Employee was stepping down to the catwalk from the conveyor after washing out the lime chute. As EE was stepping down, EE lost footing and slipped. As EE attempted to catch self, EE grabbed the metal structure of the baghouse with right hand. This caused EE to extend and twist the right shoulder. ER diagnosed with contusion to the right shoulder.

EMS was called and confirmed the low blood sugar/low BP at the time of the fall and additional ankle pain. EE refused further treatment that day and was cleared by doctor the next day for low blood sugar/BP, but was kept off work for ankle.
Employee's foot slipped off of ladder, in grabbing to stabilize self, EE cut EE's middle finger and palm on the metal guard. EE is at work, did not miss any days but is on restricted duty.
EE entered the bed of a pickup truck to retrieve a tool. The tailgate was in the up position and after retrieving the tool, the EE attempted to exit the pickup truck facing forward and caught trailing foot on the top of the tailgate and fell forward injuring their knee. EE was taken for evaluation and returned to work. On 4/13/21 orthopedist ordered EE out of work.
EE entered the primary hopper to assist with dislodging a rock hung up in the grizzly bars at the jaw crusher. While EE's left foot was firmly planted and wedged into a tight location EE lost footing with EE's right foot, causing EE's left knee to twist. The employee was taken to a local clinic for evaluation where it was diagnosed as a strained MCL and torn meniscus ligament.
Office employee fainted, due to an ongoing personal medical issue, and fell striking right shoulder on the ground. This resulted in a dislocated shoulder and small fracture to one of the bones in the shoulder.
EE EXITED CAB AND WALKED ACROSS TOP OF RADIATOR TO CLEAN RIGHT SIDE VIEW MIRROR. WHILE RETURNING TO CAB EE AGAIN WALKED TOP OF RADIATOR AND FELL APPROXIMATELY 8 FEET TO THE GROUND BREAKING LEFT LEG.
EE was attempting to access the cab of the drill when they stepped onto the rear section of the track and then onto the first step, when their muddy right boot slipped across the surface. When catching themselves and trying to pull themselves up into the cab they strained their back.
Employee was working on the #1 conveyor belt at the big jaw crusher that was choked down with material. Upon completion, employee noticed more rock underneath of the belt. As employee reached to remove the rock, the belt started, causing employee to jump clear of the belt and upon landing, right ankle rolled over causing injury to right ankle.
While standing on the fuel tank, steam cleaning 769C Cat truck, employee stepped backwards and fell approx. four feet injuring left foot. Medical treatment resulted in modified work activity.
