Audiometric testing was done on 8/06/2025 showing hearing loss.

Contractor was walking towards Rainbow pond when the contractor slipped and fell then twisted ankle. On Monday 4/7/25 the contractor called to inform us that the ankle was broken.

Employee bent to check oil level at the crusher on 3/3/25 when employee started to experience discomfort in lower back. The employee reported the incident, but thought the discomfort would go away. The employee went to the local clinic on Friday 3/21/25. Restrictions; no more than 20lbs Occasionally, no climbing, no twisting, in effect until 4/7/25.

Audiometric testing preformed on 08/22/2023 showed that the employee had hearing loss. Audiometric retesting on 9/19 confirmed employee had hearing loss.

A Shipping Center operator was packaging material in paper bags when a 50 lb. paper bag was hung up on the bag chair still connected to the fill spout. The bag began overfilling and the operator attempted to move the bag at which point the bag expanded beyond capacity and erupted. The operator was wearing safety glasses; however, material entered the operator's right eye.

Operator was descending from a railcar after setting handbrake, but did not step properly causing them to roll left ankle. NOTE: Employee failed to return to work with modified work available, stopped communicating with company - job abandonment case and termination- "Return to Duty" info has been adjusted so no "lost" days associated with injury reported.

Employee was descending stairs of loader, missed a step, and hit knee off the ladder. Resulted in a contusion/abrasion of the left knee.

Employee was hanging a bulk bag and missed the hook, causing right hand to make contact with the steel bracket. Currently a strain/sprain requiring immobilization.

Operator was trying to align a chute and sliding collar in place, pushed too forcefully, and felt a pain in right arm/elbow. Iced and cared for. Did not fully improve over the course of a month. Results now show partially torn biceps muscle.

Employee slipped on a step while performing maintenance on a palletizer. Footing was not secure, access step is narrow, rushing/frustration, some talc on the surface from bag that broke. Sprained knee with possibly more damage - waiting on MRIs - out of work.

Moving a steel beam - beam rolled into ankle

Employee was turning the pellet mill with a crowbar when it slipped and employee stumbled, twisting right ankle and putting excessive weight on it. Sprained ankle.

Employee was repairing a broken wooden pallet with a hammer and nails. Employee missed the nail and struck their left thumb with the hammer, resulting in a cut, bruise, swelling, and hairline or "turf" fracture.

Operator was performing general housekeeping (sweeping) when EE felt a pull/bee sting burning sensation in EE's left shoulder. Mobility of arm was impacted.

Individual had a piece of metal fall and hit hardhat. It then cut the back of EE's neck. Six stitches. No lost time.

Moving a Rol-a-Lift to trailer to be picked up, the piston was unsecured, lifted up and pinched the employee's right index finger. Deep laceration, finger tip fractured, and part of finger tip partially amputated, but re-attached and expected to heal.

Operator was performing pre-use inspection on Telehandler. EE was exiting the cab while holding the steering wheel and door handle bar, lost EE's footing, and caught self with EE's right arm that was still holding on to the steering wheel. When EE did, EE put all EE's weight on EE's right arm/shoulder.

Finishing work at the cone crusher, operator lifted his leg up onto the platform, pushed himself upwards, and felt his knee twist. Operator iced the knee until he was able to walk on his own. Employee returned to work the next day. Employee made an appointment with his regular physician. On 2/26/15, the employee was told that he had torn a muscle and would need surgery to fix it.

Employee reports that he strained his low back on 3/5/13 while using a sledge hammer to break ice/ore. The employee sought medical treatment and was diagnosed with nerve impingement on 4/5/13. The employee received a series of medical visits which included treatment beyond first aid.

Operator was descending from the bottom rung of an 8'step ladder. As he was stepping from the rung, he turned to begin walking in the intended direction of travel. Upon doing this, he lost balance and stumbled backward. While attempting to catch his balance, his left forearm contacted an angle iron bracket, resulting in a 5 cm by 1 cm laceration.

Employee reached onto conveyor to straighten misaligned bag near entry into (double conveyor) incline flattener. Arm was drawn in to shoulder. Co-worker shut down system and helped release employee.

Reaching into steel pipe under railcar to obtain sample, employee cut right index finger on sharp edge where 2 pipes come together at a Y. Received 5 stitches.

CONTRACTOR injured left pinky finger when it was caught between cable and pulley of crane block. Used his hand to steady or guide the block. Given proper first aid at site, he elected treatment after shift which included a stitch to prevent loss of fingernail. Contractor is VERY INFREQUENTLY used at site. His experience in this submission is estimated as unknown at this time.

Employee packaging talc at four spout paper bag packer. Bag kicked off incorrectly, spout re-triggered resulting in stream of talc released outside of the bag. Employee entered talc stream to hit e-stop by spout . Got talc in his eye - flushed on site; further irritation so he was transported to local clinic where eye was flushed - given preventative antibiotic ointment.

Employee loading truck with forklift in shipping area when piece of dust or debris got in his eye. Irritation got worse and eventually it was decided he needed the attention of a doctor when flushing did not work. Debris was gone at hospital; employee given prescription to prevent infection.

Employee slipped and fell while decending haul truck access ladder. Striking left elbow on step. On 4/4/05, employee was seen by a physician and returned to work with restrictions - no use of left arm. On 4/8/05, employee was seen by an orthropedic surgeon, diagnosed with left elbow fracture, returned to work with restrictions - no climbing ladders.

Employee was carrying a 4' long bar used to free skirting frozen to conveyor belts. He slipped on icy surface and fell to the ground injuring his right wrist and second finger knuckle.

The employee was emptying trash to a large dumpster outside the plant. The ee slipped on wet ice that had accumulated in front of the dumpster & fell. The fall resulted in one cracked rib. The ee continued to work the current subsequent shifts.

EE SUFFERED A MID-BACK STRAIN AND HAS BEEN RELEASED BACK TO FULL DUTY WITH NO PERMANENT IMPAIRMENT. THE INJURY WAS INCURRED WHILE LIFTING A PORTABLE POWER SUPPLY UNIT.

THE EMPLOYEE WAS CHANGING OUT THE FUEL FILTERS ON THE CAT 365 EXCAVATOR. SHORTLY AFTERWARD HIS LOWER BACK AREA BECAME SORE. AWKWARD POSITIONING IS THOUGHT TO BE THE REASON FOR THE BACK PAIN SI NCE ACCESS TO THE FILTERS IS DIFFICULT. AN EXAM AND CHIROPRACTIC CARE HAS BEEN REQUIRED.

2 EE'S WERE PLACING 6'LONG I-BEAMS ON TO THE DECK. AS THEY PLACED ONE BEAM DOWN, ONE EE HAD HIS HAND ON THE END OF THE BEAM. AS THEY PUSHED THE PIECE INTO POSITION THE INJURED EE CUT HIS RT. SMALL FINGER. FIVE SUTURES WERE REQUIRED.

A NEW ELCTRONIC CONTROL SYSTEM WAS BEING INSTALLED THROUGHOUT THE SHIPPING CENTER. THE FUSES IN ONE STARTER BLEW OUT AND NEEDED TO BE REPLACED. IN SO DING THE EE CHANGED FUSES CORRECTLY. MOMEN TS LATER THE FUSES WITHIN THE CABINET BLEW AGAIN. EE'S HAND RECEIEVED 2ND DEGREE BURNS.
