Employee stepped into a 14" wide by 3' long hole with a 6" drop to the new beam which had to be open due to construction. Employee caught himself on a 3' column that had a sharp corner causing a puncture wound to his left forearm. The wound required 3 stitches inside and 9 stitches outside.

Miner was fueling the plant dozer and the fuel hose was wrapped around his leg. When he finished he dropped the hose, he than lost his balance and fell from the rear platform on the dozer. EE pulled something in his shoulder and is awaiting diagnosis.

Employee's foot slipped while mounting oil check ladder, injurying his right ankle.

EE stated that he slipped after stepping on piece of metal near tail piece and fell, landing on left wrist.

Employee stated he was mounting a dozer when his right foot slipped, causing his right knee to receive the full impact of his weight. Employee stated that he felt his left knee "pop".

While hanging a miner cable, EE was injured, when he fell off of a ladder.

While building stoppings, employee backed up against a stack of Kennedy Stopping panels and fell on top of the pile.

Re-injured knee (from 11//3/07 incident) while diving under truck to avoid potential fly rock.

Employee was walking in the #4 belt entry past the 10 s/c shuttle car that he operates when he slipped and fell in an uneven area of the roadway, causing him to injure his right ankle. He was transported & diagnosed with a fracture to the right ankle which resulted in lost workdays.

Employee slipped while mounting rock truck, striking his right rib area on the metal step bracket.

Employee was attempting to unclog powder from the hose that was connected to the discharge auger. The hose came off the discharge tube and while trying to avoid the falling tube, he twisted his right knee.

It was dark outside. He turned to come down the steps of the coal truck. He lost his footing and tried to grab the rail. As he fell his left leg hit the steps. He landed on his back and left shoulder.

Injured employee was dismounting the rear of a grader after checking the engine oil. As he placed his right foot on the ground, his ankle gave out causing him to fall and fracturing the right ankle. The ground surface was smooth where he stepped. He may have stepped down on the side of his foot.

Was carrying a timber past a mantrip. Foot slipped and fell forward onto mantrip.

Employee was dismounting a Cat D-9R dozer when he slipped and fell. *He did not start missing work until 12/10/07.

EMPLOYEE STEPPED IN ATF WHILE DOING P M ON RIMPULL WATER TRUCK AND WHEN HE STEPPED UP TO GET IN TRUCK HIS FEET SLIPPED AND HE FELL HITTING ELBOW ON STEP.

Employee was dismounting a dozer by stepping onto the track, and onto the push arm. As he was stepping from the track to the push arm, his left foot slipped and he tried to catch his fall with his left hand. The hand struck a piece of raised metal installed on the push arm for foot traction.

Stepped off catwalk of dozer while steam cleaning, falling to ground.

EE was standing on top of shuttle car hanging cable and slipped and fell to the mine floor.

Employee slipped and fell from drill deck, while attempting to adjust the pull down cables.

Employee alleges that he was injured while hanging a feeder cable. He was standing on a scoop & lost his balance, jumped from the scoop, & landed on his feet. Employee stated that he was OK.

Employee was hanging a curtain from the power center when he slipped & fell off of the power center.

EE stated he was working on in-line tailpiece. He was on ladder and fell 3-4 ft off ladder onto water line. *We are protesting this claim of injury because ee did not report it. Went to hospital on 3/26/07. We got form from Brick Street. Called ee and he couldn't remember date of injury (this is his guess).

Employee slipped and fell on drill catwalk.

Injured employee had stopped by the Warehouse prior to the start of his work shift to pick up some ear plugs. As he was walking between his personal vehicle and the Warehouse, he stepped on some ice and fell on the parking lot surface. The fall resulted in a fracture of his left femur bone.

Employee was in back of service truck # 61548 to move heater when he slipped & fell.

EE was working with other crew members taking a motor w/structure to another location when they de-railed the motor. They were working to jack the motor up to get it back on rail when he stepped off of the flat car. He states his knee "popped".

Employee was loading timbers from a scoop bucket when he slipped, and struck his hand on the side of the scoop bucket while holding onto a timber.

EE fell @ belt head. He didn't trip- just felt pain in his back.

Employee was standing in some belt structure pulling on a cable when he got his feet tangled in structure causing him to fall. He said he twisted his upper body as he fell and that he felt pain in his right hip and leg.

Driver exited cab, proceeded to hook up hoses to pump off fuel from tanker to storage tanks. Driver slipped and fell on wet leaves before he was able to retrive hoses from storage tray on tanker.

EE slipped on a rock while trying to clear leaves from a pond entrance channel to allow for proper mixing of water with pond treatment chemical. When EE slipped, he grabbed a tree to prevent fall. EE did not fall, but while preventing fall by grabbing tree, he tore tendons and muscle in left arm.

Employee was attempting to lift a 25 KV power cable into a junction box, he slipped and strained his groin.

Employee was climbing on powder bin, he stepped over Ibeam and twisted knee. Treated at local ER prescribed RX meds and time off work until additional analysis.

Employee was walking from feed hopper to his end loader. There was a windrow of coal about knee high and he attempted to step over other than walk around it. As he stepped over, he tripped on a lump of coal with his right lower leg striking a sharp piece of coal causing a puncture wound. He received two sutures in treatment of this injury. Missed no work.

Employee claims to have injured his back when he jumped off of a man-up & landed on his feet & then fell.He stated that he saw the top move.

Slipped and fell while mounting D10 dozer. Right elbow struck track on dozer.

Getting out of back of lube truck, tripped over a filter, falling on left hand injuring wrist.

Employee slipped, fell & injured his knee(R).

EE was climbing down ladder. His foot missed last step, causing him to fall to the ground. EE twisted knee. Saw Dr. on 6/1/06, given off duty, medication and referred to specialist.

While inspecting the truck he fell to the ground, landing on his left hand. Incident occurred on Tuesday, May 30. Sought medical attention on Thursday, June 8.

Alleged injury occurred when employee slipped and fell as he was dismounting the bottom steps of a rock truck. No witnesses, employee found on the ground with his leg draped over a rung of the bottom steps. No physical injuries were observed. Employee complains of back pain. No medical records to date.

He was exiting the truck & step swiveled with him, causing him to slip and straining his left shoulder.

Employee twisted his knee while trying to avoid a rubber pump hose that was being reeled up on a spool. * Note: Employee did not start missing work until 3/20/06.

Employee was pulling the plug out of the #2 scoop. The plug slipped & he fell back & struck his hand on the lightguard.

ground gave way - fell over 6 ft ledge - suffered a compound fracture at his left elbow
