Injured states he was carrying a bag of B-Bond and a jug of water, he slipped in the mud and strained his groin area. The injury was diagnosed as a hernia on 10-10-14 that will require surgery to repair. The employee is currently working his regular job. The accident will become reportable as a NFDL (LTA) once he has the surgery.

IE stated that while walking to catch up with a co-worker, who was carrying a hammer, he stepped into an irregularity in the bottom causing him to stumble forward hitting into the hammer being carried by his co-worker. As he fell, the hammer struck his lip causing a laceration requiring 3 sutures.

IE was bending a roof bolt when he twisted his right knee. IE has worked full duty up until surgery on 07/31/14.

Employee was walking along the RHS when his foot slipped, stuck in the mud and he felt pain in his right knee. He continued to work his regular job until and MRI revealed a torn miniscus.

Employee was lying on the catwalk reaching out to paint steel angles when he lost his balance and slipped falling through the belt structure landing on uneven ground striking a rock and fracturing his pelvis.

IE was tramming miner, he was walking in a right to left direction. The miner was stopped and IE stepped into some uneven bottom and felt a popping sensation in his lower back.

Employee states reeling oil hose at the oil trailer stepped into a hole catching his toe on a rock causing him to fall, spraining his left hand. No medical attention until 6/27, X-ray of the hand revealed a fracture to the metacarpal bones in his hand.

Employee went to check the 4-inch water valve at the fresh water pond. As he turned to walk away, he tripped over the 4-inch waterline and fell onto his left wrist causing a hairline fracture that required a cast.

Employee was getting off of Red Johnson and slipped and twisted left knee on wet ground. Employee is a Shift Foreman. ** AMFIRE IS FILING A PROTEST ON HAVING TO COMPLETE THIS REPORT!

Employee had just completed setting Rail Jack for Tail. It was just about tight when employee got up, he placed his left hand on Tail when the Jack left go falling, striking his left hand between the Jack and Tail Piece.

Employee starting to bolt an open 20 foot cut in which roof rock had been dislodged and fallen out. After setting the ATRS and while swinging out his safety post canopy, the roof began chipping and the injured was struck by a piece of falling roof rock estimated to be 3.5' long x 2' wide x 10" thick. The rock caused him to fall striking his right shoulder and ribs on the bottom.

Injured was coming down off an overcast when his feet slipped out from under him causing him to twist his right knee.

While walking in surface screen area, employee slipped on the ice, landing on his right ankle. The bones above the ankle were fractured from the FSL.

Employee walked and crawled to back of roof bolter, when he turned to go around the bolter he felt pain in right knee. Employee has worked full duty since 11/30/2010. At 01/18/11 doctor's appointment he was instructed to work modified duty with surgery to be on 01/24/11.

Employee claims that as he was getting out of his scoop tractor, he slipped and felt pain in his lower back on the right side. Employee continued to work until April 21.

Employee was pulling miner and mobile cable when he rolled and twisted his left ankle. The conditions were wet and sloppy.

Employee states that as he was walking from the parking lot to the portal building prior to the start of his regular work shift, he stepped in a hole at the top of the landing, causing him to twist his ankle.

Employee slipped off the last step while dismounting the front end loader. He initially told the foreman that he was OK and worked without incident for the rest of the week. On the morning of 3/8/09 he awoke to find his right leg swollen and black and blue. The ER @ Dubois Hospital diagnosed an infection under the unbroken skin on his leg. His first day off work was 03/09/2009.

Employee states that he was dismounting from his Cat D10R Dozer when his feet went out from under him and he fell to the ground onto his right shoulder. He believes that he may have slipped on snow and ice on the tracks/structure.

Employee was climbing down from a rock truck, when he missed the handrail and lost his balance, causing additional strain on his hand and arm still holding onto the handrail.

Employee stated that as he was operating the continuous miner, he turned and felt his knee pop and that he continued to experience pain in his knee as the shift went on.

Employee was setting ATRS, when he pushed "on" lever to set ATRS, his right hand slipped off control, he lost balance and fell hitting his right hand on tire or bolter or bottom.

Employee was removing a rub-rail from the side of the tail bridge when he lost his grip on the rail. His left hand was caught between the rail and the RHS causing a laceration and fracture to his finger.

