Employee was repairing a light fixture on the top level of Plant 1 Tyler System. While repairing the fixture, employee fell down a flight of stairs. The fall dislocated his left shoulder, ring and pinky finger of his left hand.
Employee tripped and fell over a pallet. While falling the employee extended his left arm out and fell on his his left hand and elbow. The fall caused a bruise along the medial side of his left elbow. Prognosis is good. The employee was returned to work without any restrictions.
Employee was walking down a stairway and missed the third to last step causing him to lose his balance, fall forward and strike the concrete floor with his chest. Diagnosis - Chest Contusion.
Walking and stepped up when knee pain began. During his Christmas vacation, EE sought medical treatment with his private physician. Notified us of this action on 12/30/05.--220060680011--orthoscopic surgery on 3/8/06 to access damage to ligaments and cartilage.
Employee directed a truck in the work area to move forward. When he turned to walk away he slipped on some material on the ground and fell, hurting his left knee and right wrist.
The management employee slipped on wet pavement while traveling to the office and twisted his knee. WASS RETRIEVING SAMPLE BUCKET FROM WORK PLATFORM WHEN THE PLATFORM BROKE CAUSING THE EMPLOYEE TO FALL. EMPLOYEE SPRAINED BOTH ANKLES.
EE WAS WALKING OFF A STOCKPILE WHEN HE LOST HIS BALANCE CAUSING HIM TO FALL LANDING ON HIS RT SHOULDER. THIS RESULTED IN 2 STIGHTLY FRACTURED RIBS. *EE WALKED ONTO A STOCKPILE TO RETRIEVE A SI GN. WHEN HE CAME TO THE BOTTOM HE FELL TO THE GROUND.
EMPLOYEE WAS ATTEMPTING TO CLIMB ON A DOZER WHEN HIS FOOT SLIPPED, HE FELL AND LANDED ON HIS WRIST AND FOREARM.
EMPLOYEE ALLEDGES THAT HE SLIPPED, HYPER-EXTENDING HIS GROIN AREA WHILE WALKING OFF A PLATFORM OF A SUPER SACK MACHINE. EMPLOYEE WAS OBTAINING A SAMPLE OF PRODUCT. EMPLOYEE WAS DIAGNOISED WI TH A HERNIA ON 3-28-00.
