Employee was roof bolting when the sleeve of his coveralls got caught on drill steel thus wrapping right arm around drill steel.
While performing his normal duties as a Roof Bolter/MBC Operator - employee experienced pain in right hand. Employee continued working until October 14, 2010 at which time he started missing work making this reportable. This document supersedes #1305920.
Employee was performing his normal duties as a belt cleaner when he experienced pain in right forearm. Employee continued working until August 12, 2010, at which time he went for a medical evaluation and was diagnosed as tendenitis and has since started missing work, now making this reportable.
Employee was removing a block wall that was used to store mud behind it in order to be dried and later removed from the mine via Scoop. As he started to remove blocks some mud came loose thus pushing employee's leg against the blade of the Scoop.
Employee was installing pin on a bridge when he caught finger under the pin. Employee did not miss any work however he did go to a medical facility to get an x-ray done and it was determined that it was fractured therefore making this accident a reportable.
Employee was drilling hole on a motor base when the drill bit jammed in hole. He reversed the drill and while pulling back out on the drill the drill struck employee's mouth thus breaking his front teeth. Employee is getting necessary dental work done.
Employee jumped out of the bed of the company pick up truck near the mine office at the completion of his shift when he experienced pain in right knee. Employee continued working until January 7, 2010 at which time he started missing work making this accident reportable.
Employee was pulling rock down with bar when he experienced pain in right shoulder. Employee continued working until he was placed on restricted duty effective 08/19/2009 now making this reportable.
Employee was attempting to pull a piece of rock off of the continuous miner when his hand slipped and he caught his left wrist on a sharp edge of the rock sustaining a laceration. He did not miss any work however he did receive eight (8) sutures thus making this report a medical.
Employee was directing the fork lift operator on changing batteries on scoop. He directed the fork lift operator to set batteries down; at the same time he reached for cap on battery jack on scoop. He got left thumb pinched between cap of jack and battery.
Employee was roof bolting, when a piece of rock dislodged from the roof, striking employee on the back of the head, neck and between shoulders.
An unplanned roof fall occurred in the #3 entry of 1 Right, 1 x-cut inby survey station #683. Fall was approximately 4' plus high, 19' wide, and 35' long. Area was timbered and dangered off. Did not affect any ventilation but did cover about 35' of belt and structure. Will not be cleaned up. Area was bolted with 4' compression bolts and was also center posted.
An unplanned roof fall was found in the #2 entry 1 x-cut outby survey station #996 fall was approx. 30' long, 30; wide and 6' plus high. Fall did not affect any ventilation or equipment, roof fall will not be cleaned up, area was timbered and dangered off.
An unplanned roof fall occurred in the belt entry (#3) 1 x-cut outby survey station #8. Fall was approximately ten (10') high, nineteen (19') wide and thirty five to forty feet (35' to 40') long. Area was bolted with four (4') fully grouted bolts with x-bars installed as per our approved initial roof control plan. Fall will be cleaned up. Did not affect any ventilation.
Employee was pulling plastic water line when he lost his footing on uneven bottom slipping and landing chest first on bottom.
Employee was in the process of putting his left foot on the first rung of the ladder to enter loader. The right foot had been on uneven bottom when his foot rooled to the side.
Employee was attempting to punch a hole in a piece of belt for a date board when the employee swung the hammer to strike punch and missed striking his left hand. The employee did not miss any work however he received four (4) sutures thus making this accident report a medical.
Employee had lowered the head of the roof bolter to remove drill steels. The finisher steel had stuck in the roof. As the employee was putting the pusher steel away, the finisher steel fell out of the roof, striking the employee on the ring finger of his left hand.
Employee was attempting to clean material that had built up between the head roller and the chute on the no. 11 belt with a roof bolt when the roof bolt got up against the belt thus causing an avulsion of the left index finger.
An unplanned roof fall was discovered in the return air course in a room panel off 10 Right Main.This fall is located at spads #5241 and 5242. It is approximately 10 feet high and 60 feet long. The area was supported with 6 foot, fully grouted resin rods. The area was dangered off. The fall did not affect the ventilation at this mine.
While employee was attempting to use the drill head of the roof bolter to move coal laying in the bottom in order to obtain clearance for the second set of drill steels, the canopy swung, thus catching the webbing of his left hand between the canopy and drill steels that he was holding on to.
An unplanned roof fall was discovered in the return air course in a room panel off 11 Right Main. This fall is located 60 feet inby spad #5323. It is approximately 10 feet high and 200 feet long. The area was supported with 42 inch fully grouted resin rods. This area was dangered off. The fall did not affect the ventilation at this mine.
Over the road truck driver pulled in to get loaded with coal. Driver was doing his truck inspection. He bent over to check air hoses. When he stood back up he got dizzy, lost balance, fell and hurt his ankle.
Employee had just placed rub rail onto frame of continuous miner & needed to move the cover plate in order to permit the pin to pass through the rub rail to secure to the miner, when the rail fell off the frame of the miner onto the top of employee's feet.
Employee was tramming continuous miner to the right side of cut when he caught his foot under pan of miner.
Employee was backing a personnel carrier up when his hard hat caught on a roof bolt thus pushing his hard hat down onto forehead causing a laceration. Employee did not miss any work, however he did receive eight (8) sutures therefore making this report a medical.
Employee was roof bolting when a drill steel fell out of roof striking employee on left hand.
Employee pinched hand between the head of roof bolter and roof bolt plate. He missed one (1) day due to this injury. This occurred over the Thanksgiving/deer season holiday of which we were idle. We returned back to work on November 28, 2007. His day missed was on Friday, November 23, 2007 only.
Employee twisted lower back while setting fibercribs. Employee did not miss any work however after seeking treatment for ongoing pain the physician placed him on restricted duty effective 12/12/2007 now making this report classified as a medical.
An unplanned roof fall occurred in the #3 belt entry of Southwest Mains just outby the face. Fall was approximately 7' high, 18' wide and 45' long. The affected area was bolted with 6' tension roof bolts according to the approved roof control plan. Did not affect any ventilation or equipment. Fall will be cleaned up.
Employee was operating scoop, when he ran over a hump on the roadway, thus causing him to strike his head against a roof bolt and caused a laceration. Employee did not miss any work, however he did receive sutures, making this a medical report.
Employee was servicing his front end loader when he slipped and fell to the ground injuring his lower back and groin area.
An unplanned roof fall was found at approximately 10:30A.M. in the #2 entry of 1st Left, 1 x-cut outby survey station #81. Fall was approximately 4' high, 19' wide and 10'-12' long. Area was bolted with 36" fully grouted resin bolts according to the approved roof control plan. Area was timbered and dangered off. Did not affect any equipment or ventilation.
Employee was running levels. After doing surveying the employee started to duck walk due to sore shoulders when his right knee popped and partially locked up. Employee continued working until February 28, 2008 at which time surgery was performed to repair a meniscus tear of the right knee now making this a reportable.
Employee was removing rock from the #1 bridge when he experienced pain in his lower back. Employee continued working until December 3, 2007 at which time he started missing work due to this injury, therefore, now making this a lost time accident.

