Mostrando 50 de 100Two maintenance employees were engaged in horseplay involving a pocket knife. The injured employee attempted to grab the hand of the other employee but instead grabbed the blade of the knife causing a laceration the palm of EE's right hand.
Driver was rolling tarp. EE released the handle, it spun back hitting ee on the cheek.
Employee was assisting with the installation of a tail roller which required lifting and maneuvering the part in a tight small area. At the end of the shift the employee was experiencing some minor discomfort in lower back. As the evening progressed the pain increased. Employee was taken to the doctor and diagnosed with a minor lower back strain.
Employee was in the process of getting down off of the drill rig and slipped and fell hitting leg on the mud pan.
Employee was adjusting the kick plate on a bagging station when they placed left hand between the raised kick plate and the support structure underneath. Driver was removing a lump of material from tailgate of dump truck. Driver instructed a 2nd driver to close the latch. Driver#1 placed hand on the pin where the clasp locks the tailgate. Middle finger on right hand was pinched inside the clasp.

Employee was inspecting a v-belt and inadvertently got left little finger caught between the v-belt and pulley.

The employee was wearing a pair of safety glasses at the time of the incident. A piece of debris flew in-between the employee's safety glasses and forehead and lodged in his (R) eye.

EE was removing an air exhaust muffler from a compressed air dryer system.After releasing the residual air from the air tank and line the EE unscrewed the valve not realizing there was still some stored pressure inside the line which blew small desiccant beads out striking the back of both hands and the back of both legs.
EE was lifting a machine part using a mobile crane.EE placed left hand on the cable of crane to move the part away from I-beam,the glove he was wearing got caught in head pulley, pinching his finger between the pulley and the cable, causing a laceration and a break at the end joint of his pinky finger on the left hand.
Employee was repairing a light fixture on the top level of Plant 1 Tyler System. While repairing the fixture, employee fell down a flight of stairs. The fall dislocated his left shoulder, ring and pinky finger of his left hand.

Employee tripped and fell over a pallet. While falling the employee extended his left arm out and fell on his his left hand and elbow. The fall caused a bruise along the medial side of his left elbow. Prognosis is good. The employee was returned to work without any restrictions.
Employee was picking up a pallet that had fallen off a conveyor onto the ground. The employee reached down and grabbed the pallet with his right hand and began lifting it, when he felt a pain in his right shoulder.
Employee bent over and picked up a bag of material without bending his legs and felt a pain in his lower back. Diagnosis - acute lumbar strain. Prognosis - Good.
Employee was walking down a stairway and missed the third to last step causing him to lose his balance, fall forward and strike the concrete floor with his chest. Diagnosis - Chest Contusion.
