During his normal shift employee was inspecting the head pulley for the surge plant stacker after completing his inspection employee was walking down the cat walk when he tripped and fell injuring his left shoulder. Employee immediately reported the injury to his supervisor. Supervisor drove employee to urgent care center.

Mostrando los 36Employee had completed his lunch and was returning to the water truck when he attempted to step over one of the indents designated as equipment "ready line" at which point he lost his footing, slipped and fell spraining his right knee.
The miner was performing the routine task of cleaning around a screen support structure when he inadvertently stepped off of the concrete foundation to the surrounding grade; twisting his knee.
Employee was washing out the main dozer trap feeder with a water hose. When he exited the feeder he slipped on the wet rock and cut his right hand on a sharp rock. He was not wearing gloves at the time of the accident. Went to doctor and received three stitches.
Emp. was checking PC-6 head section drive belts, 14" from ground. He climbed onto belt, got on his knees, leaned out over the box, he grabbed a piece of angle iron that was not connected and slipped away. He lost his balance and fell into head box (5 1/2" deep). He reached up with his left hand & grabbed side of head box to break his fall, dislocating his left shoulder.
On pit pump in main pit to remove float and pump from service. Needed to disconnect from discharge hose and the float flipped in the water. Employee fell into the water and hit his knee as he was going in and cut his leg requiring stitches.
Inappropriate personal fall protection, he tied off in a unsafe area, caused employee to fall while taking down a surge bunker off the side of a wet tower. Injuries were a small laceration to the back of his head and a broken right forearm.
EE was pulling a fuel hose across the floor of the pit. The ground was uneven and has he stepped down he twisted his ankle resulting in a sprain.
Emp was starting load out system, slipped on concrete pad falling into 3 feet of soft material, he managed to free himself. He went to mmedical clinic and received a return to work on 9/5/09. He received pains and anti-inflamatory medications. Upon follow up 9/8/09 he was given physical therapy for 2 weeks 3 times a week. He returned to full time work with no loss time.
Employee was installing a wear plate on a conveyor that feeds a twin shaft coarse material washer. He was standing on a partition between the two tubs. He removed his gloves to retrieve hardware. At this point, he lost his balance and fell aproximately 3'. He braced his hand during and cut his left hand in between the index and middle finger that required 10 sutures.
Injury was working from a ladder next to a conv. belt when finish he step down off the ladder and strain his left ankle.
As the employee was going up a step he slipped causing him to twist his lower right leg. Diagnosis: Knee/Leg Sprain.
Employee was at the top of the stairwell, was attempting to come down when he missed the first step, falling down 7 steps.
As the employee exited the water truck he slipped on a step causing him to fall on his back.
Employee was shoveling in muddy area (below conveyor #3), slipped and fell back on his right side. sprained/strained cervical.
Employee finished operating bobcat loader, dismounted machine & stepped on a rock or uneven surface-twisted right knee causing sprain.
West Coast Sand and Gravel truck driver slipped & fell at self loading bunker #6, he was stepping over a space of 32" wide & 34" tall; hitting his head, side of chest, right hand & elbow abrasion. Weather was drizzling & damp. After the accident a catwalk & handrail was installed.
EE came into the office on 4/24/07 to fill out a incident report for a sore knee. He indicated that while coming down the staircase at the #1 Crusher he slipped on the stairs about 4 to 5 steps from the top and was able to catch himself with the handrails. He did not fall but said he twisted his left knee and did not need to go to the clinic.
Employee was moving the screen down by the crush plant with the forklift. He got out of the forklift to check on the forks when he tripped over the screen and fell and cut open his left knee.
Employee was standing with one foot on a conveyor frame and the other foot on a cross brace of the conveyor tunnel wall. While he was striking the side of a material hopper with a hammer to clear some blockage, his foot slipped and he struck his back against the wall of the tunnel. Lower back contusion.
