Employee cut left hand while attempting to remove a plastic wire tie.

Employee stated that employee strained employee's right shoulder when employee's foot slipped from the step as employee was dismounting Cat excavator. Employee did not begin missing work until 6/1/2017

Fractured foot due to metal falling on the foot. I was dismantling a excavator bucket and it fell on my foot.

Employee stated employee felt pain in employee's right shoulder, right arm, back and groin area while attempting to move a power cable.

Employee states he was carrying a laptop up the stairs of #24512 830 truck. When he got to the top and turned he twisted his back causing a strain. On 8/7/2015 he went to CAMC Memorial and was treated by Dr. and diagnosed with a lower lumbar strain and taken off work.

Employee was operating #21909 994H Loader. Employee states the loader bucket struck a large rock while loading and stopped the loader immediately. Employee felt a strain in his lower back. on 7/30/2015 employee was treated by Dr. Hill and taken off work for a lower lumbar strain. Employee was released to return to work on 8/3/2015.

Employee states he was dismounting #20603 D11 dozer and his foot slipped on the bottom step. Employee struck the lower right side of his body on the lower step. On 06/22/2015 employee went to Logan Regional Medical Center and was treated by Dr. and diagnosed with herniated discs C-5 thru C-7 and bulging discs L-3 thru L-5 and taken off work.

Employee was making welding repairs to the Dragline bucket and a hot piece of metal burned a hole in his pants and went down inside his boot and burned the top of his left foot. Employee went to CAMC Urgent Care on 06/19/2015 and taken off work for a Partial Thickness Burn on his left foot.

Employee states he was mounting #21133 930H Loader and felt a pop in his right knee. Employee was treated at Boone Memorial Hospital on 7/14/2015 and 7/22/15 and given an MRI. On 8/17/2015 the employee was treated at Scott Orthopedic by Dr. and diagnosed with a torn meniscus and taken off work.

Employee dismounted his loader to check the coal and slipped on ice and fell straining his right shoulder. Employee was treated at Health Plus on 2/28/2015, 3/23/2015, 4/16/2015, and 4/30/2015 by Dr. Employee was taken off work 5/1/2015 with a contusion to his right shoulder and referred to an orthopedic specialist.

Employee bent over lifting a quick drop valve and felt pain in his lower back. On 2/10/2015 employee went to McKinney Family Chiropractic and was treated by Dr. and was released to return to work.

Employee was adjusting the height of an adjustable Caterpillar jack stand. The tube slipped from his hand and his little finger on his right hand was in the bottom adjustment hole and the tip of his finger was amputated. Employee was transported to CAMC General but the tip could not be re-attached.

Employee stated that he was mounting his coal loader and experienced pain in his left wrist.

Using a torch to cut out bearing and hot metal flew off into Left ear.

Employee was operating #24909 789 Haul Truck. As the loader began to dump the first bucket, employee stated that he was jarred when a rock and material struck the truck bed. Employee stated that he felt pain in his neck, back, and legs. He was transported to CMAC where he was diagnosed with a neck/back strain.

Employee was dismounting a D-10 Dozer. He stated that his foot slipped off of the bottom step and he fell to the ground. The ground in this area was level, dismounting hand rail and steps were in good condition. Employee was wearing his gloves and hard hat at the time of the fall.

Employees were attempting to install a left rear wheel and tire onto a motor grade 16M at the shop. The wheel was positioned to be mounted to the grader hub and employee began starting the bolts. The wheel shifted and pinched his finger between the wheel and bolt head. He finished his shift, went to the hospital and received several stitches.

Employee was grinding on #21302 992G loader. Grinding dust settled in his hood and when welding helmet lenses came down material went into employee's right eye. Employee went to Mountain State Eye Associates and was seen by Dr. A piece of metal was removed that was free floating but employee received a prescription.

Employee was operating a 630 Rock Truck. He went over to cut off the heater valve. He stated that when he lifted the radiator cover, he strained his right shoulder. He finished his shift and continued to work until 6/18/2014.

I went to the tool room to get a new tip and nozzle for my welder, and I got tangled up in the torch hoses, and fell out of the tool room on to the ground. After getting up something popped in my back and that is when I tried to walk the pain out but it didn't help and I decided to go to the Emergency Room to get checked out.

