Chelmsford Quarry Metal/Non-Metal

Was chaining a block when a piece fell off of the block. Crushed right thumb and lost fingernail.

Employee was cutting metal with a grinder and disc shattered and penetrated right arm.

EE was pounding on 18" wedges to split a stone. EE missed and the hammer bounced back and split head open. EE cut forehead.

EE was running the quarry saw when a wire broke. EE was pulling the wire and walking backwards when EE fell on some grout. This was human error, EE has done this job for months. EE fractured ankle.

Was pulling on a rail when ee heard a pop, cannot lift shoulder.

Jumped from one ground level to another three feet lower, rock on higher level broke loose and fell after employee jumped landing on lower leg.

Miner got dirt particle in eye while cleaning heating unit

While splitting stone, seam opened up landing stone on miners right leg

miner encountered pain in back and hip while drilling

forklift operate dropped stone lacerating left ankle

Miner's left eye became red and blurry. Diagnosed with conjunctivitis.

Miner cut his lower leg on sharp stone.

miner looked up while climbing into forklift and strained his neck

Air hose disengaged from drill unit cutting miner on thigh and pinning him under control panel.

Air line hose connection released from jackhammer striking miner on bridge of nose and teeth 3 times.

Steel wedge was shattered while being struck by hammer cutting miner in neck.

miner was striking steel wedge with sledge hammer. wedge broke off and lodged in miner' s right calf

Miner tripped over himself hurrying to attend wire saw. While extending arms to break his fall, he broke bones in right hand and wrist.

He was pushing curb down the line on rollers all day. When he got home, he showered and then began feeling pain in his lower back.

He was at the middle level of the quarry pulling chain up from the tension car at the bottom of the quarry (chain was approx. 100'L) when he felt a pull in his lower back.

He had his left foot on a post about a foot off the ground and was pulling in diamond wire off the wire saw to check the crimps when he felt a tug in his groin area on the left.

MINER STRUCK A STEEL WEDGE WITH A SLEDGEHAMMER SHATTERING THE WEDGE SENDING STEEL FRAGMENTS INTO MINER'S NECK.

He was in a work box using a jackhammer in the quarry. The box began to shift outwards from the wall so he let go of the jackhammer and grabbed the railing. The box shifted back towards the wall pinching his right middle & index fingers between the railing and the wall.

Setting up a stone to saw. Pushed it in & hit clamp instead of stops. Tried to free it causing the clamp to knock stone forward & his fingers on left hand were caught between the stone & rollers.

He stepped out of the company van and didn't realize the ground outside the vehicle was uneven so when his left foot hit the ground he rolled his ankle.

He was driving down into the quarry when he stopped to speak to another employee. He got out of his truck and walked 4-5 steps when he slipped on snow covered ice and fell on his back. The back of his head hit the ground. He was briefly dizzy and disoriented and at first had no feeling in his hands which quickly subsided.

He was using a jackhammer all day during 9 hr. shift. Next morn. he woke to pain in lower back. Pain increased over the week so he reported it. Doctor diagnosed lower back strain May 5 & put on modified duty. Currently still on modified duty until May 28-no lifting over 40 lb.

Employee was trying to bend a piece of round steel when it rolled off of the work table and landed on his left foot. Employee was treated by a doctor who diagnosed a contusion. He was placed on modified duty until seen by an orthopedic doctor. The orthopedic doctor placed him on modified duty "until patient feels able"

PIECE OF STONE FELL CATCHING MINER'S HAND BENEATH CUTTING RIGHT HAND AND MIDDLE AND RING FINGER.

MINER WAS DRILLING AND SPLITTING STONE. STONE WAS NOT SECURED AND FELL ON HIS RIGHT FOOT, PINCHING IT.

SLIPPED ON ICE IN PLANT WHILE REACHING TO SHUT OFF WATER VALVE INJURING LEFT KNEE.

EMPLOYEE EXPERIENCED A POPPING SENSATION IN GROIN WHILE PULLING WEEDS

MINER FELT FOREIGN DEBRIS IN HIS LEFT EYE

Miner ended shift with discomfort in left eye.

While lifting a load of stone with overhead crane. Four pieces of stone held up by chains. Left arm reached in under load to remove wooden block from load. This caused release of stones onto arm. This caused bruising to the left arm.

STONE DEBRIS WAS STRUCK IN REMOVAL TRUCK FALLING AND HITTING MINER IN SHOULDER AND LANDED ON HIS LEFT FOOT.

