Injured was strapping down supplies on a supply trailer and slipped and fell injuring his shoulder and wrist.

Employee was on a hill when he slipped and came down on his leg wrong.

Employee was dismounting a ladder when he fell straining his back.

Employee was pulling cable out of the way when he tripped and fell landing on his rescuer. At the time of the incident employee didn't take any medical treatment. Employee is now having pain and is seeking medical treatment.

Employee was walking through the yard when he tripped on banding and fell striking his lip against a mandoor. Employee was taken to Good Samaritan sustaining a laceration to his lip that required stitches.

Employee was walking in a crosscut when he tripped over a rock and fell to the ground. Employee complained of back pain but didn't received doctor's care. Employee was treated by the doctor on March 1, 2013 and was ordered to remain off work for an uncertain period of time.

Employee was loading supplies for the bolter when a rock fell startling employee. He stumbled over supplies and fell, he tried to extended his arm trying to catch himself and felt crack or pop. Employee was taken to Good Samaritan, sustaining a fracture that has to be surgically repaired and will remain off work for an uncertain period of time.

Walking, carrying boards, stepped in hole, twisted ankle, ankle is sprained and muscles and ligaments pulled.

Employee was rolling up the curtain for the miner when he stepped in a hole rolling his ankle. Employee went to doctor where he was ordered to remain off work for an uncertain period of time.

Employee was walking when he tripped over a berm and cables falling to the ground. Employee was taken to Good Samaritan where they couldn't diagnose his condition of his ankle at the present time due to a pre-existing condition. Employee has to see specialist in Indianapolis and will remain off work for an uncertain period of time.

Employee was alone, switching rides when he stepped out of mantrip and his back "Popped". Employee worked the rest of shift, didn't report accident to foreman and worked the next couple days. Employee called in on the 13th and reported accident and came in on the 14th to file report. Went to Dr. where he was ordered to stay off work for an uncertain period of time.

A surface mechanic was removing a step ladder from the bed of his work truck, as he was removing he lost balance and fell to the ground. He tried to break his fall and sustained a fracture to his wrist.

Injured was climbing off of his coal hauler and hit his right leg on the steering cylinder. Injured was seen later in the week by an orthopedic and a tibial platueau fracture was discovered.

Employee was constructing a stopping when he tripped and fell hitting his lower back on a half block. Employees SCSR was located on his lower back in which is where his pain originated. Employee finished his shift then went to Good Samaritan in Vincennes, Indiana where the doctor ordered him to stay off work for an uncertain period of time.

EE was stepping over car cable with cable under his foot to pick up curtain when equipment was energized and cable reel came on. EE was thrown from equipment, resulting in a dislocation to his right shoulder. EE has since then been referred to an Orthopedic Surgeon for possible surgery and will be off work on Workmans Comp for an uncertain period of time.

Employee was stepping onto a crib when he twisted his right knee resulting in a torn meniscus. Employee will be off work for surgery and therapy for an uncertain period of time.

Injured was changing batteries on a scoop, when he lost he footing and fell into the corner of the scoop bucket. Injured sustained a cut lip and a chipped tooth, making this a reportable incident.

Employee had just washed off dozer and was performing the required pre - operational check, and about to check the fire extinguisher when he slipped off of the equipment landing on his left wrist. Employee sustained an incomplete non - displaced fracture of the left radius.

Employee had just returned from 4 days off on a 10 on 4 off rotation. He was walking down the steps in the breaker building and slipped as a result of mud on his boots, striking the handrail with his right arm.

EE was carrying belt rollers through the crosscut when he tripped on metal band canding falling onto left shoulder. EE continued to work until 09/10/09 when he had surgery on shoulder.

Stepping down from drop deck of semi-trailer, lost footing, fell and broke right knee and leg.

Employee stepped down off the operator's deck onto the ground and stepped onto a loose rock on the ground causing him to twist his ankle.

EE was climbing down a ladder that had not been tied off at the top. When he was about 5'-6' from the bottom of the ladder, it slipped out from under him on the floor. When the ladder fell, he fell on top of the ladder. The ladder was not secured at top because he was getting ready to replace it.

Employee stepped out of shuttle car into hole in mine floor and twisted right knee.

Employee was pulling on Rockdust hose in crosscut when he let hose go and attempted to walk out of crosscut 11 on the 2 West 1. He tripped over rock and fell dislocating right thumb. Employee missed work on 8/10/07.

EE WAS DOING REPAIRS IN #2 SUMP. HE LOST HIS FOOTING WHILE MOVING AND FELL. AREA WAS WET AT THE TIME.

Employee was replacing a cover over main breaker panel on the battery scoop. As he was replacing the cover he lost his footing causing him to drop the lid on his hand.

EE WAS WALKING WHEN HE STARTED TO FALL AND TWISTED RIGHT KNEE. EE CONTINUED TO WORK AFTER TWISTING KNEE. MRI TEST REVEALED BONE SPUR ON RIGHT KNEE REQUIRING ARTHROSCOPIC SURGERY ON 3/7/05. ACCIDENT BECAME A LOST TIME INJURY ON 3/7/05.

CLIMBED ON TO TRAILER TO CHECK TARP, THEN JUMPED OFF & SUSTAINED A SIMPLE FRACTURE OF LEFT LEG.

EE WAS WALKING DOWN A SET OF THREE STEPS WHEN HIS HEEL SLIPPED ONT HE 2ND STEPS, THEN HIS HEEL STOPPED SLIDING CAUSING HIM TO FALL FORWARD. HIS FOOT FOLDED UP UNDERNEATH HIM CAUSING A SPRAIN T O HIS LEFT FOOT. THE STEPS WERE WET AND SLICK FROM THE RAIN.

EE WAS WALKING ALONG BELT LINE CARRYING A 4'LONG TIMBER WHEN HE SLIPPED & THEN FELT A BURNING SENSATION IN HIS LEFT HIP.

CRANE OPERATOR SLIPPED AND FELL WHILE DESCENDING FROM OPERATOR'S CAB.

EMPLOYEE JUMPED TO THE MINE FLOOR FROM AN ELEVATED POSITION, UPON LANDING HIS ANKLE ROLLED UNDER. EMPLOYEE SUSTAINED A FRACTURE TO THE RIGHT ANKLE.