Employee was pulling power cable to put on the tub at the Dragline. His right foot slipped twisting his lower back. On 2/28/2014 employee went to Logan Regional Medical Center and was examined by Dr. and diagnosed with a Lubosacral Strain and taken off work and referred to a specialist.

Employee states he hit a dip in the road at the bottom of the 495 Shovel ramp causing pain to his lower back. Employee was examined by Dr. and taken off work and referred to a neurosurgeon.

Employee states he backed over a rock operating #20529 D10 dozer and was jarred straining his left leg and lower back. Employee was examined by Dr. at Lincoln Primary Care on 2/11/2014 and taken off work for a strain to his lower back.

Employee stepped back away from #23501 993K Loader after dismounting and stepped on frozen rock and fell backwards to the ground. Employee felt pain in lower back and groin area. Employee was transported to CAMC General and examined by E.R. Physician. Employee was diagnosed with a muscle strain to the back and taken off work for three days.

Employee was operating #20548 a D10 Dozer on the dump and Truck #24903 a 789 backed up and struck the cab area jarring the employee. Employee completed his shift and went to Logan Regional Medical Center and was diagnosed with a Thoracic and Cervical Sprain and taken off work till 1/3/2014. Employee was examined by a specialist on 1/3/2014 and taken off work thru 3/3/2014.

Employee states that he slipped after stepping to the ground from #23207 16M Grader. Employee states he fell on his left side injuring his left shoulder, elbow and hip. Employee was examined by Dr. on 12/3/2013 and diagnosed with a strain/sprain of the left hip and lower lumbar. 12/3/2013 was the employee's first day away from work and is now a Lost Time Injury.

Employee was grinding brackets on a ladder platform. Employees left eye started itching and he removed his hood, safety glasses, and gloves and rubbed his eye. A small metal particle got into his eye and had to be removed by an Ophthalmologist and he was given prescription eye drops to prevent infection.

7/27/2013 employee advised that he jumped from the push arm on dozer injuring his back and right knee. 7/29/2013 employee went to Med Express and was treated for a lumbar strain relating to a prior injury including degenerative disc disease. 9/11/2013 employee started missing work. The insurer having found this injury compensable, this is being reported as a Lost Time Injury.

Employee states he was dismounting his dozer a D11 R and slipped on the step beside the engine compartment and fell between the dozer and tracks. On 7/28/2013 employee was treated at Boone Memorial Hospital by Dr. and diagnosed with a lower lumbar and left shoulder strain and taken off work. This is now a lost time injury.

Employee states he was lifting an 8 inch plastic pipe and felt a strain to his lower back and stomach area. Employee was seen by Dr. and taken off work on 7/25/2013 with a lower lumbar strain and lumbar disc and nerve damage. This is now a lost time injury.

Employee states he was installing an exhaust turbo on D10 R dozer and felt a sharp pain and burning in his lower back. Employee was transported to CAMC General and diagnosed by Dr. with a lower lumbar strain and taken off work and this is now a lost time injury.

Employee was being loaded by the 495 Shovel. The Shovel had an electrical fault and the bucket struck the rear of the truck injuring his back, neck, and left leg. Employee was transported to CAMC General by Boone County Ambulance and diagnosed with a cervical and lumbar strain and taken off work thru 5/21/2013. This is now a lost time injury.

Employee was inside the 1570 Dragline and slipped and fell down steps injuring his hip and lower back. Employee was treated by Dr. on 5/8/2013 and was taken off work with a lumbar strain. This is now a lost time injury.

5/2/2013 employee advised he slipped and fell crossing over a lift jack on a dozer injuring his neck/shoulder. 5/5/2013 he went to ARH Emergency Room and was examined by Dr. and released. 5/6/2013 he went to Dr's office and taken off work with a neck strain. The insurer having found this injury compensable, this is being reported as a Lost Time Injury.

Employee was being loaded by the 495 Shovel and the bucket struck the right rear of the truck injuring the employee's left shoulder. Employee was transported to Logan Regional Medical Center and referred to a specialist. Specialist took the employee off work with a sprained left shoulder.

Employee states on 2/22/2013 he injured his back while installing a cover plate on a 993K loader. Employee sought medical treatment and was diagnosed with a lumbar strain. MRI revealed degenerative disc disease. The insurer having found the injury compensable, this is being reported as a lost time injury.

