Employee moving forks on wheeled telescoping forklift to center position. Employee grabbed side of mast near for support bar free travel area. Fork hung up on support bar bracket and employee pushed fork to center. When fork fell into place, support bar fell through free travel area contacting employees thumb.

Mostrando los 29Employee reported pain in shoulder verbally to supervisor on 7/29 but was unsure if work related or not. Supervisor did not complete an incident report and SH manager was unaware. Employee called on 8/12 to say that employee saw doctor, and doctor thinks there may be herniated disk. Employee is on temporary layoff as of 8/10.
Employee walking along top of the railcar when SRL locking mechanism engaged pulling the employee back and striking leg against a hatch which caused injury. Employee declined medical attention because he thought it was bruised/sprained. Employee requested medical evaluation on 6/2/15 because it was still sore. Upon evaluation a fractured fibula was discovered.
Contract employee injured left knee while attempting to operate a switch which was difficult to move. The employee did not report the incident immediately but tried to self treat. Employee reported the incident on 02/25/2015.

Employee was walking from a control panel to a check a temperature probe for a kiln. While walking the employee stepped onto the concrete slab of the kiln building with the foot partially overhanging the slight ledge. When the employee put the weight down, it caused the ankle to twist and sprain.
Employee was climbing onto a D-8 Dozer and slipped. Employee caught themselves with the arm that was maintaining a point of contact with the handhold. This caused the arm to extend and strain, resulting in a tear of a tendon in the bicep.
Employee audiogram indicated standard threshold shift.
Employee was opening up a hatch toward the bottom of a hopper connected to a bucket elevator. The door swung open and hyper extended the employees arm causing a strain to the shoulder.
EE was removing an electric motor from a conveyor - he was lifting it up and in the process a bolt caught the edge of a guard lying nearby, causing it to fall and hit him in the tooth. When he was hit with the guard, he also dropped the motor on his finger resulting in a cut on his left ring finger.
Wind blew sand into left eye while walking through plant yard.
Walking to collect sand samples. Employee checked the time on his watch and tripped and fell over large pipe. Landed on his right side, right elbow.

Employee was pushing a high pressure sprayer and lost traction with his footing. When he fell forward the handle on the sprayer pinched his left hand.
Employee was removing cutting edge bolts and his hand was near the head of the bolt. The bolt slipped while removing with a pneumatic wrench and hit the palm of his left hand. The contact cause a laceration to the employee's palm.
Employee was walking across company owned property to report to his work station. He tripped over ground deviation and fell, straining his right shoulder.
Employee was not walking on the path, but chose an alternate route and tripped over a water hose. When he fell he injured his right hand.
Employee was walking to work on site and stumbled but did not fall. Foot was sore but did not report it. On October 3, 2012 she went to the dr. and found foot was broken and then reported it. Left foot. No restrictions or treatment.
There was no accident, this is a claimed occupational illness. EE, a Stout Construction employee, goes to family doctor, thinks he has bronchitis because his sister has it at this current time. Doctor tells him that he should see a lung specialist. EE went to lung specialist 9/7/12 and was told not to return to the sand mine at this current time until more testing was completed.
While preparing a belt splice the employee slipped on the conveyor frame falling forward until his eyebrow and bridge of nose made contact with a conveyor cover hoop.
While repairing pump on pontoon, employee struck bridge of nose with angle iron while bending over in water. Root cause was lack of awareness of surroundings.
Employee walked out of lab and ran into a cigarette butt can that was out from and to the left of the doorway. This caused employee to fall and when she placed her hands on the ground to stop her fall she sprained her right wrist. she did not wish to see a doctor but saw one after her shift. Root cause was not watching where she was walking.
Contract welder had an apparent seizure. He has a history of seizures. His partner observed him laying over a steel beam that he had been welding on, gasping for air. There were no apparent injuries other than minor blood coming from his nose. His breathing was fine and pulse strong although slightly fast. He left in an ambulance.
