Employee was getting ready for shift performing daily maintenance on D11N Dozer. He was carrying a 5 gallon oil can and slipped on dozer tracks causing him to try to catch himself without falling. He skinned his shin and pulled muscle or injury in neck/shoulder.

Mostrando los 21Employee was stepping down from Hitachi Excavator by use of the ladder. He stepped onto the ground into a hole/uneven surface and fell down catching himself with his left arm causing contusion and pain to left shoulder.
Hydraulic hose blew. Fire started under cab. Fires spread to rest of loader. Burnt beyond repair.
EE stepped into a hole at mine site that was covered by snow. He twisted his ankle and hurt ankle and lower back.

D 11 N Dozer fire started under floor boards of the cab. Our men put the fire out in approx. 7-8 minutes. Fire re-ignited and the fire dept. had arrived and got the fire out in approx. 30-40 minutes.
Operator fell off top of operator's cab of drill onto his back. He fell approx 8' to the ground. He was assisting mechanic, as steel had come out and they were attempting to push back in . EE has broken rib and lung was punctured. Also broken shoulder.
We believe starter area of dozer shorted out. Cab burnt, back part of engine compartment, and pumps below cab burnt. Burnt beyond repair.
Employee was working on loader. He was grinding out cracks on bolt holes on front rear end. He was wearing safety glasses. Piece of foreign matter (metal) got into eye.
Employee was installing a universal joint and pounding with a hammer. He had no symptoms until pupil became enlarged. Dr. found piece of metal behind eye. Will be removed in two weeks.
Operating rock truck, he bounced & hit door with shoulder.
Employee was helping drill operator change bit. Chain slipped and cable bushing came down pinching left fore finger.

Employee was running rock truck. Door flew open & he fell out of truck landing on ground. Laid there until other operator realized he was not in rock truck.
Dozer operator was in pit when dozer caught fire, causing damage to belly pan. Reason of fire is undetermined.
Cab burnt and destroyed - cause undetermined (excessive heat on combustible material). Steering line blew hole and oil hit turbo which was hot.
Truck driver & mechanic were attempting to set engine in D11 Dozer with boom truck. Outrigger slipped off block, engine shifted pinning both legs between dozer & engine.
TRIPPED AND FELL ON AN AIR CLEANER WHILE WALKING ON THE DECK OF 785. TRIPPED ON ONE AND FELL ON A SECOND AIR CLEANER.