1/22/2013 employee advised his truck slid and struck the berm straining his back. Employee was examined by Dr. on 4/4/2013 and diagnosed with a back strain/sprain and an MRI was requested which revealed degenerative disc disease. 7/22/2013 employee started missing work. The insurer having found the injury compensable, this is being reported as a Lost Time Injury.

Employee stepped off platform and did not see a 1" X 16" bolt. Employee slipped and fell striking his right arm and elbow on a junction box. Employee was transported to CAMC General and was given x-rays with negative results and released. Employee went back to CAMC General and was taken off work due to swelling. Severe contusion and strain.

Employee notified the company on 1/18/2013 of an incident that supposedly happened six days prior. Employee stated he backed up and ran over a rock. A proper investigation could not be completed and employee started missing work on 2/13/2013. Employee was diagnosed with a cervical strain and ulnar neuropathy and is now compensable and being reported at this time.

Employee was operating a D10 dozer #20550 and backed over a rock with his left track jarring him and straining his back. Employee went to Thomas Memorial Hospital after work and was diagnosed with an acute cervical sprain and was taken off work for two days.

Employee stated he was taking a hubcap off a Komatsu 630E and slipped and fell to the ground injuring his left hip and right knee. Employee's family physician referred him to St. Marys Occupational Health Center. Dr. diagnosed him with a sciatic nerve issue on his left hip and sprained right knee and took him off work.

Employee reached down into his box to retrieve a small toolbox and when he pulled up on the small toolbox felt a burning sensation in his genital area. Employee went to CAMC Memorial on 10/06/2012 and was diagnosed with a direct inguinal hernia and orchitis.

Employee was descending the steps on the HD325 fuel truck and he slipped and caught himself with his right arm straining his right shoulder and neck. Employee completed his shift and went to Williamson Memorial Hospital and was taken off work for a possible pulled muscle.

While a customer was loading a 50' long and 30" tall section of pipe, it struck the driver knocking him off the high flat trailer.

Employee placed a ladder on the right rear track of a D-11 R dozer. Employee climbed up the unsecured ladder and attempted to turn the fuel shutoff valve back on. The ladder slipped and the employee fell to the ground. Employee was taken off work for a lower lumbar strain and contusions to the left hip and back.

Employee was using a chain hoist to pull the track back together on the 495 Shovel. Employee felt pain in his left side and middle back. Employee completed his shift. Employee went to HealthPlus and was seen by Dr. and was taken off work for a possible pulled muscle in his back.

WHILE FUELING EXCAVATOR AT FUEL TANK EMPLOYEE SLIPPED ON EXCAVATOR TRACK AND FELL OFF TRACK AND HIT FUEL TANK.

Employee was using a dull pocket knife to cut a rope. Placed the knife in a loop and pulled the knife toward himself and the knife came out of the loop and jabbed into his left elbow area. Required two sutures to close the wound.

Employee was attempting to walk across the rear platform of #21305 992 G Loader and lost his balance. The employee had to jump to the ground and slid and fell on his buttocks fracturing his left foot.

Employee was stepping down off his drill and his right hip and knee gave out and he fell to the ground. MRI still not completed at the time of this report.

Employee stated she hit a hole in the roadway and jarred her neck, shoulder and lower back. Employee missed first shift on 6/13/2011. This accident is now reportable. Unable to obtain any medical records until E.R. Doctor signed off on initial visit.

Employee was inside the first floor of the 1570 Dragline and stepped over a manhole and slipped and fell on the drag trough rail and fractured two ribs.

Employee was stepping down to the lower level on the walk-way around the fuel tank of the anfo trailer. He slipped and fell into a metal support beam from the trailers frame, resulting in bruising of his right side rib area.

Employee was installing a drag chain repair link. Employee placed his hand on link and other link fell over and smashed his little finger on his left hand. X-ray revealed a fracture on last joint and he recieved four sutures to close the wound.

Employee was pulling on an air hose and slipped in the middle of the haul road and landed on his left hip causing a contusion. On 9-27-2010 saw Doctor and was referred for an MRI. MRI was given on 10-20-2010 and results were obtained on 10-27-2010 and were negative. This is now a lost time reportable injury.

EE backed up to and thru the berm and turned the below listed truck over causing a laceration to his head and left shoulder area. EE was transported to Logan Regional Medical Center by Boone County ambulance where he received 7 sutures to his head 17 sutures to his shoulder and was released.