While operating a loader, hit a bump in the road, felt severe pain in lower middle of back.

Pulling pin out of stone and measuring a slab, he had both feet under the stone, and stone shifted, pinning both feet.

While drilling, felt pain in stomach and groin areas. Lost time as of 2/9/06.

WHILE SWINGING A SLEDGE HAMMER, HIS LEFT HAND SCRAPED AGAINST A STONE.

WHILE LIFTING JACKHAMMER, FELT ARM TIGHTEN UP & FELT A RIPPING SENSATION IN RIGHT SHOULDER.

EE HAS HAD OTHER INCIDENTS OF BACK PROBLEMS. IT IS UNKNOWN WHEN OR WHAT AGGREAVATED HIS BACK CONDITION THIS TIME.

WHILE LIFTING A PIECE OF 4 X 6 WOOD FELT PAIN IN LOWER BACK.

JUST PULLED STONE OUT, FELL WHERE STONE CAME FROM AT BOTTOM OF LADDER.

WHILE TRANSFERRING DIRT FROM BUCKET TO BOX HE FELT PAIN IN GROIN AREA. STARTED MISSING WORK ON 1/16/01.

EE WAS REMOVING A PIECE OF GROUT WHICH WAS NOT THAT BIG OR HEAVY FROM THE ROLLERS, BUT FELT LOWER BACK PAIN AS A RESULT.

EMPLOYEE TRIPPED OVER A ROCK OUTSIDE OF BREAK SHACK.

EMPLOYEE WAS UNFASTENING AIR HOSE.

EMPLOYEE WAS DRAGGING NEW CHAIN UP A LADDER AND THROWING IT OVER AND UNDER A STONE AND FELT PAININ LOWER BACK.

AFTER HOOKING UP A PUMP, EMPLOYEE JUMPED OVER STEEL RAILING LANDING ON THE EDGE OF A STEEL WALKWAY.

HE WAS AT STRINGER, WENT TO BEND THE WIRE TO WELD; WIRE SNAPPED, CAUSING LACERATION TO LEFT FOREARM.

PUTTING ON HIS HELMET AGAIN, DUST & DIRT FELL FROM HIS HELMET INTO HIS RIGHT EYE.

HE WAS TALKING WITH DRIVER ABOUT A CUSTOMER'S STONE. HE JUMPED DOWN OFF THE FORK TRUCK AND DRIVER STARTED TO PULL AWAY. THE BACK RIGHT WHEEL (THE STEERING WHEEL) OF THE FORK TRUCK STRUCK HIM , KNOCKING HIM TO THE GROUND & RUNNING OVER HIS RIGHT ANKLE. HE WAS WEARING STEEL TOED BOOTS.

WHILE ON RAMP GOING INTO QUARRY CLIMBING BANK WITH BROKEN SNAD WIRE, A STONE ROLLED ONTO HIS FOOT.

EE GOT DUST IN HIS EYE AND IRRITATED IT BY RUBBING IT.

WHILE PULLING UP A BICKET OF DULL JACKHAMMER BITS ON A ROPE, HE FELT PAIN IN HIS BACK.

EMPLOYEE HAD DUST GO INTO LEFT EYE FROM WIND. EMPLOYEE HAD SAFETY GLASSES ON.

GRANITE DUST BLEW INTO LEFT EYE. (EMPLOYEE HAD SAFETY GLASSES ON).

EE WAS TRYING TO LINE-UP BOLTS ON THE BASE PLATE FOR THE PUMP, WHEN HIS FINGER GOT PINCHED BETWEEN THE BOTTOM OF THE PUMP AND THE BOLT.

EE WAS ROLLING A DRIVE SHEAVE TO PUT A BELT ON, HE PULLED A MUSCLE IN LEFT ARM AND SHOULDE.R

EMPLOYEE STATES HIS HANDS STARTED TO FALL ASLEEPAND HIS ARMS ACHED ONE MONTH AGO.

EE STATES HE FELT PAIN IN HIS SHOULDER WHEN HE WAS DRIVING THE LOADER. HE TOUGHT IT WOULD GO AWAY. WHEN HE GOT UP THE NEXT MORNING IT WAS PAINFUL ENOUGH TO GO TO THE HOPSITAL.

AFTER DRILLING A PIN HOLE, HE PULLED THE JACKHAMMER OUT OF THE HOLE ADN SLIPPED ON SOME MUD DROPPING THE JACKHAMMER ON HIS RIGHT FOOT.