Area was bolted with 4' fully grouted resin bolts with 5 rows across per the approved Roof Control Plan. Area was timbered and dangered off. This did affect the travelway.
An unplanned roof fall occurred in the #2 entry of 2 Left, 2 x-cuts inby survey station #325. Fall was approximately 19' wide, 24' long, and 4' plus high. Area was bolted with 4' tension re-bar bolts with 5 rows across per the approved Roof Control Plan. Area was timbered and dangered off. This did affect the travelway.
Employee was operating a continuous miner when he was struck by coal from the pressure of an uncharted drill hole.
An unplanned roof fall occurred in the #5 entry of 1 Right Mains at survey station #682. Fall was approximately 19' wide, 25' long, 4' plus high. Area was bolted with 4' tension rebar bolts with 6 rows across per our approved Roof Control Plan. Did not affect any travelways, ventilation or equipment. Area was timbered and dangered off.
Employee was checking breaker in Motor Control Center Building. As he went to step up on a bucket with screwdriver in right hand he apparently lost his balance and as he went to catch himself he accidently contacted the top side of breaker with screwdriver thus receiving flash burns.
Employee was drilling roof to install roof bolt when a piece of rock dislodged from roof striking employee on left side of head causing laceration of left ear and contusion of left leg. Employee did not miss any work however he did receive sutures making this a reportable.

Area was timbered and dangered off. Fall area was bolted with 4' tension re-bar bolts with 5 rows across according to the approved Roof Control Plan.
Employee was operating a Damascus Personnel Carrier. He went to back Personnel Carrier up into a nearby x-cut to get out of the way of an incoming Scoop and while doing so the employee jammed his head against the roof. Employee did not miss any work, however he did seek medical treatment making this a reportable.
An unplanned roof fall occurred in #5 entry of the South Mains, 2 x-cuts inby survey station #227. Fall was approximately 18' wide, 55' long, and 4' plus high. Fall did not affect any ventilation or travelways. Area was timbered and dangered off. Area was bolted with 4' tension rebar bolts with 5 rows across. Fall will not be cleaned up.
An unplanned roof fall occurred in the #4 room of 1 Right Room, 2 X-cuts inby survey station #691. Fall was approximately 19' wide, 30' long, 4'plus high. Fall did not affect ventilation, equipment, but did affect travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered at approximately 11:15PM.

An unplanned roof fall occurred in the #4 entry - 2nd intersection inby the mine opening. Fall was approximately 11 feet high, 19 feet wide and 38 feet long. Area was bolted with 5 foot fully grouted resin bolts along with 6 foot tension re-bar bolts and 10 foot cable bolts. Fall was on intake escapeway so fall was cleaned up.
An unplanned roof fall occurred in the #5 entry of 1 Rt. Rooms 1 x-cut outby survey station #668. Fall was approximately 25' long, 19' wide, 4' plus high. Area was bolted with 4' tension rebar bolts with 6' tension rebar bolts spotted through the area along with straps. Did not affect any equipment, ventilation or travelway. Fall will not be cleaned up.
While employee was exiting the steps of the IT 28 (forklift) he inadvertently stepped on a piece of rock with his heel permitting the right ankle to twist. Employee did not miss any work however he was placed on restricted duty effective May 1, 2007 thus making this a reportable.