Employee was removing segment bolts from Unit #20524 a D10 Dozer. Employee was using a breaker bar and the socket slipped off the nut and struck the employee in the chin and mouth area. Employee was transported to CAMC General were he recieved 5 sutures to his chin and 5 to his lower lip and also chipped a tooth. Employee returned to work the next day.

Employee slipped/tripped/fall on the steps leading onto the 495 shovel. When the employee fell he hit his mouth on another step resulting in a laceration that required 5 stitches.

Employee was stoning the #3 motor on the 1570 Dragline. While stoning the motor it flashed causing a burn to the top of his left hand. Employee continued to work and went to the doctor on 6/14/2010 at which time the doctor took him off until 6/21/2010 and he did return to work on this date.

Employee was lifting a grid box cover off unit #24602 a 630 Komatsu truck. While lifting the cover off his little finger on his left hand got caught and it pinched the nail and tip of his finger off. He was transported to Logan General Hospital where he was referred to a specialist. He was seen by Dr. Culpepper a plastic surgeon who applied nine sutures to close the wound.

Employee was welding on a excavator bucket. Employee reached over to pick up a shank and felt a twitch in his lower back. Employee did not miss any work until 6/11/2010 and had surgery on 6/14/2010 to repair disc L5/S1.

Checking battery cables when battery cover door fell on left hand, smashing & cut left middle finger.

Employee was struck in the mouth by a dewatering pump, requiring seven sutures to his lower lip.

Employee mounted a ladder and did not secure it. The ladder shifted and the employee fell to the ground. Employee was transported to CAMC General Hospital and x-rays were negative and referred to a specialist. Employee saw another doctor who advised that he had a compression fracture at L-2 and this accident became reportable 7-20-2009.

Employee was using his right hand to cut a strap. He was using a utility knife and after cutting the strap the knife struck the palm/thumb area of his left hand causing a laceration. He was taken to CAMC General and recieved 8 stitches. He was seen by a specialist on 04-03-09 and was given a full release to return to work.

Employee was lifting a ladder back up and felt a pull and pain in his lower lumbar. Employee was referred to a specialist and has been off pending an MRI. Employee was referred to a Dr. after results of the MRI. Employee has three herniated discs in his lower lumbar and is now reportable due to not being released to return to work.

Employee was operating a 789 Rock Truck and was about to be loaded. A 992 K Loader pulled up to dump into the bed and the loaders front tires encountered a mud seam and three rocks fell out of the bucket and inside the truck bed, jarring the employee. He was diagnosed with a back strain and was scheduled for an MRI on 02-13-09.

Employee was mounting his truck after checking the oil filter for a leak. The employee slipped and caught himself with his left arm, causing a burning sensation in his left arm and shoulder. Employee was diagnosed with a strain by a specialist and will follow-up on 02-17-09 with the specialist.

Employee was trying to pry a deck bushing back into place and slipped and fell mashing his right thumb between a slate bar and dust curtain bar. Employee finished his shift and was diagnosed at Boone Memorial Hospital with a dislocated and fractured thumb. After seeing specialist there was no fracture or dislocation only tissue damage.

Employee was dismounting a D-11 dozer. He stepped on a round rock with his heel and he twisted his left knee. The employee finished his shift. On 10-17-08 he went to his family physician who referred him to a specialist. The specialist advised that he had a torn miniscus and needed surgery. He missed his first shift of work on 12-29-08 and had surgery on 12-30-08.

Fitting broke on the steering valve spraying hydraulic fluid on the turbo which ignited. Employee then jumped from the truck and received 1st and 2nd degree burns to his arm and ear. No damage was done to the truck.

A forklift operator was placing a load of boring equipment onto a tractor & trailer. While loading the boring equipment, the container was bumped and a 2 1/4 inch bar jumped out of the cradle and fell from the trailer. The employee on the trailer jumped down when the bar fell and the bar bounced and hit the employee in the lower left leg, resulting in a fracture to leg.

Employee placed the vehicle in park. After placing the vehicle in park his right hand came into contact with the exit door handle holding arm causing a laceration to the top of his right hand requiring five stitches.

Employee's foot slipped while mounting oil check ladder, injurying his right ankle.

