While installing a guard cover on the #1 shaker motor, the cover dislodged pinching the miner's left hand. The miner pulled their hand from the pinch point revealing cuts and bruising to their pointer, middle, and ring fingers. Miner went to urgent care where the cuts were cleaned and glued shut. Miner then returned to shift.

Mostrando los 23EE was walking from secondary shaker to the tool room when EE began to feel weak and thought EE might pass out. EE was told to rest in the office with AC. EE continued to feel worse and was taken to Urgent Care where EE was treated with an IV for dehydration. EE was then sent home to rest until 9/2/2025.
Employee was moving a mobile crusher when, after hearing a pop from the tracks, a sliver of metal struck them in left cheek causing them to bleed. Witness quickly applied a nearby rag and drove them to the hospital where the metal was removed, the wound was cleaned, and stitches were applied.
While performing maintenance on a WA500 Loader, employee's wrench slipped off a drain plug, causing them to twist their back and pinch a nerve. They declined immediate medical attention but visited a doctor the following Monday. Employee was prescribed a muscle relaxer and is currently off duty recovering
Miner removed hard hat to check for a problem on main stacker feed belt when hard hat would not fit, when getting out, miner stood and hit EE's head on channel iron. Another EE checked cut and recommended urgent care. 4 staples where used to close wound. No duty restrictions recommended by medical professionals.
While lifting screens to the top of shaker #2 by Skytrack Forklift, Operator lost communication with EE and continued forward. EE became pinned between the forklift and load, and the headbox resulting in injury to the face and hands as EE attempted to stop the motion. Operator was told to back up and EE was unpinned and immediate first aid was applied.
Employee was closing cab door to the equipment when a gust of wind closed the door hard squeezing EE's left thumb in the door.
Employee was helping mechanic when EE began cramping. After attempting to cool employee down, employee was brought to urgent care. IV was administered by medical professionals. Diagnosis was de-hydration and over-heating.
A driver rear ended the haul unit which the injured employee was in. The driver & IE were taken to the doctor for drug screening and evaluation. IE received an anti inflammatory shot and prescribed muscle relaxers and pain killers. IE was released back to work same day with restrictions.
Employee was cleaning out the crusher and attempting to free a stone with a hammer and a pry bar. A metal chip dislodged and entered in their right hip. EE was taken to the doctor who numbed them and attempted to remove the metal with a small incision of the skin and a pair of tweezers. The attempt was unsuccessful. EE later removed the metal flake on their own that night.
Employee was attempting to lift a clutch assembly onto the back of an engine and felt a "pop" in left shoulder. Employee was placed on restrictive duty after a doctor visit on 9/23/2022. Diagnostic testing was preformed and "articular surface partial tear" was diagnosed on 10/12/2022.
Employee was cleaning off scale. With hands full, employee slipped and fell, hearing an audible pop of the ankle. The injury began to swell and was later confirmed to be broken.
Employee was walking around the front of the sand screw on ground level and right foot sunk and tripped with top body weight. Received pain in right ankle.

Miner was operating a manual come-along. As he was pulling the handle, he heard a pop in his right shoulder.
Employee was clearing a blocked feeder gate with a pry bar, and as the gate was freed, his left hand fell on to the conveyor belt and a large rock dropped onto it. Employee was wearing gloves, but the impact caused pain, so he was taken to the doctor for injury determination.
Employee was using a cheater pipe and wrench to tighten bolts on dozer tracks. The pipe slipped & employee began experiencing pain in his neck the next day. He was sent to the doctor for examination when the pain he was experiencing was made known to the plant manager. Just was diagnosed as needing surgery, thus the delay in reporting.