Injured and co-workers were attempting to dislodge filter cake stuck to the inside and door openings of the stacker tube, when some loose material fell from the stacker. Injured was struck by the material, fell and rolled down the side of the 5 foot high coal pile that he was standing on. Injured had the air knocked out and felt pain in his lower back and right side.

Walking toward telephone in Surface Shop tripped over ground clamp cable and struck right knee on concrete floor.

Employee was participating in a practice alternate escapeway drill while crossing over a belt cross-over he lost his balance and fell off the cross-over striking his right side. Accident was not reported until 12/4/06 at that time he was sent to the doctor.

Employee lost his balance while crawling and fell forward hyperextending the little finger on his right hand causing a simple fracture. Employee did not go to doctor until 9/20/06

Employee was attempting to break off a roof bolt which had bent during installation. He had used a hacksaw to partially saw through the bolt and was rocking the bolt back and forth to break it off. He gave one extremely hard push, the bolt broke off, he lost his balance falling onto the roof bolt plate causing a laceration on his right forearm.

Employee was walking along the RFM in the beltline to check the haulage dolly for blockage when he either slipped and fell onto the moving belt or stepped onto the moving belt (the employee is unsure). He proceeded down the belt and became jammed between the dolly and the belt

Employee was greasing the screen belt drive when he caught his coveralls on the scrapper bar causing him to twist his lower back. He slipped and fell against the railing.

Employee, prior to the start of her shift, slipped while walking across the parking lot causing her to twist and fracture her right ankle.

Employee was climbing ladder to get into front-end loader when he slipped on a rung hyperextending his left knee. It was raining when the accident occurred.

Employee, while observing grates over surface feeder for blockage, slipped and fell causing a mild fracture to his left wrist.

Employee was carrying a bottom belt idler when he tripped on a cable lying on the bottom causing him to fall. The weight of the bottom roller struck the top knuckle of his middle finger on his left hand causing a fracture.

Employee was trying to get a pallet out of garbage tub while standing in tub. He was jerking on pallet when it became dislodged causing employee to fall back against dumpster striking his back.

Employee was moving shuttle car snub from #2 Entry to #4 Entry. He stepped into a hole on runway and sprained left knee.

Walking into shower room

EE was crawling on his hands and knees when his right elbow gave out and he fell onto his chest and head. Immediately employee had sharp pains in his right elbow.

While employee was decending down steps, while carrying water hose, hose caught at corner of railing pulling employee back enough to lose footing on stair treads. He fell onto right elbow on metal stair tread.

While employee was working on the belt he crossed over the belt, got caught in the cables and fell off the belt. The belt was not Running.

While EE was putting a tie-rod end on a 21SC shuttle car his wrench slipped and he fell striking his right side on the wheel unit hub.

While loading supplies on the roof bolter, employee twisted to grab another handful of bolts when he felt his knee "pop". He fell down grabbing his knee in pain; when knee was checked there was no swelling or visable signs of injury.

Employee was checking wheel unit bolts on a shuttle car when he bumped and twisted his left arm.

While climbing up loader ladder, employee's feet slipped out from under him. He fell down 2 or 3 steps, his right arm caught on railing and the weight of his body pulled his right shoulder out.

Injured and co-workers were replacing decking material on the main belt catwalk on the previous operating shift. The workmen left this job site prior to replacing a 2' x 8' section of flooring that had been removed. The area was not gaurded to prevent entry into the area. Injured was walking down the catwalk when he fell through the hole falling approx. 9' to the ground below.

EE was getting back into the High Lift - there are 2 hand grabs to help you get into the High Lift - one is easily accessible - the other is close to the door-according to EE as he was reaching to get the handle that was close to the door, his foot slipped and he missed grabbing the handle, he fell backwards on the ground. *Employee quit 8/5/2004.*

Employee stepped in a low place caused by uneven mine floor and twisted his right knee.

While working on flooring in Plant Employee slipped and hyperextended his right knee.

STEPPING AROUND THE FUEL TANK TO SHUT FUEL OFF, HE STEPPED ON THE FUEL HOSE AND HOSE ROLLED RIGHT FOOT UNDER IT.
