Employee was moving a continuous miner when he slipped straining his left knee. The employee continued working until he stopped to undergo knee surgery on July 12th. After the accident investigation was completed it was determined that the knee surgery was in fact due to the injury reported on June 10th.

Employee slipped on a metal walkway, resulting in a laceration on his lower leg, that required 5 stitches to close.
Closing doors of railroad cars with a prybar and slipped and fell on some ice.
Employee had two buckets of miner bits in his hands, when he stumbled while walking back to the portal. This resulted in the twisting of his back.
Employee stepped out of the operator's compartment of the scoop he was operating and slipped, twisting his knee. He began losing time on 06/11/08.
EE stated he was working near the section feeder, when a ramcar caught the suspended miner cable and bolter cable and tore them down. When they fell, the cables struck him in the back, knocking him down. On 3/26/08, he was diagnosed with a torn ligament of the right knee. He had surgery on 4/4/08.
Employee was walking in an area that was 5 breaks outby. The face when he slipped and twisted his ankle. Left. On 8/20/07 He went to the dr and was diagnoised with a hairline fracture of the left fibula approx. 3" above the ankle. Accident became reportable on 8/20/07.
Employee stated that he was pulling on a cable when he felt "something give" in his right knee. Mine floor was wet and muddy. He had been diagnosed with a torn ligament in the right knee.
Employee stated he was walking in the #3 Entry at 25 x-cut when he stepped on something in the mud. He stated he felt his knee twist. He continued working the rest of his shift.
EMPLOYEE WAS INSTALLING A PIECE OF DUCTWORK THROUGH A BLOCK WALL. HE WAS STANDING WITH ONE FOOT ON THE 4TH STEP OF A STEP LADDER & ON A PIECE OF HANDRAIL WHEN HIS FOOT SLIPPED OFF. HE FELL 4' STRAINING HIS RT. KNEE.
EE WAS TIGHTENING LUG NUT ON SCOOP TIRE WHEN THE SOCKET SLIPPED OFF THE LUG NUT. EE WAS PUTTING DOWN PRESSURE ON THE 3/4" RATCHET WHEN THE SOCKET SLIPPED. HE LOST HIS BALANCE AND FELL FACE-FIRST INTO THE LEFT REAR FENDER OF THE SCOOP.
EE TRIPPED ON A SMALL MOUND OF DIRT. AS HE TRIED TO REGAIN HIS BALANCE HIS RIGHT FOOT TWISTED, CAUSING HIM TO FALL IN A SITTING POSITION WITH THE MAJORITY OF HIS BODY WEIGHT LANDING ON HIS RIGHT ANKLE.
EE WAS WALKING ON HAULRAOD. HE STEPPED ON A FLAT ROCK AND HIS FOOT SLIPPED OFF THE EDGE OF THE ROCK. THIS CAUSED HIS LEFT FOOT TO TWIST SIDEWAYS RESULTING IN A FRACTURE OF HIS LEFT FIBULA APPR OXIMATELY ONE INCH ABOVE THE ANKLE.
HE WAS REMOVING A TILT JACK ON A D-11 DOZER. HE HAD ONE FOOT ON THE PUSH ARM AND PLACED THE OTHER FOOT ON THE TRACK. THE TRACK WAS MUDDY AND HISFOOT SLIPPED CAUSING HIM TO FALL ON THE TRACK.
EMPLOYEE WAS GETTING OUT OF THE BACK OF HIS PICK UP TRUCK, HE STEPPED DOWN FROM THE REAR BUMPER.HE PLACED HIS RIGHT FOOT ON A ROCK. THE ROCK ROLLED, CAUSING THE EMPLOYEE TO SPRAIN HIS RIGHT ANKLE.
EMPLOYEE WALKING THRU THE WALKWAY OF A FLETCHER WALK THRU ROOF DRILL. HE TRIPPED ON AN UNKNOWN OBJECT AND CAUGHT HIS FALL ON HIS OUTSTRETCHED LEFT HAND. THE IMPACT ON HIS LEFT HAND CAUSED A FR ACTURE OF THE LEFT RADIAL BONE AT THE WRIST.
HE WAS WALKING BEHIND A ROOF BOLTER WHEN THE TRAILING CABLE MOVED, STRIKING HIM IN THE LOWER LEGS AND CAUSING HIM TO FALL. HE FELL ON HIS LEFT ARM. HE SUFFERED A FRACTURE OF HIS LEFT FOREARM.
