Mostrando los 27EE was cleaning material from under the hopper with a shovel. EE scooped up some material and turned to toss the material off the shovel and felt a pull in EE's back.
Employee was moving culvert pipe on a trailer. They were going to unload the pipe but they were wedged together. Employee picked one end of the middle pipe up and pulled their back.
EE was helping to change the tail pulley can on hopper. EE pulled real hard on the can and it slipped and caught finger between the pulley and the metal beam.

They did some blood work and said there was a problem not pertaining to EE getting too hot and told EE to follow up with family Doctor on Monday 8/1/16. Which EE did.
A haul unit was being loaded by a track hoe and a slush pond dam broke and the slush came down on top of them burying them.
A track hoe was loading a haul unit with gravel to take to wash plant and the dam on a slush pond broke and covered up both the tack hoe and the haul unit with the workers in them.
A clod had stopped up the harper and he was trying to break the clod up. He had a metal rod and was jobbing the clod and the rod slipped and he hit his hand on a metal piece on side if the harper.
EE said he had fallen at home over the weekend and had hurt his right knee. He was going up some steps to help work on a screen and fell hitting his left knee.
He was pumping oil into a bucket with a hand pump and twisted to see if it was full and felt a pain in his back it got worse as time went on.
He was helping to change the runner on gravel plant on the dredge when the jaws on the pipe wrench broke and hit him on the hand and chest.
He was walking down the belt line checking it and the belt started to slip so he started walking fast back to cut the belt off and stepped in a hole turning his ankle.

They were changing screens on the sand plant. Employee's foot sliped and he caught his leg on a piece of iron and cut it.
They were trying to get the door to the clay washer hopper opened wider. LC was using a maul to hit the door. He hit the door a couple times and raise up to hit it again and noticed blood and when he looked his finger was dangling.
Scale operator was going into scale office made a miss step and fell twisting her foot and causing her to sprang her ankle.

He was getting ready to move the Grove Crane was waiting for the air to build up.EE got on back of the grove to rip up the engine. When the air was up the operator flipped the switch to bring the outrigger in and EE's foot got caught between the body of the machine's frame and the outriggers.
They were helping to change teeth on the dragline. EE was watching while the foreman was trying to knock the teeth out and a piece of the tooth chipped off and hit the EE going into his right leg above his knee.
They were putting a face plate on a pump. He had his hand on the pump and did not realize it. When they lowered the part, it mashed his left hand and cut it. Had to have stitches.
They were unloading pipe off a trailor with a end loader and he did not turn the pipe loose and it swing back and caught his had between pipe and trailor.
He was jacking the track back on the trackhoe with a come-a-long and the chain broke on the come-a-long. The track fell on his left leg hitting him on the leg about the shin area.
We are putting up a sand plant, they were taking the speed reducer off. EE had walked around the end of the plant at the same time they took the pins out. The speed reducer flipped off and hit him in the head knocking him to the ground and landed on his legs.
Some more workers were taking a rubber sleeve off a pipe with a chain band. He was standing in front of them leaning aganist a belt winder when the sleeve came off it just hit his leg and pinned it up aginst the belt winder breaking his right leg just above the ankle.
EE was standing by the belt. a loader was pulling the belt. the chain broke and the chian hit him on. the hand breaking the index finger on his left hand. they were putting in a new belt line.
EE WAS COMING DOWN A LADDER OFF THE PLANT - WAS ON THE BOTTOM STEP & HIS FOOT SLIPPED ON A ROCK AND HIS KNEE HIT A PIECE OF STEEL ON PLANT.
