Employee was operating a Damascus Personnel Carrier. He went to back Personnel Carrier up into a nearby x-cut to get out of the way of an incoming Scoop and while doing so the employee jammed his head against the roof. Employee did not miss any work, however he did seek medical treatment making this a reportable.

Employee was riding on a personnel carrier directly behind the operators compartment when apparently his cap lamp cord caught a roof bolt thus pulling his hard hat off his head. As he went to duck down his left shoulder struck the roof and rolled his body back over the fender of the personnel carrier. Employee did not miss any work however he had been placed on restricted duty.
Employee had arrived in x-cut #3 to #4 just as loading crew was backing out of cut when he was struck by bridge.

EE WAS SCOOPING THE BOTTOM OF THE #3 ENTRY IN 5 RIGHT ROOMS WHEN HE STRUCK HIS HEAD AGAINST THE ROOF.

THE OPERATOR HAD JUST COMPLETED TESTING THE BRAKES AND WAS STARTING DOWN INTO THE PIT WHEN THE REAR END OF THE PERSONNEL CARRIER WENT OUT THUS LOSING BRAKING CAPABILITY.EE ALONG WITH CO-WORKERS JUMPED OFF WITH EE LANDING ON WRIST.
EE WAS CLEANING TRAVELWAY WITH SCOOP WHEN HE INADVERTENTLY RAN OVER A PIECE OF ROCK THAT WAS LAYING ONT HE TRAVELWAY THUS CAUSING THE EE TO STRIKE HIS HEAD AGAINST THE ROOF LINE. JAMMING NECK. EE STARTING RECEIVING PHYSICAL THERAPY ON

EMPLOYEE WAS OPERATING PERSONNEL CARRIER WHEN HIS LEFT SHOULDER STRUCK LOW SPOT ON THE ROOF LINEJAMMING LEFT/RIGHT SHOULDERS, RIGHT ELBOW AND CHEST AREA.
EMPLOYEE WAS CLEANING UP ROADWAYWITH SCOOP WHEN HE INADVERTANTLY CAUGHT ON A PIECE OF COROLON PIPE, WHEN THE PIPE SPRUNG FREE IT CAUGHT A PIECE OF ROCK FROM BUCKET STRIKING EMPLOYEE ON BRIDGE OF NOSE THAT REQUIRED SUTURES.
WHILE EE WAS OPERATING SCOOP HE GOT HAND PINCHED IN PIVOT POINT.
WHILE EMPLOYEE WAS MOVING BELT STRUCTURE, EMPLOYEE EXPERIENCED PAIN IN RIGHT ARM. EMPLOYEE HAD AN ONGOING PROBLEM WITH THIS ARM AND ON JUNE 8,2001 HE WENT TO DR. AND WAS PLACED ON RESTRICTED D UTY AS OF JUNE 8, 2001.
EE WAS OPERATING A PERSONNEL CARRIER GOING DOWN TO PIT. WHEN NEARING THE PIT, HE APPLIED BRAKES AND WHEN DOING SO HE PUT THE CARRIER INTO A SPIN AND HIS LEFT HAND WAS ON THE SIDE RAIL WITH FIN GERS ON THE UNDER SIDE. THE RING FINGER APPARENTLY CAUGHT A ROCK BETWEEN BOTTOM AND UNDER CARRIEAGE OF PERSONNEL CARRIER. EE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS APPLYING STONE ON TRAVELWAY WITH SCOOP WHEN SIDE OF SCOOP OPPOSITE THE OPERATORS COMPARTMENT SANK IN A SOFT SPOT, THUS PERMITTING THE OPERATORS SIDE TO RAISE AND IN DOING SO EMPLO YEE STRUCK HEAD AGAINST ROOF AS WELL AS A ROOF BOLT. EMPLOYEE RECEIVED STAPLES FOR LACERATION.
