Employee stated that he was riding 3-wheeler mantrip to #6 head when he decided to cross the track in a place he says he probably wasn't supposed to cross. He stated that he had to go really fast to cross there and when he hit the track it caused his head to bump top.
Employee was carrying a H. Jack and stepped on a rail & slipped.
EE was trying to put a bottom roller in the #1 Belt when he tore a laceration in his chest where his ICD was located. He did not start missing work until 12/1/2006.
EE was in man trip, when person driving jeep got too close to rib. EE's arm got caught and pulled over his head, pulling EE out of jeep.
Employee was crawling by the bolter when rib rolled out & piece of cap coal hit hip & lower back.

Employee was riding in a mantrip. He rose to see where he was and hit his head on a steel beam.
Employee said bolt was bent & he had a hold of the bolt trying to get it out of drill pod & it kicked back on his left wrist.
Doing bolting rotation, twisted wrist and bolt came back and hit his left wrist.
Employee was hoisting a belt with a set of come-alongs when the come-alongs slipped & struck him in the face. THIS IS NOT A LOST TIME ACCIDENT, BUT SUTURES WERE REQUIRED.
Employee was struck by a rib roll while walking towards a MRS.
While tramming a scoop in #4 Entry, employee contacted a metal sign & lacerated his face. THIS IS NOT A LOST TIME ACCIDENT, BUT REQUIRED SUTURES.
EE states he was rock dusting belt line, when it caught his finger on accident on roof bolt plate, and roof bolt plate cut his finger. Note: EE did not report accident until 11/16/06, when EE said it got infected.

The slate bar that the EE was using struck him in the mouth when the material that he was pulling down broke loose.
Employee injured his back while lifting a foam filled tire out of a mantrip.
Employee injured himself while squaring a belt. The razor knife slipped & cut his leg when it cut through the belt.
EE was driving 3-wheeler to the outside with co-worker following on another 3-wheeler pulling the duster when they went thru swag. Co-worker asked injured employee to wait at the top of the swag in case he needed him to pull him up, which he did. Injured employee went to back up and the brakes went out causing him to flip the 3-wheeler.
Employee was struck by a piece of rib 3'(L) x 8"(T) while roof bolting. He did not start missing work until 7/24/2006.
Employee inadvertently cut his finger while cutting belt. THIS IS NOT A LOST TIME ACCIDENT.
Unintentional Roof Fall measuring 80'(L) x 20'(W) x 4'(T) found @ 12:30 PM on 6/7/06. It was located @ spad 299 in # 3 Entry.
EE stated he was going out on the 4-wheeler transport and hit a bump, hurting tailbone and lower back.
Bridge operators were going up swag & making right hand turn & they slid into 4x4 posts that turn belts on/off causing 4x4 to fall into EE's left shoulder.
Employee injured his back while sliding an 8' timber around in a scoop bucket. He was preparing to saw the timber. He did not start missing work until 6/14/2006.
Unloading overcasts off of track car which were stacked on top of one another; the overcasts were getting stuck by the bolts connecting them & they were sliding them before lifting when his helper pushed at the wrong time casing a pair of over- casts to fall on EE's foot.
EE alleges to have injured his back when he moved a Heintzmann Jack. This alleged accident was not reported until the end of his shift on 5/10/2006.

Employee was bolting top when a rock measuring 2"x4"x22.5" fell & struck him on his foot(L).
Employee received fatal injuries when he became caught in the dolly of a mobile bridge system.
Employee was standing behind the roof bolter and was knocked off his feet when the other operator took up the cable. Management questions the validity of this accident. Employee notified management on 4/13/2006, that the reason he'd missed work was due to this accident.
While bursting rock with a pick hammer, employee struck his hand(L) with the hammer.

Ee was shoveling wider the belt in mud and twisted his back the wrong way as he was shoveling mud onto the belt.