Employee's thumb was cut when cab door was shut unexpectedly.

Employee was struck on the right hand by a high voltage fuse that fell from the hot stick. * Employee did not start missing work until 3/10/08.

Cutting cable with torch, burned left knee, 2nd degree burn.

While mounting dozer, employee felt a "pop" in his shoulder. He was diagnosed with a rotator cuff injury. NOTE: Employee did not begin missing work until 4/16/08.

Employee slipped while mounting rock truck, striking his right rib area on the metal step bracket.

Employee was operating a D11 dozer. He backed over a rock and was jarred.

Employee was dismounting his mechanics' truck when he struck his right upper arm on a vice mounted on the trucks' bumper. He suffered a laceration that required 5 sutures.

A dog was struck by truck in front of guard shack so employee attempted to render aid for canine. It became aggressive and bit his left leg below the knee so he called the sheriff's dept. Dog was destroyed; employee was treated in ER and returned to work next day. Must take shots.

End loader lunged backwards, throwing employee forward against the seatbelt. Employee was diagnosed with a strained abdominal muscle. NOTE: EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 8/21/07.

Cleaning engine mounts, got debris in eye. Was using a die grinder with a Scotch Brite pad. Rotation threw debris under safety glasses.

Employee slipped and fell from drill deck, while attempting to adjust the pull down cables.

Employee slipped and fell on drill catwalk.

Employee has been diagnosed with carpal tunnel syndrome.

Employee has been diagonsed with carpal tunnel syndrome.

Metal entered employees' eye while grinding. However, employee did not go to the doctor until 12/04/2006 then reported injury to employer 12/05/2006.

Employee was attempting to lift a 25 KV power cable into a junction box, he slipped and strained his groin.

Employee stated that he backed a D-11 dozer over some uneven ground; the rear of the dozer came down hard, jarrring him and straining his lower back.

Employee called and reported he was under doctor's care for a lower back problem on 9-27-06. Medical records show no changes since previous exam in 2004.

Employee cut a tree. As the tree fell a dead limb became dislodged and fell striking him in the left shoulder. Limb was not seen during preshift or puring observation of tree before cutting.

Employee lost his balance and fell approximately 15 feet from the shovel manlift.

EE complained of back (lower) pain after boarding his haul truck. EE finished this shift and returned for his following shift on 7/29/06. He completed the 7/29/06 shift. He did not miss work until 7/31/06 Monday.

Employee slipped and fell while attempting to check the engine oil during his preshift inspection.

Employee was using a heating torch (Rosebud) on a bar on a 777A. After finishing he laid the torch on the truck bumper, then when removing the adapter the torch was still hot burning his right hand.

Employee was dismounting crawler on 495 shovel in a different locatin from where he boarded. Was coming down a ladder- the ladder slipped and employee fell approximately 4 feet.

EE was grinding on swing pinion using a die grinder. Jam nut came off and struck EE in right eye breaking his prescription eye glasses.

Employee smashed his finger with a hammer.

Employee was lying down on the revolving frame welding, went to get up and hurt his back.

Carpal Tunnel syndrome

TIGHTENING BELT ON A/C COMPRESSOR WITH PRY BAR, FELT A "POP" IN HIS SHOULDER.

Employee twisted his knee while trying to avoid a rubber pump hose that was being reeled up on a spool. * Note: Employee did not start missing work until 3/20/06.

MRI report shows employee is off work due to Degenerative Disc Disease. Employee began missing work on 3/14/06.

Employee was diagnosed with heat exhaustion.

Employee pinched his finger between a sledgehammer handle and a metal bulk prill bin, resulting in a fracture to his left ring finger.

Installing right steering cylinder, driving in bearing using hammer, hammer slipped out of hands hitting employee in left leg above knee.

EMPLOYEE WAS ARC GOUGING METAL WHEN SOME METAL BOUNCED OFF THE WALL BEHIND HIM AND ENTERED INTO HIS WELDING HOOD AND INTO HIS EYE, ALSO EYE WAS BURNED BY WELDING RAYS.

Employee was striking a loader tooth with a hammer when a metal fragment struck him in the left thigh.

Employee was tightening segment bolts on a D10 Dozer with an impact wrench. He suffered an abdominal strain or hernia. * Note: Employee did not start missing work until 4/14/05.

