Mostrando los 9Miner was washing the pins to disengage the bucket of skid steer. The bucket disengaged and fell on EE's right foot
Employees were placing a 200lb steel plate on a structure, the plate slipped off and fell 15" striking employee's left foot just behind the steel toe portion of their boot.
Cone crusher discharge belt stopped. Employee climbed onto belt without locking out/tagging out conveyor. Another employee energized conveyor and when it moved employee jumped off conveyor (approximately 9 ft.) and broke leg due to the fall.
EE went to tower 1 to check clips on conveyor belt. Ee grabbed ladder at hinge point, when ee placed weight on the ladder, it smashed/pinched left pinky, ring & middle fingers. Resulting in the amputation of middle finger, just below the nail. EE recalls pulling hand out, from the hinge point, which ee thinks resulted in the separation of middle finger.
Employee was dismounting excavator and slipped straining shoulder.
2 ee were cutting a section of conveyor belt w/a straight edge & a razor knife. 1 ee was holding the straight edge while the other was cutting the belt on the ground when the knife slipped & cut the right hand of the employee holding the straight edge. Resulting in a 1" laceration. It was determined that the employee needs medical attention & was taken to Prompt Med
Reinstalling screen panels (punch plate) on tower 1 screen. While installing last panel on the top deck employee had his hand between a rivet and the hook rail on the screen panel catching the end of his left thumb when the panel was pushed into place. This caused a laceration to the left thumb requiring 3 stitches.
EE was digging frozen material out of a feed chute that transfers rock from an overhead surge bin onto a belt feeder. He reached into chute to remove piece of rock, placing his arm under opening of bin. Piece of frozen lime fell/rolled from side of bin, striking his arm. Quarry had rec'd apprx 4" of rain over weekend, then temps dropped, washing material onto belts & freezing it
