and struck employee on the left side of body on the Ribs and lower back.

Employee was working on pump line, pulling a broke band out of crimping tool, when the broke band came out of the tool it struck employee on the ring finger on the right hand requiring stitches.

Employee was operating roof bolter and as he started drill steel into the roof a piece of draw rock fell striking his left hand index finger requiring 9 stitches in finger.

A Roof Fall occurred 21 crosscuts underground from the portal in the # 5 Entry. The fall measured 20'Long x 50' Wide falling from the gob hole thru #5 entry into #4 Right. The fall was approximately 4' to 8' thick.

A roof fall was discovered at the 7th cross-cut inby the Pathfork portal in the No.2 entry. The fall measures 20'x 20'x 3'-5' thick.

Employee was hanging the Feeder cable and hurt his left shoulder

Employee was cutting a band that was around a bundle of timbers, when the band broke the timbers rolled and pinned his leg against the scoop bucket.

Employee was backing out of a shed on a three wheel buggy and pinched his hand between the shed door jam and the buggy steering handle cutting his hand and requiring stitches in his left hand.

Employee was pushing buggy up a hill, the buggy slipped and lost traction pinning the employee between the buggy and the belt structure.

Employee was operating walk thru roof bolter, as he was stepping from the rear of the machine onto the mine floor he twisted his left knee.

Employee was operating a Roof Bolter, while installing outside bolt a piece of rock fell striking him on the left hand.

The crew was moving power up. EE was pulling Crawler cable slack, when he pulled something in his back.

EE was dragging a 18 H.P. Pump that weighs approx. 300lbs. off # 6 panel approx. 800 feet. EE hurt the lower part of his stomach.

He was shoveling on #2 beltline, as he leaned forward, his face was too close to the moving belt. The edge of the belt at a splice came in contact with his right eye. He had dirt in his eye and a scratch on the side of his eye.

In the No.4 Entry (primary escapeway) between x-cuts No.4 and No.5 going underground from the surface, there was a rock fall measuring 15' wide, 8' long, and 4 1/2' thick.

#3 Entry (belt-line)- Around 2:20 P.M. on 3-31-11, A large piece of rock measuring approx. 16' wide, 10' high, and 2' thick, fell off the highwall knocking down part of the canopy and the Main Belt line.

During the weekly exam of air counts a rock fall was discovered on 3/9/11 at 6:25am in the #4 intake air course. Approx 20' inby the mine fan. The rock fall is approx 10' wide X 8' long X 4' high.

He was standing on a ladder, when a shuttle car cable knocked him off the ladder, landing on left leg.

Victim was a security guard on duty at RB #5 mine and shot himself in the head with a handgun.

Foreman at RB#5 Mine reported a roof fall, he said it was in the #3 entry inside the mine from the back side. The fall measured approx. 20' X 20' X 4' thick. There was no one injured. He said it fell sometime between the morning and that afternoon.

He was bolting top when the wrench slipped and hit him in the face or mouth.

EE was bolting top 002 section, while bolting he was positioning a strap in place when a rock fell pinching his left little finger between the canopy and the strap. No lost time accident happened Wed. before Thanksgiving weekend. EE RTE on next reg. shift. 11/29/10.

He was hanging a curtain at the miner in #3 heading. The conveyor chain hung up on side board. When it freed up it threw a piece of metal and struck him on the elbow on his left arm. A med treated him and took him to Dr. where he received 5 stitches to his left arm.

While working on a miner in the re-build shop EE was hammering on a bit when a piece of metal struck him in the left eye. He was wearing safety glasses. He went to Dr. where they removed the foreign object. No lost time.

He was in #3 road way intake side working on water line a piece of hammer chipped off and struck finger.

When getting down off of the belt structure EE's right elbow struck the fork on the fork lift.

EE stated that he was hanging his tie off to a bolt anchored in the roof. He twisted his body when he reached up to hang the shuttle car cable tie off, he felt pain in his neck and right shoulder.

The EE was spreading rockdust from the rockdust bag with one hand, while holding the bag of rockdust with the other hand. He stated that he felt a sharp pain in his lower back. He left work at that time, driving himself home, he went to his doctor the next day.

The EE stated that he was pulling on the cable and the cable stopped he then jerked the cable and he felt pain in his back. He left the job site but returned to work the next day and worked the next 4 days. EE didn't come to work on 4/19/10 and didn't call his foreman. The next day the foreman called him, EE stated that he would be off for a week or two requested by a Doctor.

The injured employee stated that he was installing a spill rubber at the No.2 head drive. He was between the head drive and backboard in a small area. He tried to turn around and he twisted his back.

A rock fall measuring approx 20' long X 20' wide X 3' thick was discovered at 1:30PM on 2/19/10. The exact time of the fall is undetermined. The fall was located in the 9th crosscut inby the north portals in the #5 entry. The #5 entry was dangered off on all accessible entrances and the #4 entry will become the designated travel way and primary escapeway.

A roof fall was discovered in the main return air course 55 crosscuts outby MMU 002-0 while making a weekly examination. The roof fall was approx. 4' thick 20' wide by 30' in length. The roof fall did not interfere with the air flow. The roof fall was cribbed and the travelway was moved to the other entry.

Employee was flushing out water lines and was hit in the face with debris.

EE was loading the powder truck while climbing down the steps he fell striking his right knee on one of the steps.

The injured EE stated that he was shoveling material from underneath the belt conveyor when he slipped and fell landing on a 4" crib block. The crib block was under the foot of a piece of support structure.

The injured miner was pulling a 4/0 crawler from beltline to the section power center when he felt pain in his lower back. The miner also stated that he had been injured at a former employer that was still paying his medical bills on his back. The foreman should have asked for help when dragging the cable.

The employee was changing rollers out on a conveyor belt in the prep plant when the chain fell from the I beam letting the roller and the chain fall catching the injured persons left hand between the roller and the electrical box.

EE was cutting a ring off of bit on the drill and air blew thru it blowing hot grease on his face.

The injured employee was putting guard back on the No. 1 conveyor belt tail pulley and dropped the guard striking his little finger (pinky finger) on his left hand.

The injured person was removing a piece of rock out of a Long-Airdox bridge conveyor. When he let the rock fall, he didn't move his right foot out of the way letting the rock fall on his right foot.

The injured employee stated while he was moving a 18 hp water pump with a dolly the pump fell off the dolly striking him on his right foot.

The injured put his right hand on top of drill pot to retreive the drill steel from the roof and raised the boom instead of lowering it.

While crawling on his knees, the injured employee stated that he felt pain in both knees and lower part of both legs.

The injured employee was putting roof bolts into the roof, water was dripping from the roof and got the injured wet. While holding onto the drill steel, he felt an electrical shock in his arms and chest.