Employee tripped over tire and fell (torn ligament in shoulder - surgically repaired on 6/2/05).

Employee was removing a crib block from underneath a D10 dozer track after the track had been bolted together. Two of the track pads came together, severing the tip of the employee's right little finger just below the fingernail.

D11N Cat dozer caught fire in the engine compartment. Dozer appears to be a total loss. No reportable injuries.

Employee stepped in an open 8" drill hole, fell backwards to the ground and his foot became stuck. His foot was quickly dislogded from the hole and he suffered strains/sprains to his right hip and lower back.

Employee was trouble shooting a brake problem on the Cat Lowboy trailer. He began to loosen the bleeder plug to check for pressure. The bleeder plug blew out of the cylinder causing fluid to spray into the employee's eyes.

Employee's portal vehicle was struck by a deer while driving on mine property. The air bags deployed causing the driver to enter the ditch and into a sump along the roadway. Employee suffered a fracture to his left ring finger. * Medical records diagnosing and confirming the injury were not available until 12/10/04.

EE WAS PULLING FUEL HOSE TO TRUCK TANK, FELT PAIN IN LOWER BACK. DID NOT REPORT UNTIL 11/08/2004, TREATED BY DR, GIVEN RX AND ONE WEEK RESTRICTED DUTY.

After boarding the 16G grader, the employee lost his balance, turned and jumped to the ground. He sprained his ankles and left shoulder. * Note - Employee did not start missing work until 10/6/04.

Employee bent over to pick up a piece of nylon rope and experienced pain in his lower back. He was diagnosed with a lumbar strain.

Employee was using a razor knife to cut an old radiator hose. His forearm came in contact with the open blade, resulting in a cut that required 14 stitches to close.

EMPLOYEE WAS CHANGING BRAKE ROTORS ON HAULPAK 830 TRUCK. THE WRENCH SLIPPED & HE HIT HIS RT. ELBOW ON CALIPER. THE NEXT DAY HIS ELBOW STARTED TO SWELL & HE WENT TO THE HOSPITAL TO BE CHECKED. INFECTION CAUSED LOST TIME AND EE WAS SCHEDULED OFF THE FOLLOWING WEEK.

Employee was using an endloader to move a large rock from the dig face. A section of the rock broke loose, causing the endloader to jar the employee resulting in a strain to the neck and shoulder.

Employee fell from the bumper of his service truck. He was attempting to raise the hood and was using the grill section as a grab handle. The grill section broke away, causing him to loose his balance and fall onto the concrete shop floor. He has contusions to his lower back and left arm.

Employee was ejected from a hydraulic manlift and fell approximately 12-14 feet to the ground. He received fractures to his left shoulder, left knee and left femur.

Employee was struck by a flying piece of metal from a dozer track which was being unloaded. He suffered a laceration to his stomach and arm.

Employee came in contact with a dozer push arm that he was welding on and was burned.

Employee turned to look rearward while backing his loader down a slight incline and strained his lower back.

Repetitive motion, trigger finger.

EE came to work not feeling well. An ambulance was called and the ee apparently suffered a heart attack in the ambulance in route to the hospital.

EMPLOYEE WAS STRUCK ON THE LEFT INDEX FINGER BY BOLT/NUT FROM CUTTING EDGE ON 575 KOMATSU DOZER. *NOTE: EMPLOYEE DID NOT VISIT DOCTOR UNTIL 1-16-04. MANAGEMENT WAS NOT NOTIFIED OF THE DIAGNOSI S UNTIL 1-17-04. MEDICAL RECORDS WERE NOT OBTAINED UNTIL 1-21-04.

AFTER MAKING DELIVERY, DRIVER UNCOVERED 3"HOSE FROM STORAGE TANK. HE FAILED TO RELEAVE PRESSURE BY UNCOUPLING ONE SIDE AT A TIME, AS TRAINED 2 WEEKS PRIOR. ONCE UNCOUPLED THE HOSE FLEW BACK STRIKING HIM IN THE UPPER LIP CAUSING A DEEP LACERATION. HE SOUGHT IMMEDIATE TREATMENT BY PRIVATE TRANSPORTATION.

EE WAS ATTEMPTING TO RAISE A LARGE ROCK WITH THE ENDLOADER BUCKET. THE ROCK BROKE WHICH RESULTED IN A JARRING ACTION TO MACHINE. STARTED MISSING WORK 3/19/02.

