broke 2 wrist and fractured back

Fall from excavator catwalk while trying to remove a tarp. Land on right shoulder resulting pain. Sent to the ER to get X-Ray done. No fracture was found.

EE was running the quarry saw when a wire broke. EE was pulling the wire and walking backwards when EE fell on some grout. This was human error, EE has done this job for months. EE fractured ankle.

EE was descending a ladder when EE slipped on the 3rd or 4th rung from the bottom; extension ladder shifted with EE's momentum and slipped off the square block it was resting against. Wet & possibly clogged boot tread contributed to the slip on the ladder rung.

Miner tripped over himself hurrying to attend wire saw. While extending arms to break his fall, he broke bones in right hand and wrist.

He stepped out of the company van and didn't realize the ground outside the vehicle was uneven so when his left foot hit the ground he rolled his ankle.

He was driving down into the quarry when he stopped to speak to another employee. He got out of his truck and walked 4-5 steps when he slipped on snow covered ice and fell on his back. The back of his head hit the ground. He was briefly dizzy and disoriented and at first had no feeling in his hands which quickly subsided.

SLIPPED ON ICE IN PLANT WHILE REACHING TO SHUT OFF WATER VALVE INJURING LEFT KNEE.

Climbing up a ladder to a height of between 4-5'. When on the last step before stepping off of the ladder it folded and the EE fell approx. 4' to the ground injuring his back and twisted his ankle.

Climbing ladder on 560 loader when 1 step broke. Leg went through ladder causing cut on right leg.

CLIMBING UP INTO THE 988 MACHINE AND SLIPPED ON THE RUNG AND SLID BACK DOWN STRIKING HIS CHEST AND RIGHT KNEE.

EE SLIPPED ON A LADDER THAT WAS LAYING DOWN CAUSING HIM TO FALL ON HIS HAND.

EMPLOYEE WAS CLIMBING ONTO A RAILROAD CAR WHEN HIS LOWER LEG SLIPPED OUT,STRETCHING HIS LEFT KNEE.

EE WAS UNHOOKING A WIRE FROM A TREE SLIPPED ON STONE FALLING 5' FEET LANDING ON SOME GROUT.

EMPLOYEE WAS WALKING ON THE DOCK. HE TRIPPED ON A STICK ON THE DOCK. ACCIDENT HAPPENED ON CROTCH ISLAND. BACK INJURY.

ACCIDENT WAS NOT DIRECTLY CAUSED BY THE JOB HOWEVER EMPLOYEE HAD A SEIZURE WHILE USING BURNER TO CHANNEL STONE AND FELL CUTTING HEAD. HIS DOCTOR SAID THE SEIZURE WAS PROBABLY A DIRECT RESULT O DIRECT RESULT OF AN AUTOMOBILE ACCIDENT WHICH HE HAD HAD.
