Injured party was caring a base for the tooth for the bucket on a track hoe. As he was preparing to put it down it slipped out of his hands and cut the palm of his right hand requiring 4 stitches. Missed no days of work.

Injured party cut eye off of a jack on a 992 loader. The eye off the jack fell and struck another piece of the loader and bounced and landed on the top of his left foot buising it.

EE had set up crane to lift bed off 777A truck Crane was set up perpendicular to the truck & 3' lower in elevation. Rigging was hooked to bed 3 times. As truck bed was swung, the crane tilted toward truck bed slightly. EE started lowering load & began retracting the boom. Crane stopped tilting, EE thought everything was fine, but the crane suddenly laid against truck bed.
Injured party left work early on 6/16/08 saying his back was hurting. When questioned about his back, he then informed us that he had hurt his back on 6/5/08 but did not tell anyone or fill out an accident report. He said he hurt his back on 6/5 while driving a 777A back dump truck hauling rock when he ran over a hole and it jarred his lower back.
Injured party was in the process of moving a pump hose when he nicked the hose with his left foot. As he did this, his right foot slipped and caused him to fall, hurting his right knee.
A fuel truck pulled up to the fuel tanks. when the driver got out he noticed smoke & fire coming out from behind cab. Called for help. Fire Dept arrived about 15 minutes later & extinguished the fire. No one was injured. There was only about 150 gal of fuel in the tank at the time of the fire.
Injured party was driving a 777C end dump truck hauling rock out of the pit. The shovel excavator that was loading him swung around to put a load in the truck bed when it hit the back of the 777C end dump truck jarring the operator.
Injured had just got through greasing a D9N dozer and as he was climbing down his foot slipped causing him to land on his left ankle spraining it.

Injured party was walking over to the mine foreman's truck when he accidentally stepped into a hole twisting his left knee.
Inj EE was washing down his service truck when the steam jenny handle busted in his hand. Steam want was a ST-1500. Do not beleive the steam jenny had anything to do with this. The handles are abused and this probably caused the want or handle to bust. EE did not miss any work.
Injured party was pushing rock with a bull dozer, when the dozer slipped off the road, jarring the injured party and hurting his lower back.
The dozer was down and several mechanics were working trying to get the track off. As the track was coming over the sprocket, one of the bushings got in a bind and a piece of metal popped off the tip of the sprocket, striking injured party under his right eye on his face.
Injured party was walking on a belt when he sliped and fell backwards striking his left shoulder. Did not miss any work until 1/24/07. This accident happened on 10/23/06. The dr did not do anything to him and he did not miss any work. Missed his 1st day of work on 1/24/07.
Injured party was trying to pull start a gasoline pump. After the second time he pulled the start cord, it did not catch as it should have, causing the injured party to strike his left hand against a piece of equipment.
EE was G.P.S. the coal pit in north end of pit when a rock came off wall and hit him. He fell back onto other rocks hitting his head, right leg and left side.