EE OBSERVED ANOTHER EE FALL FROM A DOZER. AS HE RAN TO THE AID OF THIS OTHER EE, HE TRIPPED AND FELL, INJURING HIS HAND.

EMPLOYEE ALLEGES TO CONTRACTING COAL WORKER'S PNEUMOCONIOSIS AS A RESULT OF HIS EMPLOYMENT WITH HEBERT MINING AND WAS GRANTED A 5% AWARD BY THE WV WORKER'S COMPENSATION DIVISION.

EMPLOYEE WAS MAKING A GRADE CUT WITH A DOZER WHEN BLADE CAUGHT ON AN EMBEDDED ROCK SUDDENLY STOPPING THE MACHINE, THEN THE ROCK BROKE, RESULTING IN THE DOZER SUDDENLY MOVING FORWARD.

EMPLOYEE WAS OPERATING AN ENDLOADER ALONG THE TOE OF THE HIGHWALL WHEN THE CORNER OF THE BUCKET CAUGHT ON A SOLID PIECE OF THE WALL, RESULTING IN A SUDDEN STOP.

EMPLOYEE WAS BOARDING DOZER, AS HE WAS IN THE PROCESS OF STEPPING FROM TOP OF BLADE PUSH ARM TO CRAWLER HE FELT PAIN IN HIS BACK. WE ARE CLASSIFYING THIS INCIDENT AS THERE IS NO INSTANTANEOUS TRAUMATIC EVENT.

WAS STANDING ON THE SNOW COVERED REAR BUMPER OF HIS SERVICE TRUCK. SUDDENLY HIS FEET SLIPPED ANDHE FELL FROM THE BUMPER, STRIKING HIS RT. LEG ONTHE EDGE OF THE BUMPER.

EMPLOYEE HAS BEEN DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME.IS SCHEDULED FOR SURGERY 2/5/2002

EE WAS USING A B&O HAMMER AND A 3LB. SLEDGE HAMMER TO REMOVE A PIN FROM AN ENDLOADER BUCKET TOOTH. AS HE WAS SWINGING THE SLEDGE HAMMER, IT GLANCED OFF THE TOOTH BESIDE THE ONE HE WAS WORKING ON. THIS RESULTED IN THE HAMMER STRIKING HIS LT.INDEX FINGER.

EE WAS BOARDING AN ENDLOADER. HE MAINTAINED 3 POINTS OF CONTACT. DID NOT SLIP. EXPERIENCE PAIN INHIS RIGHT LOWER BACK. I HAVE CLASSIFIED THIS INCIDENT AS AN ILLNESS, DUE TO NO TRAUMATIC EVENT.

EE STEPPED OFF A D10N DOZER AND EXPERIENCED PAIN IN HIS BACK AND FELT NAUSEATED NO TRAMATIC EVENT.

EE WAS WALKING TOWARDS THE EQUIPMENT PARKING AREA WHEN HE SLIPPED AND FELL ON THE PACKED SNOW.

EE ALLEGES TO CONTRACTING COAL WORKERS' PNEUMOCONIOSIS AS A RESULT OF HIS EMPLOYMENT WITH THIS MINING CO.& WAS GRANTED A 5% AWARD BY THE WV WORKERS' COMPENSATION DIVISION.

EMPLOYEE WAS DISMOUNTING A ROAD GRADER, LOST HIS GRIP ON THE RIGHT GRAB IRON AND FELL TOT HE GROUND, APPROX 4 FEET.

EE WAS OPERATING A DOZER & BACKED OVER A LARGE ROCK, RESULTING IN A SUDDEN JARRING OF MACHINE.

EE WAS DUMPING A LOAD OF COAL ON A STOCKPILE & THE TAILGATE MALFUNCTIONED CAUSING THE TRUCKS FRONT END TO SUDDENLY RAISE & JUST AS SUDDENLY, DROPPED DOWN.

EE WAS LIFTING A 8 LB. BUCKET OF BIT GREASE AND EXPERIENCED A BURNING SENSATION IN HIS GROIN AREA. MANAGEMENT WAS INFORMED ON 1/28/02 THAT THIS INJURY RESULTED IN A HERNIA AND EE WILL START MI SSING WORK 1/29/02.
