The employee (contractor) was working with a belt mechanic installing skirts in the tailpiece at the T-1 belt drive on the #5 belt. The employee was crossing the conveyor belt at a crossover for additional supplies. As he exited the crossover his right foot slipped resulting in a twisted knee.

The section mechanic had completed repairing the water supply line to the continuous miner which was located in the #4 right crosscut. As he was traveling through the crosscut to the #5 heading to return his tools to a personnel carrier he tripped on a rock measuring 2.5 x 5 x 6 inches and injured his left knee area.

The security guard was performing a walk around patrol when a patch of snow covered ice collapsed under her foot. This created a hole 6 inches deep and 12 inches wide resulting in her falling and fracturing her right wrist.

Employee was walking from the No. 4 entry to the No. 3 Entry on the 001-0 section on a slight downhill grade. His right foot slipped and he fell on his left foot, fracturing his left lower leg and ankle.

EE was installing a replacement water line over the section tailpiece where the feeder dumps into the beltline. EE stepped off of the tailpiece when a shovel that was laying upright against the tailpiece caught his buttock and caused him to fall backwards. EE landed on the section booster pump and fixtures. EE was placed on restricted work duty on 10/22/09.

The employee had come out of the mine after working a couple of hours late. As he was traveling down an inclined walkway from the mine shop to the bath house, his foot slipped in some loose gravel.

THE EE STATED HE WAS PULLING LINE CURTAIN IN THE #3 HEADING ACROSS THE LAST OPEN CROSSCUT AFTER HE HAD BOLTED THE PLACE. AS HE WAS PULLING ON THE CURTAIN, HIS RIGHT FOOT SLIPPED FROM UNDER HIM AND HE FELL BACKWARD ONTO HIS LEFT FOOT.

The ee was installing roof bolts in the 1st cut that had been taken out of the No. 3 left crosscut. As the ee began walking to the head of the bolter from the tram station, he stepped upon a step up in the mine floor. As he stepped upon the 4" step up his foot slipped off and the ee twisted and hyperextended his knee.

STARTED ACROSS TAILPIECE SLIPPED AND FELL TWISTED KNEE. #6 TAILPIECE AT #7 DRIVE.

2 OTHER MEN HELPING EMPLOYEE JACK TRAM UP TO GET BACK ON THE TRACK WEH JACK BAR SLIPPED OUT OF THE JACK CAUSING EE TO TRIP AND FALL BETWEEN TRACK & RIB & THAT'S WHEN HE PULLED SOMETHING IN HIS NECK.

EE HAD TURNED BELT OFF AT TAILPIECE, GOT UP ON TAILPIECE AND WAS GOING TO CROSS OVER WHEN HIS FOOT SLIPPED AND HE STRATLED TAILPIECE HITTING HIM IN THE GROIN.

THE EMPLOYEE INJURED HIS NECK DURING AN ALTERCATION AT THE HEAVY EQUIPMENT SHOP, LOST HIS BALANCE AND FELL TO THE GROUND.

EMPLOYEE WAS CARRYING SAE2 CLEANER (70-80 LB) FROM TRUCK TO SILO. WALKWAY WAS NARROW. HE TRIPPED ON ROCK/COAL IN GRATE AND FELL ON SAME LEVEL.

EE WAS HAULING ROCKDUST TO FACE ON UNIHAULER. ITGOT STUCK ON SLICK HILL. HE WENT TO THE BACK OF THE UNIHAULER TO GET ROCKDUST TO DRY UP HILL. HESLIPPED & FELL INTO SIDE OF UNIHAULER.

BUILT UP REFUSE ON CABLE CALMP ALLOWED EE TO LOSE HIS FOOTING AND FALL APPROX. 10 FEET.
