Contractor was walking towards Rainbow pond when the contractor slipped and fell then twisted ankle. On Monday 4/7/25 the contractor called to inform us that the ankle was broken.
While going down the steps in Robo 3 the operator's boot caught the grating causing operator to miss the last 2 steps. Employee fell to the ground and feels pain on both inside and outside of knees. Employee referred to a specialist on 2/17
Employee tripped and fell against a forklift. Employee suffered a dislocation and fracture of a finger on right hand.
Operator was getting off a railcar after setting the hand brake and slipped off of the knuckle of the railcar, hitting is thigh area on the way down. Operator requested to see doctor on 11/25 and provided restrictions on 12/2.
Contractor fell from elevation. First aid was performed on site. Received additional information from contractors on 6/22/2024 indicating that contractor had a fracture in their back.
Tech lost EE's balance and fell on back. Hairline fracture in bone.

The employee was entering the plant heading to the plant break room. When the employee stepped on the top stair to descend to the break room, EE slipped causing EE to fall awkwardly down the stairs. Employee is experiencing shoulder pain post fall.
The employee was working on a light vehicle and was standing on the front bumper/cattle guard. When they went to dismount the vehicle, they slipped causing them the strike their ribs/upper torso against the cattle guard.
Employee was heading down the stairs in robo 2. The employee overstepped the last stair causing EE's foot to contact the ground with 14" step down. The employee felt a pop in EE's right knee
An operator was placing dunnage in a rail car. The operator exited the railcar and went down the pellet ramp to retrieve more cardboard. Operator was maintaining 3-point contact until EE needed both hands to grab the dunnage. At this time, EE's foot slipped and EE fell.
Two sub-contractors of Purvis Industries were installing a flange on an overhead pipe (8' high) with "come along" hoist. The workers were elevated several feet (not over 6') to physically fit/bolt the flange into the pipe. One subcontractor was on the extension ladder and lost their footing and fell to the ground. EE's right shoulder injury was the result of the fall.
Operator was descending from a railcar after setting handbrake, but did not step properly causing them to roll left ankle. NOTE: Employee failed to return to work with modified work available, stopped communicating with company - job abandonment case and termination- "Return to Duty" info has been adjusted so no "lost" days associated with injury reported.
Employee was descending stairs of loader, missed a step, and hit knee off the ladder. Resulted in a contusion/abrasion of the left knee.