Employee was walking down a slopped area, when he tripped and fell. He reached out with his right hand to break his fall, and felt a pain in his right hand. (Metacarpal fracture of right hand)

Employee was walking down a catwalk,tripped and fell injuring his right wrist. *Employee did not report this injury until 2/13/06 which is when it became reportable.*
EMPLOYEE WALKING ON DECK REBAR APPROXIMATELY 12" HIGH LOST HIS FOOTING CAUSING HIM TO SPRAIN/STRAIN HIS RIGHT ANKLE. EMPLOYEE DID NOT INFORM EMPLOYER UNTIL 11/4/05.
OUR EMPLOYEE WAS DRIVING HIS LOADER TRUCK TO OUR MECHANIC SHOP ON OUR FIELD AND WHEN HE STEPPED OUT OF HIS TRUCK HE STEPPED ONTO UNEVEN GROUND AND AND HIS FOOT TWISTED, HE THEN REACHED HIS LEFT ARM OUT TO CATCH HIMSELF AND HURT HIS LEFT SHOULDER IN THE PROCESS.
EE AND OTHER EE WERE SHOVELING IN THE #3 ROCK TUNNEL WHEN EE SLIPPED AND FELL HITTING HEAD AGAINST THE CONVEYOR STRUCTURE. EE HAD A CUT ON NOSE AND NOSE WAS BLOODY. EE WAS DISORIENTED, AND 911 WAS CALLED.
EE MISSED THE SECOND STEP COMING DOWN HIS LOADER AND FELL OFF TO THE GROUND, ONTO HIS BACK.
While stepping from machine to a ladder, pant leg got caught on the battery box. This caused him to fall, landing on his left side.
HE WAS CLIMBING UP THE LADDER AND HIS FOOT SLIPPED OFF THE STEP AND HE LOST HIS GRIP AND FELL TO THE GROUND APPROX. 7' AND LANDED ON HIS RIGHT FOOT.
CLIMBING DOWN OFF THE 992 LOADER BEHIND MINE OFFICE TO USE RESTROOM, EMPLOYEE STEPPED ON A ROCK AND TWISTED HIS ANKLE.
EE WAS WALKING WITH A PIECE OF PLYWOOD IN WHICH COVERS THE PIT. HE DID NOT SEE THE RIGHT HALF OF THE PIT WHEN HE SLIPPED AND FELL IN AND STRUCK HIS LEFT SHIN AGAINST A METAL EDGE OPENING.
SLIPPED OFF STEPS ON WATER TRUCK WHILE CLIMBING DOWN.
OPERATOR WAS IN A HURRY TO GET OFF OF THE LOADER AND LOST HIS BALANCE. HE THEN TRIPPED OVER A ROCK LANDING ON HIS HIP AND RT. LEG. HIS RT. WRIST HIT THE GROUND AND RT. ARM HIT LOADER TIRE. HE SUSTAINED A SPRAINED WRIST & SHOULDER.
EMPLOYEE WAS HELPING TO CARRY A 7'X16"X1/4" METAL PLATE INTO THE TUNNEL TO REPAIR SIDE SKIRTS ON FEEDER BOX. EMPLOYEE SLIPPED ON LOOSE GRAVEL AND CAUGHT RIGHT MIDDLE FINGER BETWEEN THE METAL P LATE AND GRAVEL. FINGER WAS LACERATED, REQUIRING SUTURES. EMPLOYEE WEARING GLOVES AT THE TIME OF THE INCIDENT.
ENEVEN FLOOR, SPRAIN TO ANKLE (LEFT).
OPERATOR WAS CLEANING GRIZZLEY BARS IN FEEDER. OPERATOR FELL APPROX. 15' HIT HIS HEAD ON ROCKS, 47 STICHES AND FRACTURED R. WRIST. 1) OPERATOR RAILED TO LOCK OUT AND TAG OUT.2) OPERATOR DID NO T WEAR HIS SAFETY HARNESS.3) OPERATOR USED UN-SAFE ACCESS.4)OPERATOR FAILED TO NOTIFY HIS CO-WORKER OF HIS ACTIONS. 5)OPERATOR FAILED TO BLOCK THE FEEDER.
